AFFIDAVIT FOR NAME CHANGE I, Tanish Ojha, son of Mr. Mahesh Ojha, aged 19 years, residing at Ward No.21, Purani Line, Bhattar School Ke Pass, VPOः Gangashahar, Bikaner- 334401, Rajasthan do hereby solemnly affirm and declare as underः
1. That I was previously known by the name Tanishk Ojha, which is my alias name and I do hereby declare that Tanishk Ojha and Tanish Ojha are one and same person, that is myself. I wholly relinquish and abandon the use of my former name i.e.,Tanishk Ojha. I hereby declare
2. that I should be called and known by Tanish Ojha only in future. 3. That for purpose of evidencing hereinafter in my all records, decds and writings and in all proceedings, dealings and transactions be it private or government, my name i. e.,Tanish Ojha shall be used in place and instead of Tanishk Ojha.
4. That I am filing this Affidavit for my name change as I need to update my name in my aadhar card bearing numberः 9643 1552 6204 from Tanishk Ojha to Tanish Ojha, so that my new name i.e., Tanish Ojha is reflected in all my records.
5. I hereby state that whatever is stated herein above are true to the best of my knowledge.
