This is for general information that I, Shishpal Singh S/o Bhopal Singh, R/o VPO Sawai Bari tehsil- Sardarshahar Distt- Churu, Rajasthan, India Pin code-331403, declare that my name in Aadhar No. 2147 0310 4941 issued by UIDAI is Shishpal Singh age 77 years. Now I have changed my name and shall hereafter be known as Richhapal Singh (रिछपाल सिंह).
If in future any dispute arises regarding my previous name Shishpal Singh and my new name Richhapal Singh (रिछपाल सिंह).
It is certified that I have complied with other legal requirement in this connection.
