AFFIDAVIT
I, Ronak Jindal S/o Shambhu Kumar, Age 29 years, R/O B 39 Bank Colony Balita Road Kota, Dist Kota - 324008 (Raj.) do hereby solemnly affirm and declare as under :-
1. That I am citizen of India. 2. That in various documents my Daughter name is mentioned as Aadhya Jindal (old name).
3. That Aadhya Jindal and AYANSHI JAIN both are my Daughter name and same Person.
4. That my Daughter correct name is AYANSHI JAIN and I, therefore, request to the concerned authorities to may please update and rectify name as AYANSHI JAIN.
5. That my above statements are true and correct.
