I, Nitin Jain S/o Surendra Kumar Jain, Age 32 years, R/O 68 Naya Katla Rampura Kota Dist Kota -324006 (Raj.) do hereby solemnly affirm and declare as under :-
1. That I am citizen of India.
2. That in various documents my son name is mentioned as Parth Jain (old name).
3. That Parth Jain and Dhruv Jain both are my son name and same person.
4. That my son correct name is Dhruv Jain and I, there fore, request to the concerned authorities to may please update and rectify name as Dhruv Jain.
5. That my above statements are true and correct.
