NOTIFICATIONS BY GOVERNMENT

HEALTH AND FAMILY WELFARE DEPARTMENT

TAMIL NADU CLINICAL ESTABLISHMENTS (REGULATIONS) RULES, 2018.

In exercise of the powers conferred by sub-section (1) of section 14 of the Tamil Nadu Clinical Establishments (Regulation) Act, 1997 (Tamil Nadu Act 4 of 1997), the Governor of Tamil Nadu hereby makes the following Rules, namely: -

RULES

1. Short title.- These rules may be called the Tamil Nadu Clinical Establishments (Regulation) Rules, 2018.

2. Definitions.- (1) In these rules, unless the context otherwise requires,-

(a) "Act" means the Tamil Nadu Clinical Establishments (Regulation ) Act, 1997 (Tamil Nadu Act 4 of 1997);

(b) "Advisory Committee" means the Committee constituted at State Level under clause (1) of section 2-A of the Act;

(c) "clinic" means a place where treatment for illness is offered without or with injections, minor operations, dressing etc., to the patients for observation. It may include endoscopic clinic, AYUSH Clinic or any other establishment offering treatment for illnesses by use of Medicines or any therapy under Allopathy, Siddha,Unani, Ayurveda, Yoga and Naturopathy or Homeopathy (AYUSH) or any other Government recognised systems of Medicine,;

(d) "clinical laboratory" means a place where bio-medical or bio-chemical or clinical pathology or biopsy or bacteriological or genetic investigations or any diagnostic tests or investigative services are carried out;

(e) "consulting room" means a place where consultation, which includes examination of the patients and issue of prescription is done and advice is given;

(f) "Dental clinic" means a clinic where treatment for dental ailments is given;

(g) "Dental hospital" means a place where patients are treated as inpatient and treatment is given for dental ailments;

(i) "Doctor" means and includes a Registered Medical Practitioner offering consultations or treatment under Allopathy or AYUSH;

(j) "Form" means a Form specifi ed in Annexure-II to these rules;

(k) "hospital" means and includes a Nursing Home or Health Centre or Treatment Centre or any other place where facilities for admission as in-patients for treatment of illness with or without surgery or conduct of delivery etc., with or without out-patient facilities and diagnostic facilities like laboratory etc., in any recognised system of medicine;

(l) "maternity hospital" means a hospital where delivery and other maternity services with or without surgeries and Gynaecological services including surgeries like Hysterectomy, Ceasarean operations are undertaken.

(m) "patient" means a person who reports himself or brought to any hospital including a dental hospital for treatment or consultation or seeking any other services rendered by the hospital;

(n) "polyclinic" means a clinic where more than one doctor offers consultation with or without treatment for illness in any recognised system of medicine;

(o) "Registered Medical Practitioner" means a person who possess any of the Government recognised medical qualifi cation and who has been enrolled in the register of the respective Council viz., Medical, Dental, Siddha, Ayurveda, Unani or Homeopathic Councils or the Board of Indian Medicine or any such Council, Board or any other statutory body recognised by the Government of Tamil Nadu;

(p) "Staff Nurse" means a person who possess the required qualifi cation from any of the Nursing Teaching Institutions, recognised by the Government of Tamil Nadu and that person must have enrolled in the Tamil Nadu Nurses and Midwives Council under the Indian Nursing Council Act, 1947 (Central Act XLVIII of 1947) or any of the State Nursing Council in India recognised by the Indian Nursing Council and those who have completed the Diploma in Integrated Nursing in Indian System of Medicine Colleges and registered in the Directorate of Indian Medicine for Ayurveda, Yoga and Naturopathy, Unani, Siddha and Homoeopathy establishments.

(2) Words and expressions used in these rules and not defi ned shall have the same meaning assigned to them in the Act.

3. Procedure, Quorum and Minutes of State Level Advisory Committee meeting.- (1) Every notice, calling for meeting of the State Level Advisory Committee shall,-

(a) specify the place, date and hour of the meeting;

(b) be served upon every member of the State Level Advisory Committee not less than seven clear days before the day appointed for the meeting.

(2) The Chairperson shall prepare and circulate to the members along with the notice of the meeting, an agenda for such meeting showing the business to be transacted.

(3) The quorum for the meeting shall be not less than four. If within half-an-hour of the time appointed for the meeting, quorum is not present, the meeting shall be adjourned to the same day in the following week at the same time and place and the presiding offi cer of such meeting shall inform this to the members present, and the notice to the other members shall be communicated by speed post.

(4) The minutes of the meeting shall be communicated to the Government .

4. Procedure, Quorum and Minutes of District Committee meeting.- (1) Every notice, calling for meeting of the District Committee shall,--

(a) specify the place, date and hour of the meeting.

(b) be served upon every member of the District Committee not less than seven clear days before the day appointed for the meeting.

(2) The chairperson shall prepare and circulate to the members along with the notice of the meeting, an agenda for such meeting showing the business to be transacted.

(3) The quorum for the meeting shall be not less than four. If within half-an-hour of the time appointed for the meeting, quorum is not present, the meeting shall be adjourned to the same day in the following week at the same time and place andTAMIL NADU GOVERNMENT GAZETTE EXTRAORDINARY 3 the presiding offi cer of such meeting shall inform this to the members present and the notice to the other members shall be communicated by speed post.

(4) The minutes of the meeting shall be communicated to the Government and the Competent Authority.

5. Duties of District Committee.- In addition to the duties and responsibilities specifi ed in the Act, the District Committee shall also perform the following duties, namely.-

(1) scrutinise the applications received as directed by the competent authority;

(2) inspect clinical establishment under the direction of the competent authority;

(3) examine the complaints, if any received pertaining to the implementation of the Act and refer the same to the Government through the competent authority.

(4) any other duties as may be directed by the competent authority or by the Government from time to time.

6. Minimum facilities of a clinical establishment.- The fl oor space and other facilities, the minimum number of staff and their minimum qualifi cation, the minimum equipment and other conditions required for a clinical establishment of different systems, for providing different medical services including specialized services shall be in accordance with the norms and conditions specifi ed in Annexure-I to these rules.

7. Application for registration.- (1) Every application for registration of a clinical establishment shall be made, either in person or by Registered Post with acknowledgement due to the competent authority in Form-I, accompanied by a fee for a sum of rupees fi ve thousand in the form of demand draft or treasury challan drawn in favour of the competent authority.

(2) If a clinical establishment is offering services in more than one recognised system of medicine separate application shall be made for each such system of medicine.

8. Certificate of registration.- On receipt of an application for registration of a clinical establishment, the competent authority shall, after satisfying itself that the applicant fulfi ls all the requirements of the Act and these rules, grant a Certifi cate in Form-II within one eighty days from the date of receipt of the application. The Certifi cate of Registration is non-transferable.

9. Time limit for renewal.- Every application for renewal of Certifi cate of Registration shall be submitted to the competent authority ninety days before the expiry of its validity.

10. Duplicate certificate.- An application for issue of a duplicate Certifi cate of Registration shall be made to the competent authority accompanied by a fee for a sum of rupees three hundred.

11. Duties of clinical establishment.- In addition to the duties and responsibilities specifi ed in the Act, every clinical establishment shall,-

(a) display a copy of the Certifi cate of Registration in a prominent place of the premises open to the public;

(b) maintain records in electronic form showing the names, addresses and the qualifi cations of its employees and the equipments maintained by it;

(c) record and preserve all changes in the employment of the staff and the equipments and intimate the same to the competent authority;

(d) maintain clinical records (i.e any paper, fi lm, printout, slide, solution, medium which can be deciphered or used to indicate and diagnose the condition of the human body or a part of it or any material taken out of it and the course of treatment administered to or undergone by the person), of its activities relating to a patient.

(e) keep every record open for inspection by the competent authority;

(f) surrender the Certifi cate of Registration and apply for fresh registration on change of ownership of the clinical establishment, on change of system of medicine for treatment;

(g) surrender the Certifi cate of Registration, on ceasing to function as a clinical establishment.

12. Maintenance of medical records.- Every clinical establishment shall maintain records with particulars relating to the clinical observation, test, investigation, diagnostic opinion advice and treatment given to the person, who has visited the hospital either as an in-patient or out-patient in Form III.

13. Annual Publication of list of clinical establishments.- The competent authority shall, during the month of January of every year, publish the list of clinical establishments in the Tamil Nadu Government Gazette in Form IV.

14. Appeal.- (1) An appeal against an order of the competent authority rejecting an application for registration under sub- section (2) of section 4 or an order of suspension or cancellation of registration under sub-section (2) of section 5 or any direction of the competent authority under section 6, shall lie to the Director of Medical and Rural Health Services, within thirty days from the date of receipt of that order or direction.

(2) The Director of Medical and Rural Health Services on receipt of such appeal, if he considers that, an interim order is necessary in the matter, he may pass such an order.

(3) The Director of Medical and Rural Health Services may within sixty days from the date of receipt of such appeal, after giving a reasonable opportunity of being heard, pass such order, as he deems fi t.

(4) The decision of the appellate authority shall be communicated to the person concerned within seven days from the date of passing of such order.

(5) The decision of the appellate authority shall be fi nal and binding.

ANNEXURE-I

(see rule 6)

A. ALLOPATHY SYSTEM

I. CONSULTING ROOM.-

(1) Infrastructure.- Consulting room shall have a space of not less than 100 square feet with suffi cient light and ventilation. Separate space for the patients waiting for consultation have also to be provided.

(2) Staff.- The examination of the patient and prescription of the treatment shall be done only by a registered medical practitioner as required under the Indian Medical Council Act.

(3) Equipment.- The following equipment and hospital accessories shall be available in the consulting room, namely:-

(a) Examination couch

(b) Blood Pressure Apparatus

(c) Stethoscope

(d) Torch light

(e) Tongue depressor

(f) Thermometer

(g) Weighing Machine for Adult and Paediatrics

(h) Knee hammer

(i) Measuring tape

(j) Special examination trays like proctoscopy, Pelvic Exam Tray(P.V. Tray), Eye testing screen and Ear Nose Throat examination trays as the case may be, and

(k) Besides screen or suffi cient privacy for examination of the patient;

(4) Records.- Every Clinical Establishment shall maintain a register having the details regarding the name, age, sex and address of the patient, date of examination, date of admission. The patient shall be given a slip containing name, age, sex and date of consultation, diagnosis (either provisional or fi nal) and treatment advised, the investigation result shall be entered, if it was undertaken.

(5) Prescription slip.- The Doctor shall sign the prescription slip with date and seal.

II. CLINIC.-

In addition to the norms prescribed for a consulting room, the following norms are required to be fulfi lled, namely:-

(1) Building.- Suffi cient space for two cots shall be available for keeping the patient under observation in case of anaphylaxis or for giving intravenous drip or for observation etc.,

(2) Staff.- Minimum of a Doctor and a paramedical staff.

(3) Equipments.- (a) Steriliser or autoclave;

(b) Arrangement for getting autoclaved or sterilised syringes / instruments or shall use only disposable syringes;

(c) Sterile Gloves;

(d) Cheetles Forceps;

(e) Scissors surgical;

(f) Adhesive plaster;

(g) Suturing trays with equipment;

(h) Kidney trays;

(i) Surgical equipments for abscess Incision and Drainage etc.;and

(j) Drugs.

(4) Waste Disposal.- Facilities to segregate the waste, dressings, etc., as per the Government of India and Tamil Nadu Pollution Control Board norms.

(5) Dispensation of Medicine.- If the clinic is also engaged in dispensing of medicines to the patients, the same shall be done only by a qualifi ed pharmacist or by the doctor of the clinic or under his direct supervision.

(6) Laboratory in a Clinic.- If the clinic is providing laboratory services, the laboratory work shall be done by a qualifi ed Laboratory Technician who holds a Diploma in Medical Lab Technician / Certifi cate in Medical Lab Technician course approved by the Government or alternately the Medical Offi cer can get the tests done by trained personnel under his direct supervision. However, urine examination for albumin sugar and blood haemoglobin can be undertaken by a trained person.

(7) X-ray (radiograph).- If the clinic is having its own X-ray unit, the X-rays shall be taken by a qualifi ed radiographer who holds a Clinical Research Associate (CRA) certifi cate recognized by the Government.

(8) Minor Surgeries.- In case of minor surgeries like cyst excision, Medical Termination of Pregnancy (MTP), Dilation and Curettage (D&C) are under taken it is advisable to have a clean room for the purpose and facilities available as given for minor Operation Theatre shall be provided.

III. POLYCLINIC.-

(1) In addition to the norms prescribed for clinic and consulting room, separate cubicles shall be made available to each specialities, if they attend simultaneously.

(2) Waiting room and provision of safe drinking water shall be made available for the patients. Names of different doctors attending shall be exhibited in the waiting room.

IV. HOSPITAL.-

(1) Infrastructure.- In addition to the norms prescribed for Clinic, Consulting room and Polyclinic, the waiting room shall be made available for the patients which shall have suffi cient space to cater for the number of out patients visiting the hospital.

(2) Wards or in-patient rooms clusters.- (a) Space.- (i) Wards or in-patient rooms shall be spacious with good ventilation and light. Electric fan shall be provided to each room and one fan for at least two beds shall be provided for common rooms.

(ii) Toilets may be common or separate, but shall be clean and provided with water.

(iii) Cleaning arrangements shall be made to swab the wards daily with antiseptic lotion.

(iv) A duty room shall be available for every twenty rooms with necessary Equipment like Oxygen, Saline Stand etc., (b) Staff.- (i) Doctors.-

- One qualifi ed doctor for every thirty beds

- One doctor shall be on call duty up to thirty beds during night and off duty hours.

- One duty doctor shall be available in the hospital physically for thirty beds and above in general side for all the 24 hours.

- If the hospital is having less than thirty beds, but if an Intensive Care Unit is functioning, then, one doctor shall be available on duty for 24 hours.

(ii) Staff Nurses.- One qualifi ed nurse and two trained nursing assistants shall be available for up to thirty beds in a hospital and then for each additional thirty beds one qualifi ed nurse shall be available.

The aforesaid human resource is the minimum requirement irrespective of the fact the availability of any number of trained personnel working in the hospital.

(iii) Nursing Assistant.- Male and female nursing assistants shall be engaged in suffi cient number depending on the bed strength to assist the doctors and nurses in their duties like dressings, enema and preparation of patients for surgery, delivery etc. but they shall not be given the task of giving injections or doing suturing etc. they shall work under the qualifi ed nurse.

(iv) Cleaning Services .- Suffi cient number of cleaning personnel (sweeper/sanitary worker) shall be available in the nursing home / hospital. At least one sanitary worker per twenty beds is the minimum requirement.

(3) Other Services.-

(a) Pharmacy.- If a pharmacy is maintained in the hospital for dispensing medicines to the patients, it shall be done by a registered qualifi ed pharmacist only and he should not prescribe any drugs.

(b) Laboratory.- If a Laboratory is maintained by the hospital, other than urine albumin, sugar, blood haemoglobin, all other Laboratory Tests shall be performed by a qualifi ed Laboratory Technician only who holds a minimum of Diploma in Medical Lab Technician / Certifi cate in Medical Lab Technician.

(c) X-Ray (Radiograph).- X-Ray department shall confi rm to the norms prescribed and X-Rays shall be taken by a qualifi ed radiographer with a Clinical Research Associate (CRA) qualifi cation.

(d) Scan.- Ultra sonogram test shall be performed by a Doctor only.

(e) Catering and Canteen.- If catering arrangements are available, the Kitchen shall be clean and the cooks shall be periodically medically examined. Clean practices shall be carried out in the canteen and kitchen.

(f) Blood Bank.- If a blood bank is maintained by a hospital, it shall confi rm to the directions issued by the Government of India from time to time.

(g) Ambulance.- It is desirable that, all Hospitals having more than hundred beds shall have an Ambulance with a driver available or alternatively they shall have tie up arrangement with another Hospital having Ambulance or some Ambulance service unit.

(h) General facilities.-

(i) Waste disposal.- Waste disposal shall be as per the Government of India norms and Guidelines issued by Pollution Control Board. Proper segregation of the waste at the point is to be ensured.

(ii) Record Keeping.- (a) A separate record shall be maintained in the form of a register, of all surgical operations, performed in the clinical establishment, which include, among others, the name of the medical practitioner who performed the operation and the name of the anaesthetist in attendance. All patients either discharged or referred to other Hospitals shall be given a discharge advice card / slip.

(b) A medical case record shall be maintained for a period of not less than ten years in the form of a register in the clinical establishment in respect of each patient, which shall include the following particulars, namely:-

(i) a detailed daily statement of the patient's health and condition.

(ii) details of any investigations made, surgical operations carried out and treatment given and (iii) records of all medical prescriptions, X-Ray reports, laboratory reports or any other report with full name, qualifi cations of the doctor.

(iv) in the case of a maternity home or a maternity case, the said person shall keep a case record of each child born to a patient. Full and detail record of pregnancy must be maintained for at least a period of ten years (iii) Medico Legal Cases.- Every Hospital which treats medico legal cases in accordance with Government Order, all such records and registers pertaining to such cases shall be preserved as per the Government orders from time to time and police shall be intimated regarding their admission, discharge, death etc., (4) Mortuary room.- An hospital having more than hundred beds shall have a mortuary room or room with freezer box facilities for the dead bodies.

(5) Water Supply.- Potable water supply shall be made available. Hot water supply also to be provided.

(6) Clothing and Linen.- Bed sheets and other linen used for the patients shall be changed daily.

(7) Fire Fighting.- Fire fi ghting equipments shall be installed as per rules in force. Only ISI marked equipments to be provided. Adequate precautions against the risk of fi re, including provisions for fi re escape, fi re extinguishers and system of giving warnings and for evacuation of all persons in nursing home or clinic or hospital shall be taken in the event of fi re.

(8) Quality Assurance.- The names of the doctors working in the hospital and the facilities made available in the hospital shall be written and exhibited in the hospital. The Hospital shall strive to maintain quality in each of it with the patients.

(9) Communication.- Telephone shall be made available for the communication of the patients with the relative and for referrals.

(10) Security.- Due protective measures shall be under taken by the Hospital to ensure safety of the patients, their properties and to prevent child lifting etc., Every Clinical establishment must exhibit relevant provisions of the Tamil Nadu Medicare ServiceTAMIL NADU GOVERNMENT GAZETTE EXTRAORDINARY 7 Persons and Medicare Service Institutions (Prevention of Violence and Damage or Loss to Property) Act, 2008 (Tamil Nadu Act 48 of 2008) in conspicuous places.

(11) Hospital accessories.- Suffi cient number of wheel chairs, trolleys, and stretchers shall be made available in the hospital.

(12) Generator.- A generator shall be provided to maintain essential services during power failure.

(13) Labour Room.- If the Hospital is maintaining a labour room and is conducting deliveries it shall have the following norms, namely:-

(a) Building.- There shall be a room for preparation of the patient for delivery, giving enema etc., with suffi cient water supply. The Labour room shall be not less than 120 square feet well ventilated with suffi cient light. Space for keeping the newborn baby and for its resuscitation is essential.

(b) Staff.- Labour shall be conducted by a qualifi ed Doctor, Staff Nurse, Auxiliary Nurse Midwife or a trained dais. Untrained persons shall not be entrusted with the Job. Even if a Staff Nurse / Auxillary Nurse Midwife conduct the delivery, a doctor shall be on call for any emergency or newborn resuscitation. If the labour ward is in a maternity hospital and deliveries are taking place regularly, a duty doctor shall be available for all the 24 hours. It is desirable to have a Paediatrician on call, wherever possible.

(c) Equipment.- The following equipments and hospital accessories shall be made available, namely:-

(i) Labour Board with lithotomic stand

(ii) Saline Stand

(iii) Oxygen Cylinder with Vent mask

(iv) P.V. Tray with equipment

(v) Dilation & Curettage (C & A) Set

(vi) Focus Lamp

(vii Trolleys for instrument and to receive the baby

(viii) Vaccine extractor / Forceps delivery set

(ix) Sterile core clamp

(x) Antiseptic lotions

(xi) Suction apparatus

(xii) Baby resuscitation set

(xiii) Baby Warmer (a light may also do)

(xiv) Baby mucus sucker (disposable)

(xv) Paediatric ambu bag

(xvi) Suturing tray for episiotomy and suturing.

(d) Drugs.-

- Injection Methergin 5 adenosine monophosphate

- Injection Pitocin 5 adenosine monophosphate

- Injection Carboprost 5 adenosine monophosphate

- I.V. Fluids 5% dextrose 2 bottles.

- 5% dextrose or Saline

- Ringer lactate.

- Inj.Diazepam 2 amp

- 2% Lignocane Vial – 1

(e) Records. -

(i) Birth Register

(iv) Register for babies and mothers to prevent child changing.

(14) Operation Theatre.-

(a) Minor Operation Theatre.-

Where septic cases and minor surgeries like abscess, Incision and Drainage (I&D) and septic cases like amputations etc., are carried out.

(i) Space.- Suffi cient space – not less than 100 square feet with adequate lighting and ventilation.

(ii) Staff.- All surgeries in minor operation theatre shall be carried out by qualifi ed doctor only. However dressings can be done by a Nurse or a Nursing Assistant.

(iii) Equipments.- Operation table.

- Focus light

- Sterilizer

- Auto clave

- Bins

- Saline Stand

- Minor surgical equipments

- Regional Anaesthesia equipment

- Sterile gloves

- Sterile syringes

- Drugs as required

- Drugs for emergency tray

- Boyle's apparatus with Oxygen cylinder, Nitrous Oxide Cylinder (Desirable)

- Suction apparatus

- Baby resuscitation set

(iv) Records.- Records of all surgeries done in minor operation theatre shall be entered in a register.

(b) Main Operation Theatre.-

(i) Space.- Shall have suffi cient space with a minimum of 150 square feet is essential, well lit preferably air-conditioned. Other than the operating room, there shall be provision for patient waiting and a cot provided for postoperative recovery. Suffi cient space shall be available for autoclave and sterilization and for scrubbing. Running water from the tap shall be available in the scrub room. Ultra violet lamps shall be fi tted in the operation theatre. Suffi cient arrangements for formalisation of operation theatre. Swab for culture to be taken from operation theatre at least once a month.

(ii) Staff.- A qualifi ed nurse shall be the head of nursing staff in the Operation Theatre. Anaesthesia to be provided by a qualifi ed Anaesthetist or by a doctor only who is trained in anaesthesia. Surgeries shall be performed by qualifi ed doctors only.

(iii) Equipments.-–Operating table

- Focus Light preferably wall mounted

- Boyle's apparatus with suffi cient cylinders

- Monitoring devices like

(a) Cardiac monitor – must

(b) Pulse Oximeters – must

- Surgical equipments for surgery

- Stretcher for shifting of patient

- Anaesthesia Kit.

(iv) Records.- Record of all surgeries performed shall be available in a register duly with name, age, sex, address of patient, Surgeon, Anaesthetist and Staff Nurse also to be entered. Biopsy reports also shall be entered.

(15) Intensive Care Unit.- An Intensive Care Unit may be maintained in all Nursing Homes / Hospital having more than hundred beds. The following norms are to be maintained, namely:-

(a) Space.- A well ventilated preferably air conditioned room with suffi cient lighting is essential. If four Cots are there, a minimum space of 240 square feet is needed. Suffi cient space for keeping ventilators and monitors to be available. Space for duty doctor and nurse shall also be available. There shall be provision for attendants outside the Intensive Care Unit.

(b) Staff.- One doctor for every ten beds for day time. If there are more than ten beds, one additional doctor for every additional fi ve beds. Similar provision shall be made for night time.

(c) Nurse.- One qualifi ed nurse for up to six beds for day time. An another qualifi ed nurse for every additional three beds.

This similar arrangement shall be made for night time.

V. MATERNITY HOSPITALS.-

In addition to facilities/ Norms specifi ed for Nursing Homes / Hospitals, a labour room and an operation theatre is preferable facilities for transport of patients like an Ambulance or tie up arrangement with a Central Ambulance Service is desirable.

VI. DENTAL CLINIC.-

(1) Building.-The consulting room if it accommodates the Dental Chair also shall be having suffi cient space not less than 100 square feet. The room shall be well ventilated and shall have suffi cient lighting arrangements. Space for keeping the Sterilizer and Trays to be provided. Suffi cient space for patient waiting to be provided outside the consulting room.

(2) Staff.- Dental Surgery, Consultation and prescription shall be performed by a qualifi ed Dental Surgeon only as per the Indian Dentist Act, 1948 (Central Act XVI of 1948). Injections and administration of anaesthesia shall be done by the dental surgeon himself. X-Ray shall be taken by a qualifi ed Radiographer or by the Dentist.

VII . DENTAL HOSPITALS.-

Out-patient department.- The outpatient department of a dental hospital shall confi rm to the norms prescribed to a Dental Clinic. In addition, suffi cient waiting room shall be provided to the patients. Wards, Operation Theatre and other aspects shall be as per the norms prescribed to the nursing homes and hospitals.

VIII. CLINICAL LABORATORIES.- They shall include clinical laboratories, pathological laboratories, genetic laboratories etc. (1) Building.- A minimum of space for sample collection, First Aid Room, Processing, Report preparation etc., 500 Square feet in Rural, 700-1500 Square feet in Urban should be available. There shall be lavatories for the patients use at least one shall be available with suffi cient water. There shall be suffi cient space for Microscopy and for Biochemical tests. The room shall be well ventilated with suffi cient light. Space for keeping a cot for the use of patient shall be available. If x ray is also functioning norms as for an X ray centre shall be followed. The laboratories shall ensure adequate space in relation to the following:-

(a) Patient's reception

(b) Sample collection

(c) Isolation for Bio hazardous materials

(d) Radioisotope related work as per the regulatory agency Atomic Energy Regulatory Board (AERB) requirement.

The Laboratories shall ensure standards prescribed by the National Accreditation Board for Testing and Calibration of Laboratories for the personal, qualifi cation etc., Quality assurance in Lab Service, Internal Quality and External Quality should be ensured.

(2) Staff.- (a) The Biopsy examination and reporting shall be done by a Pathologist or by a Trained Doctor. When culture and sensitivity tests are carried out, a Microbiologist either Medical or non-Medical shall be available, Cytology reports shall be given by a Pathologist only.

(b) Qualification for technical staff .- The staff performing the technical tests shall have the following qualifi cations, namely:-

(i) Graduate in Medical laboratory Technology; or

(ii) Two years Diploma course in Medical Laboratory Technology; or

(iii) One year Certifi cate course in Medical Laboratory Technology with two years experience; orTAMIL NADU GOVERNMENT GAZETTE EXTRAORDINARY 11 (iv) Graduate in science with one year experience in Medical Laboratory; or

(v) Diploma in medical radiation and radioisotope technology (DMRIT) ; or

(vi) Certifi cate in Cytotechnology by Indian Academy of Cytology for screening of ex-follative cytology in addition to the above qualifi cation for Cytotechnologist.

(c) In a genetic laboratory, all tests shall be carried out and reporting done by a qualifi ed Genetist or qualifi ed doctor only. If amniocentesis is done as permitted by law it shall be done only in a Minor Operation Theatre and all facilities as needed for a Minor Operation Theatre shall be available and it shall be done by a qualifi ed doctor under sterile conditions only.

IX. X-RAY (RADIOGRAPH) CENTRES.-

This Centre shall include all centres of Imaging, Sciences namely, Ultra Sonogram Scan Centres, Computed Tomography (C.T.) Scan Centres, Magnetic resonance imaging (MRI) Scan Centres, X-Rays (Radiograph) as well as Electro Cardiogram (ECG), Electroencephalography (EEG) and Treadmill Echocardiography.

1. Buildings.- Suffi cient space as the equipment in use demands. Suffi cient space shall be provided for patient waiting and resuscitation of the patient in case of any anaphylaxis occurring, during contrast injection or after a treadmill test patient feels like resting. X ray and imaging centres shall fulfi l the "Atomic Energy Regulatory Board" (AERB) norms

2. Staff.- All tests shall be carried out by qualifi ed technical persons only as follows:-

- Radiographer with Clinical Research Associate (CRA) Qualifi cation Ultra Sonogram - by a qualifi ed Doctor or by a Radiologist or

Concerned Specialist as per the provisions of the Pre-Conception And Pre-Natal

Magnetic resonance imaging (MRI) - Radiologist or by the Concerned Specialist

3. Equipments.- Equipments according to the concerned machine.

4. Records.- Name, age, sex, address of patient, the nature of investigation done referral doctor, and result of the investigation shall be available in a register. Number of cases treated free of charges shall also be available along with name, age, sex, address and result etc.

B. INDIAN SYSTEM OF MEDICINE AND HOMOEOPATHY

I. CONSULTING ROOM / CLINIC / POLYCLINICS

1. Building.- The Consulting room shall be spacious, well ventilated and having suffi cient light. The space shall be not less than 100 square feet. There shall be suffi cient space for waiting of the patients etc., If it is a polyclinic, different cubicles shall be available for each doctor. The names of visiting doctors and their system of medicine shall be exhibited in front of the clinic.

2. Staff.- The clinics namely, Ayurveda, Yoga and Naturopathy, Unani, Siddha and Homoepathy Clinics shall be manned by the registered Medical practitioner . If the pharmacy attached with the clinic, dispensing of medicines shall be done by a Pharmacist qualifi ed under the respective system or by the doctor himself.

3. Equipment.- (a) Diagnostic equipments ordinarily needed for all AYUSH, Indian System of Medicine and Homeopathy and Yoga and Naturopathy Clinics:

(i) Thermometer

(ii) Sphygmomanometer

(iii) Stethescope

(iv) Knee hammer

(v) Tongue Depressor

(vi) Torch/ கிரிடோஸ்கேப் லென்சுடைய டார்ச்

(vii) Weighing machine/ உயரம் பார்க்கும் அளவை

4. Drugs.-The drugs dispensed to the patients shall contain a label indicating the name of medicine and the name of patient to whom it is given and quantity to be given etc., the date of expiry shall be specifi ed in the label, if the drug has an expiry date. The drug to be given internally and the drug to be used externally shall be indicated and white and red labels to be provided respectively with clear writing as "For Internal use" or " For External Use" in vernacular.,

5. Records.- A record of all patients seen as to their name, age, sex, diagnosis and treatment shall be available. The patient shall be provided with a slip with name, age, sex, diagnosis of treatment given.

II. NURSING HOMES AND HOSPITALS.-

1. Building.- The norms prescribed for consulting room or outpatient in the Allopathy system of medicine shall be fulfi lled . It shall also have suffi cient space for patient waiting separate cubicles for each doctors to work. The names of the doctors and then Specialists and different facilities available in the hospital shall be put up in a board.

2. Wards or inpatient rooms.- Inpatient wards shall be spacious with ventilation and lighting. Electric fan shall be provided to each room and in common rooms one fan for at least four beds shall be provided. Toilets be common or separate but shall be clean and provided with water from tap. Cleaning arrangements shall be made to swab daily the wards with antiseptic lotion.

3. Staff.- (a) Doctors.- The hospital shall employ only registered medical practitioner to treat the patients. The doctor shall also be registered in the Council under his respective system of medicine or in the Council/Board for Indian System of Medicine.

(b) Nurse.- A qualifi ed nurse shall be available at the ratio of one nurse and two Nursing Assistant for upto thirty beds.

(c) Pharmacist/Therapist.- Pharmacist/Therapist qualifi ed in the respective system of medicine shall be employed for dispensing of medicines. If the hospital has more than thirty beds, for less than thirty beds the doctor himself can dispense the medicines or employ a qualifi ed pharmacist/Therapist.

(d) Other Staff.- Other staff like Mazdoor etc. shall be employed as per need. Suffi cient number of cleaning staff shall be available at least one for every thirty beds.

In addition to above, fi rst aid and emergency management need to be provided. Suffi cient number of Wheel Chairs, Stretchers shall be available at least one per every thirty beds.

7. Records.- (1) A record of all patients admitted / treated shall be available along with the investigations diagnosis and treatment. The patient shall be provided with a discharge slip containing the details of investigation, diagnosis, treatment and follow up etc.,

(2) The following register shall be maintained. It is the duty of the clinical establishment to give all the relevant particulars to the patients on demands.

(i) Admission and Discharge Register

(ii) Contagious and infectious disease register.

(iii) Medico legal cases register (in case medico legal cases are brought there for treatment at any stage) Records with full address shall be kept ready for verifi cation by the District Committee.

C. GENERAL CONDITIONS TO BE FULFILLED BY THE HOSPITALS UNDER ALLOPATHY AND AYUSH.

1. Communication.- A telephone connection shall be available for use by patients (on payment)

2. Security.- Suffi cient security shall be provided for the safety of inmates and to prevent theft.

3. Fire Fighting:- Fire fi ghting equipment with I.S.I. mark shall be provided as per rules in the hospital.

4. Kitchen.- If food is provided to inmates, the kitchen shall be clean and the cook(s) shall be periodically, medically examined for any infection or contagious diseases.

5. Clothing and Linen.- It shall be clean and changed daily.

6. Water Supply.- The potable water shall be provided to the patient.

7. Waste Disposal.- It shall be as per the Government of India norms and shall follow the guidelines of Tamil Nadu Pollution Control Board.

8. Record Maintenance.- (a) Every Clinical Establishment shall maintain the permanent records pertaining to details of the employees as well as the clinical records pertaining to the patients. The records shall be kept open for inspection by the competent authority or any other offi cer authorized in this behalf.

(b) Every Private Clinical Establishment shall make available a copy of list of observation, treatment etc., pertaining to the patient on payment of necessary charges within the reasonable time. Every Clinical Establishment shall display at a prominent place the charges for obtaining such information.

(c) Every Clinical Establishment may refuse to furnish such information, if such information is likely to cause injury to the person or his family members or if the treatment has been conducted on the direction of a public authority.

அறிவிக்கை

1997ஆம் ஆண்டு தமிழ்நாடு மருத்துவ நிறுவனங்கள் (முறைப்படுத்துதல்) சட்டத்தின் (தமிழ்நாடு சட்டம் 4/1997) 14ஆம் பிரிவின் (1)-ம் உட்பிரிவில் வழங்கப்பட்டுள்ள அதிகாரங்களைக் கொண்டு, தமிழ்நாடு ஆளுநர்அவர்கள் கீழ்க்காணும் விதிகளை ஈங்கிதனால் இயற்றுகிறார். அதாவது:

விதிகள்

1. குறுந்தலைப்பு.- இந்த விதிகள், 2018ஆம் ஆண்டு தமிழ்நாடு மருத்துவ நிறுவனங்கள் (முறைப்படுத்துதல்) விதிகள் என அழைக்கப்பெறும்.

2. பொருள் வரையறைகள்.- (1) இந்த விதிகளில், தறுவாய் வேறுபொருள் குறித்தாலன்றி, மற்றபடி

(a) "சட்டம்" என்பது, 1997ஆம் ஆண்டு தமிழ்நாடு மருத்துவ நிறுவனங்கள் (முறைப்படுத்துதல்) சட்டம் (தமிழ்நாடு சட்டம் 4/1997) என்று பொருள்படும்.;

(b) "ஆலோசனைக் குழு" என்பது, இந்தச் சட்டத்தின் பிரிவு 2-A எனும் பிரிவின் கூறு (1)இன் படி, மாநில அளவில் அமைத்துருவாக்கப்பட்ட குழு என்று பொருள்படும்.;

(c) "மருத்துவ மையம்" என்பது நோயாளிகளைக் கண்காணிப்பில் வைத்து, அவர்களுக்கு ஊசி மருந்துகள், சிறிய அளவிலான அறுவை சிகிச்சைகள், காயங்களுக்குக் கட்டு கட்டுதல் முதலியவற்றுடன் அல்லது இவை எவையுமின்றி நோய்களுக்குச் சிகிச்சையளிக்கப்படுகிற ஒரு இடம் என்று பொருள்படும். இவை உடற்குழாய் உள்நோக்கல் (எண்டோஸ்கோப்பி) மருத்துவ மையம், ஆயுஷ் (AYUSH) மருத்துவ மையம் அல்லது மருந்துகளைப் பயன்படுத்தி அல்லது ஆங்கில மருத்துவ முறை, சித்த மருத்துவ முறை, யுனானி மருத்துவ முறை, ஆயுர்வேத மருத்துவ முறை, யோகா மற்றும் இயற்கை மருத்துவ முறை அல்லது ஓமியோபதி மருத்துவ முறை (ஆயுஷ்) அல்லது அரசால் அங்கீகரிக்கப்பட்ட ஏனைய யாதொரு மருத்துவ முறையின் கீழ், நோய்களுக்கு சிகிச்சை அளிக்கும் ஏனைய யாதொரு மருத்துவ நிறுவனமும் இதில் அடங்கும்.;

(d) "மருத்துவ சிகிச்சை முறை ஆய்வகம்" (Clinical Laboratory) என்பது உயிரி மருத்துவ ஆய்வு அல்லது உயிரி வேதிப்பொருள் அல்லது நோய்க்குறியியல் அல்லது உடல் திசு ஆய்வு அல்லது நுண்ணுயிரியல் ஆய்வு அல்லது மரபியல் ஆய்வுகள் அல்லது யாதொரு நோய் கண்டறிதல் பரிசோதனைகள் அல்லது பிணி ஆய்வு முறைகள் மேற்கொள்ளப்படும் ஓர் இடம் என்று பொருள்படும்;

(e) "மருத்துவ ஆலோசனை அறை" என்பது நோயாளிகளை பரிசோதனை செய்து, அவர்களுக்கு மருந்துச்சீட்டு மற்றும் ஆலோசனை வழங்கப்படும் ஓர் இடம் என்று பொருள்படும்.;

(f) "பல் மருத்துவ நிலையம்" என்பது பல் சம்பந்தப்பட்ட நோய்களுக்கு சிகிச்சை அளிக்கும் மருத்துவ நிலையம் என்று பொருள்படும்.;

(g) "பல் மருத்துவமனை" என்பது நோயாளிகள் உள்நோயாளிகளாகக் சேர்க்கப்பட்டு பல் நோய்க்காக சிகிச்சை அளிக்கப்படும் இடம் என்று பொருள்படும்.;

(h) "மாவட்ட குழு" என்பது, இச்சட்டத்தின் பிரிவு 2-D எனும் பிரிவின் கூறு (1)-இன் கீழ், ஒவ்வொரு மாவட்டத்திலும் அமைத்துருவாக்கப்பட்ட குழு என்று பொருள்படும்.;

(i) "மருத்துவர்" என்பது ஆங்கில மருத்துவ முறை அல்லது `ஆயுஷ்' மருத்துவ முறையில் ஆலோசனை வழங்குகிற அல்லது சிகிச்சை அளிக்கிற பதிவு பெற்ற மருத்துவர் ஒருவர் என்று பொருள்படும்.;

(j) "படிவம்" என்பது இந்த விதிகளின் பிற்சேர்க்கை II-ல் இணைக்கப்பட்ட ஒரு படிவம் என்று பொருள்படும்;

(k) "மருத்துவமனை" என்பது யாதொரு அங்கீகரிக்கப்பட்ட மருத்துவ முறையில் ஆய்வுக்கூடம் முதலிய புறநோயாளி வசதிகள் மற்றும் நோய்க்குறி அறிவதற்கான வசதிகளுடன் அல்லது இந்த வசதிகளின்றி, அறுவை சிகிச்சையின்றி அல்லது அறுவை சிகிச்சையுடன் நோய்க்கான சிகிச்சை அளிக்கிற அல்லது பிரசவம் பார்ப்பதற்கு உள்நோயாளிகளாகச் சேர்ப்பதற்கான வசதிகள் கொண்ட சிறிய மருத்துவமனை அல்லது சுகாதார மையம் அல்லது சிகிச்சை மையம்/மருத்துவமனை என்று பொருள்படும்.;

(l) "மகப்பேறு மருத்துவமனை" என்பது அறுவை சிகிச்சை செய்து அல்லது அறுவை சிகிச்சையின்றி பிரசவம் நடத்துதல் மற்றும் பிற மகப்பேறு சேவைகளையும் கருப்பை நீக்கம், கருப்பை மேற்திறப்பு (Caesarean Section) மூலம் மகப்பேறுக்கான அறுவை சிகிச்சைகள் உட்பட மகளிர் நோய்க்கான மருத்துவ சேவைகளையும் வழங்குகின்ற மருத்துவமனை என்று பொருள்படும்.;

(m) "நோயாளி" என்பவர் மருத்துவமனையால் அளிக்கப்படுகிற சிகிச்சை அல்லது ஆலோசனை அல்லது ஏனைய யாதொரு மருத்துவ சேவைகளைப் பெறுவதற்காக, பல் மருத்துவமனை உட்பட யாதொரு மருத்துவமனைக்கும் தானாக அல்லது பிறரால் கொண்டு வரப்படும் நபர் என்று பொருள்படும்.;

(n) "பலநோய் மருத்துவ சிகிச்சை மையம்" என்பது அங்கீகரிக்கப்பட்ட யாதொரு மருத்துவ முறையில், சிகிச்சையுடன் அல்லது சிகிச்சையின்றி ஒன்றுக்கும் மேற்பட்ட மருத்துவர்கள் ஆலோசனை வழங்குகிற மருத்துவ மையம் எனப் பொருள்படும்.;

(o) "பதிவு பெற்ற மருத்துவர்" என்பவர், அரசு அங்கீகாரம் பெற்ற ஏதாவது மருத்துவத் தகுதி பெற்றுள்ள அது தொடர்பான அமைப்பில் அதாவது, மருத்துவம், பல் மருத்துவம், சித்தா, ஆயுர்வேதா, யுனானி மற்றும் ஓமியோபதி கழகங்கள் மற்றும் இந்திய மருத்துவ வாரியம் அல்லது தமிழ்நாடு அரசால் அங்கீகரிக்கப்பட்ட அத்தகைய யாதொரு கழகம், வாரியம் அல்லது ஏனைய யாதொரு சட்டப்படியான அமைப்பில் (Statutory) பதிவு செய்த ஒரு நபர் எனப்படுவார்.;

(p) "செவிலியர்" என்பவர், தமிழ்நாடு அரசால் அங்கீகரிக்கப்பட்ட யாதொரு செவிலிய பயிற்சி நிறுவனங்களிலிருந்து தேவையான தகுதியினை பெற்றிருந்து அந்த நபர் 1947ஆம் ஆண்டு இந்திய செவிலிய குழு சட்டத்தின் கீழ் (மத்திய சட்டம் 48/1947) தமிழ்நாடு செவிலியர் மற்றும் தாதியர் குழுமம் அல்லது இந்திய செவிலிய குழுவால் அங்கீகரிக்கப்பட்ட ஒரு மாநில செவிலியர் குழுவில் பதிவு செய்தல் மற்றும் இந்திய மருத்துவ அமைப்பு கல்லூரிகளில் ஒருங்கிணைந்த செவிலியர் பட்டய படிப்பினை முடித்து, ஆயுர்வேதா, யோகா மற்றும் இயற்கை, யுனானி, சித்தா மற்றும் ஓமியோபதி நிறுவனங்களுக்கான இந்திய மருத்துவ இயக்குநரகத்தில் பதிவு செய்துள்ள நபர் ஒருவர் என்று பொருள்படும்.;

(2) இந்த விதிகளில் பயன்படுத்தப்பட்டு ஆனால் பொருள் வரையறை செய்யப்படாத அனைத்து சொற்கள் மற்றும் சொற்றொடர்களும், சட்டத்தில் அவற்றுக்கு குறித்தளிக்கப்பட்டுள்ள அதே பொருளைக் கொண்டிருக்கும்.;

3. மாநில அளவிலான ஆலோசனைக் குழுவின் நடைமுறை, கூட்டம் நடப்பதற்கு தேவையான நிர்ணயிக்கப்பட்ட நபர்களின் குறைந்த எண்ணிக்கை மற்றும் கூட்ட நடவடிக்கை குறிப்புகள்.- (1) மாநில அளவிலான ஆலோசனைக் குழு கூட்டத்திற்கு அழைப்பு விடுக்கின்ற ஒவ்வொரு அறிவிப்பில் பின் வருபவை குறிப்பிடப்பட்டிருக்கும்,-

(a) கூட்டம் நடைபெறும் இடம், நாள் மற்றும் நேரம்.

(b) கூட்டம் நடைபெறுவதற்கு நிர்ணயிக்கப்பட்ட நாளுக்கு முன்னதாக, முழு ஏழு நாட்களுக்கு குறையாத கால அளவில், மாநில அளவிலான ஆலோசனைக் குழுவின் ஒவ்வொரு உறுப்பினருக்கும் அறிவிப்பு வழங்கப்படும்.

(2) தலைவர், கூட்டத்திற்கான அறிவிப்புடன், செயல்படுத்தப்பட வேண்டிய பணிகள் குறித்த அத்தகைய கூட்டத்திற்கான நிகழ்ச்சி நிரல் குறிப்பு ஒன்றை தயாரித்து, உறுப்பினர்களுக்கு சுற்றறிக்கையாக அனுப்புவார்.

(3) கூட்டம் நடப்பதற்கு தேவையாக நிர்ணயிக்கப்பட்ட நபர்களின் குறைந்த எண்ணிக்கை, நான்கு உறுப்பினர்களுக்கு குறையாமல் இருக்க வேண்டும். கூட்டம் நடைபெறுவதற்கு நிர்ணயிக்கப்பட்ட அரை மணி நேரத்தில், கூட்டம் நடப்பதற்கு தேவையாக நிர்ணயிக்கப்பட்ட குறைந்த எண்ணிக்கை நபர்கள் இல்லையெனில், கூட்டம், அடுத்து வருகின்ற வாரத்தில், அதே நேரம் மற்றும் இடத்தில், அதே நாளுக்கு ஒத்தி வைக்கப்படும். அத்தகைய கூட்டத்தின் தலைமை அலுவலர் அங்கு கூடியுள்ள உறுப்பினர்களிடம் இதை தெரிவிப்பார். ஏனைய உறுப்பினர்களுக்கு, விரைவு அஞ்சல் வாயிலாக தகவல் தெரிவிக்கப்படும்.

(4) கூட்ட நடவடிக்கை குறிப்புகள் அரசுக்கு தெரிவிக்கப்படும்.

4. மாவட்ட குழு கூட்டத்தின் நடைமுறை, கூட்டம் நடப்பதற்கு தேவையாக நிர்ணயிக்கப்பட்ட நபர்களின் குறைந்த எண்ணிக்கை மற்றும் கூட்ட நடவடிக்கை குறிப்புகள்.- (1) மாவட்ட அளவிலான ஆலோசனைக் குழு கூட்டத்திற்கு அழைப்பு விடுக்கின்ற ஒவ்வொரு அறிவிப்பில் பின்வருபவை குறிப்பிடப்பட்டிருக்கும்,-

(a) கூட்டம் நடைபெறும் இடம், நாள் மற்றும் நேரம்.

(b) கூட்டம் நடைபெறுவற்கு நிர்ணயிக்கப்பட்ட நாளுக்கு முன்னதாக, முழு ஏழு நாட்களுக்கு குறையாத கால அளவில், மாவட்ட அளவிலான ஆலோசனைக் குழுவின் ஒவ்வொரு உறுப்பினருக்கும் அறிவிப்பு வழங்கப்படும்.

(2) தலைவர், கூட்டத்திற்கான அறிவிப்புடன், செயல்படுத்தப்பட வேண்டிய பணிகள் குறித்த அத்தகைய கூட்டத்திற்கான நிகழ்ச்சி நிரல் குறிப்பு ஒன்றை தயாரித்து, உறுப்பினர்களுக்கு சுற்றறிக்கையாக அனுப்புவார்.

(3) கூட்டம் நடப்பதற்கு தேவையாக நிர்ணயிக்கப்பட்ட நபர்களின் குறைந்த எண்ணிக்கை, நான்கு உறுப்பினர்களுக்கு குறையாமல் இருக்க வேண்டும். கூட்டம் நடைபெறுவதற்கு நிர்ணயிக்கப்பட்ட அரை மணி நேரத்தில், கூட்டம் நடப்பதற்கு தேவையாக நிர்ணயிக்கப்பட்ட குறைந்த எண்ணிக்கை நபர்கள் இல்லையெனில், கூட்டம், அடுத்து வருகின்ற வாரத்தில், அதே நேரம் மற்றும் இடத்தில், அதே நாளுக்கு ஒத்தி வைக்கப்படும். அத்தகைய கூட்டத்தின் தலைமை அலுவலர் அங்கு கூடியுள்ள உறுப்பினர்களிடம் இதை தெரிவிப்பார். ஏனைய உறுப்பினர்களுக்கு, விரைவு அஞ்சல் வாயிலாக தகவல் தெரிவிக்கப்படும்.

(4) கூட்ட நடவடிக்கை குறிப்புகள் அரசுக்கும் தகுதிறம் வாய்ந்த அதிகார அமைப்புக்கும் தெரிவிக்கப்படும்.

5. மாவட்ட குழுவின் கடமைகள்.- இந்த சட்டத்தில் குறிப்பிடப்பட்டுள்ள பணிகள் மற்றும் பொறுப்புகளுடன், மாவட்டக் குழு கீழ்க்காணும் கடமைகளை மேற்கொள்ளும். அதாவது:-

(1) தகுதிறம் வாய்ந்த அதிகார அமைப்பால் உத்தரவிடப்பட்டவாறாக, பெறப்பட்ட விண்ணப்பங்களை கூர்ந்தாய்வு செய்தல்;

(2) தகுதிறம் வாய்ந்த அதிகாரியின் உத்தரவின் கீழ், மருத்துவ நிறுவனங்களை ஆய்வு செய்தல்;

(3) இந்த சட்டத்தை செயல்படுத்துவது தொடர்பாக ஏதேனும் புகார்கள் பெறப்பட்டிருப்பின், அவற்றை ஆய்வு செய்து, தகுதிறம் வாய்ந்த அதிகார அமைப்பின் வாயிலாக அரசின் பார்வைக்கு அனுப்புதல்;

(4) அவ்வப்போது தகுதிறம் வாய்ந்த அதிகார அமைப்பால் அல்லது அரசால் உத்தரவிடப்பட்டவாறான யாதொரு ஏனைய கடமைகள்.

6. மருத்துவ நிறுவனங்களின் குறைந்தளவு வசதிகள்.- சிறப்பு சிகிச்சை உட்பட இடவசதி மற்றும் ஏனைய வசதிகள், வெவ்வேறு மருத்துவ சேவைகளுக்கு வகை செய்வதற்கு வெவ்வேறு முறையிலான மருத்துவ சிகிச்சை நிறுவனத்திற்கு தேவைப்படும் பணியாளர்களின் குறைந்தளவு எண்ணிக்கை மற்றும் அவர்களின் குறைந்தளவு கல்வித் தகுதி, குறைந்தளவு கருவிகள் மற்றும் ஏனைய பணிநிலைமைகள், இவ்விதிகளின் இணைப்பு-I இல் குறிப்பிடப்பட்டிருக்கும் அளவுகோள்கள் (Norms) மற்றும் நிபந்தனைகளுக்கேற்ப இருக்க வேண்டும்.

7. பதிவு செய்வதற்கான விண்ணப்பம்.- (1) மருத்துவ நிறுவனம் ஒன்றை பதிவு செய்வதற்கான ஒவ்வொரு விண்ணப்பமும், தகுதிறம் வாய்ந்த அதிகார அமைப்பின் பெயரில் எடுக்கப்பட்ட ரூபாய் ஐந்தாயிரம் தொகைக்கான கேட்புக் காசோலை அல்லது கருவூல செலுத்துச் சீட்டுடன் படிவம் I-இல், ஒப்புகை சீட்டுடன், தகுதிறம் வாய்ந்த அதிகார அமைப்பிடம் நேரிடையாகவோ அல்லது பதிவஞ்சல் வாயிலாக அளிக்கப்பட வேண்டும்.

(2) ஒரு மருத்துவ நிறுவனம் ஒன்றுக்கும் மேற்பட்ட அங்கீகரிக்கப்பட்ட மருத்துவ முறையில் சேவைகளை வழங்குவதாக இருப்பின், அத்தகைய மருத்துவ முறை ஒவ்வொன்றுக்கும், தனியாக விண்ணப்பம் அளிக்க வேண்டும்.

8. பதிவுச் சான்றிதழ்.- மருத்துவ நிறுவனத்தை பதிவு செய்வதற்கான விண்ணப்பம் பெறப்பட்டவுடன், இந்த சட்டம் மற்றும் விதிகளில் தேவைப்படுகிற அனைத்தையும் விண்ணப்பதாரர் நிறைவு செய்துள்ளார் என்று தகுதிறம் வாய்ந்த அதிகார அமைப்பு மனநிறைவு அடைந்த பின்னர், விண்ணப்பம் பெறப்பட்ட நாளிலிருந்து 180 நாட்களுக்குள் படிவம் II-இல் சான்றிதழ் ஒன்றை அளிக்க வேண்டும். இந்தப் பதிவுச் சான்றிதழ் மாற்றத்தக்கதல்ல.

9. புதுப்பிப்பதற்கான காலவரையறை.- பதிவுச்சான்றிதழை புதுப்பிப்பதற்கான ஒவ்வொரு விண்ணப்பமும், அதன் செல்திறன் கால அளவு முடிவடைவதற்கு, 90 நாட்களுக்கு முன்னர், தகுதிறம் வாய்ந்த அதிகார அமைப்பிடம் அளிக்கப்பட வேண்டும்.

10. சான்றிதழ் நகல்.- பதிவுச் சான்றிதழின் நகல் வழங்குவதற்கான ஒவ்வொரு விண்ணப்பமும், முன்னூறு ரூபாய்க்கான கட்டணத் தொகையுடன் தகுதிறம் வாய்ந்த அதிகார அமைப்பிடம் அளிக்கப்பட வேண்டும்.

11. மருத்துவ நிறுவனத்தின் கடமைகள் மற்றும் பொறுப்புகள்.- இந்த சட்டத்தில் குறிப்பிடப்பட்டுள்ள கடமைகள் மற்றும் பொறுப்புகளுடன், ஒவ்வொரு மருத்துவ நிறுவனமும்,

(a) பொது மக்கள் பார்வையில் படும் இடத்தில், வளாகத்தின் முக்கிய இடத்தில் பதிவுச் சான்றிதழின் நகல் ஒன்றை பார்வைக்கு வைக்க வேண்டும்;

(b) நிறுவனத்தின் பணியாளர்களின் பெயர்கள், முகவரிகள் மற்றும் கல்வித் தகுதிகள் மற்றும் அதனால் பராமரிக்கப்பட்டு வரும் சாதனங்கள் ஆகியவற்றை கொண்ட பதிவுருக்களை மின்னணு வடிவில் பராமரிக்க வேண்டும்;

(c) அனைத்து ஆவணங்கள் மற்றும் பணியாளர்கள் வேலையில் அனைத்து மாற்றங்கள் மற்றும் சாதனங்கள் பதிவு செய்து, பாதுகாத்து வரப்படுவதுடன் இது குறித்து தகுதிறம் வாய்ந்த அதிகார அமைப்பிற்கு தெரிவிக்கப்பட வேண்டும்.;

(d) நோயாளி தொடர்பான அதன் செயல்பாடுகள் குறி்த்த மருத்துவ பதிவுருக்களை (அதாவது, மனித உடலின் அல்லது அதன் ஒரு பகுதியை அல்லது அதிலிருந்து எடுக்கப்பட்ட யாதொரு பகுதியின் நிலை கண்டறிவதற்கு பயன்படுகின்ற யாதொரு30 TAMIL NADU GOVERNMENT GAZETTE EXTRAORDINARY காகிதம், படச்சுருள் அச்சு நகல், காட்சி வில்லை, கரைசல், ஊடகம் மற்றும் நபர் ஒருவருக்கு வழங்கப்படுகிற மருத்துவச் சிகிச்சை முறை) பராமரிக்க வேண்டும்;

(e) தகுதிறம் வாய்ந்த அதிகார அமைப்பால் ஆய்வு செய்ய ஒவ்வொரு பதிவுருவும், பார்வைக்கு வைத்து வர வேண்டும்;

f) மருத்துவ நிறுவனத்தின் உடைமை உரிமை மாறுகின்ற போது அல்லது மருத்துவ சிகிச்சை முறை மாறுகின்ற போது, பதிவுச் சான்றிதழ் திரும்ப ஒப்படைப்பு செய்யப்பட்டு, புது பதிவிற்கு விண்ணப்பிக்க வேண்டும்;

g) மருத்துவ நிறுவனமாக செயல்படுவது அற்றுப் போயின், பதிவுச் சான்றிதழை திரும்ப ஒப்படைப்பு செய்யப்பட வேண்டும்.

12. மருத்துவ பதிவுருக்களை பராமரித்து வருதல்.- ஒவ்வொரு மருத்துவ நிறுவனமும், படிவம் III-இல் மருத்துவமனைக்கு உள்நோயாளியாக அல்லது வெளி நோயாளியாக வருகிற நபர்கள் தொடர்பான மருத்துவ கண்காணிப்பு, பரிசோதனை, ஆய்வு, நோயைக் கண்டறிந்து கருத்து தெரிவித்தல், ஆலோசனை மற்றும் வழங்கப்பட்ட சிகிச்சை ஆகியவை தொடர்பான விவரங்கள் அடங்கிய பதிவுருக்களை பராமரித்து வர வேண்டும்.

13. மருத்துவ நிறுவனங்களின் பட்டியலை ஆண்டுதோறும் வெளியிடுதல்.- தகுதிறம் வாய்ந்த அதிகார அமைப்பு, ஒவ்வொரு ஆண்டும் ஜனவரி மாதத்தில், படிவம் IV-இல் மருத்துவ நிறுவனங்களின் பட்டியலை, தமிழ்நாடு அரசிதழில் வெளியிடுதல் வேண்டும்.

14. மேல்முறையீடு.- (1) தகுதிறம் வாய்ந்த அதிகார அமைப்பு பிரிவு (4) உட்பிரிவு (2)ன் கீழ் பதிவு செய்வதற்கான விண்ணப்பத்தை நிராகரித்தல் அல்லது பிரிவு (5) உட்பிரிவின்(2) கீழ் பதிவினை தற்காலிகமாக நிறுத்தி வைத்தல் அல்லது பதிவினை ரத்து செய்யும் ஆணை அல்லது பிரிவு 6ன் கீழ் தகுதிறம் வாய்ந்த அதிகார அமைப்பு யாதொரு கட்டளையை எதிர்த்து, மேல் முறையீடு ஒன்று, அந்த ஆணை அல்லது கட்டளை பெறப்பட்ட நாளிலிருந்து முப்பது நாட்களுக்குள் மருத்துவ மற்றும் ஊரக நலப் பணிகள் இயக்குநரிடம் அளிக்கப்பட வேண்டும்.

(2) அத்தகைய மேல்முறையீடு பெறப்பட்டவுடன், மருத்துவ மற்றும் ஊரக நலப் பணிகள் இயக்குநர், இந்த விவகாரத்தில் இடைக்கால ஆணை தேவை என்று கருதினால், அத்தகைய ஆணையை அவர் பிறப்பிக்கலாம்.

(3) மருத்துவம் மற்றும் ஊரக நலப் பணிகள் இயக்குநர், மேல்முறையீடு பெற்ற நாளிலிருந்து 60 நாட்களுக்குள், விசாரணை செய்வதற்கு நியாயமான வாய்ப்பினை அளித்த பின்னர், பொருத்தமானது எனக் கருதப்படக்கூடிய ஆணையை அவர் பிறப்பிக்கலாம்.

(4) அத்தகைய ஆணை பிறப்பிக்கப்பட்ட நாளிலிருந்து ஏழு நாட்களுக்குள் சம்பந்தப்பட்ட நபருக்கு, மேல்முறையீட்டு அதிகாரியின் முடிவு தெரிவிக்கப்பட வேண்டும்.

(5) மேல் முறையீட்டு அதிகாரியின் முடிவு இறுதியானது மற்றும் கட்டுப்படுத்துவதாகும்.

[விதி 6 ஐக் காண்க]

அ. ஆங்கில மருத்துவ முறை

I மருத்துவ ஆலோசனை அறை.-

(1) உள்கட்டமைப்பு.- மருத்துவ ஆலோசனை அறையானது போதிய வெளிச்சம் மற்றும் நல்ல காற்றோட்டத்துடன், 100 சதுர அடிக்குக் குறையாமல் போதிய இடவசதியுடன் இருக்க வேண்டும். ஆலோசனை பெறுவதற்காக நோயாளிகள் காத்திருப்பதற்கு தனி இடவசதிக்கு வகைசெய்யப்பட வேண்டும்.

(2) பணியாளர்கள்.- இந்திய மருத்துவக் குழு சட்டத்தின் கீழ் கோரப்பட்டுள்ளவாறு, பதிவுசெய்யப்பெற்ற மருத்துவர் மட்டுமே நோயாளியைப் பரிசோதித்து, சிகிச்சை மருந்து சீட்டு அளிக்க வேண்டும் .

(3) மருத்துவச் சாதனங்கள்.-மருத்துவ ஆலோசனை அறையில் பின்வரும் மருத்துவச் சாதனங்கள் மற்றும் மருத்துவமனை துணைப் பொருட்கள் இருக்க வேண்டும் :-

(a) நோயாளியைப் பரிசோதிப்பதற்குப் பயன்படுத்தப்படும் படுக்கை

(b) இரத்த அழுத்தமானி (Blood Pressure Apparatus)

(c) இதய துடிப்பு மானி (Stethoscope)

(d) டார்ச் லைட்

(e) நாக்கு அழுத்தி (Tongue Depressor)

(f) தெர்மாமீட்டர்/ வெப்பமானி (காய்ச்சல் மானி)

(g) பெரியவர்கள் மற்றும் குழந்தைகளுக்காக எடை பார்க்கும் இயந்திரம்

(h) முழங்கால் எலும்பை பரிசோதிக்க உதவும் கருவி (Knee Hammer)

(i) அளவு நாடாTAMIL NADU GOVERNMENT GAZETTE EXTRAORDINARY 31 (j) நேர்விற்கேற்ப, புரோட்டோஸ்கோப், (Protoscope) பி.வி. தட்டு, (P.V. Tray) கண் பரிசோதனை திரை மற்றும் கண், மூக்கு, தொண்டை ஆய்வுக்குப் பயன்படுகிற தட்டுகள் போன்ற சிறப்பு ஆய்வுக்குப் பயன்படுகிற தட்டுகள்

(k) இதைத் தவிர, நோயாளியை பரிசோதிப்பதற்காக திரை வசதி அல்லது போதிய அளவில் தனிமை வசதி (4) பதிவுருக்கள்.- ஒவ்வொரு மருத்துவ நிறுவனமும் நோயாளியின் பெயர், வயது, பாலினம் மற்றும் முகவரி, பரிசோதனை செய்யபட்ட நாள், அனுமதிக்கப்பட்ட நாள் ஆகிய தகவல்களைக் கொண்டிருக்கும் பதிவேடு ஒன்றை பராமரித்துவர வேண்டும். நோயாளியின் பெயர், வயது, பாலினம், மற்றும் கலந்தறியும் நாள், கண்டறியப்பட்ட நோய்க்குறி (தற்காலிகம் மற்றும் இறுதி) மற்றும் அறிவுறுத்தப்பட்ட சிகிச்சை முறை, பரிசோதனையின் முடிவு (எடுக்கப்பட்டிருந்தால்) கொண்ட சீட்டு ஒன்று நோயாளியிடம் வழங்கப்படுதல் வேண்டும்.

(5) மருந்துச்சீட்டு.- மருத்துவர், மருந்துச்சீட்டில் நாள் மற்றும் முத்திரையுடன் கையொப்பமிடுதல் வேண்டும்.

II மருத்துவ மையம்.- மருத்துவ ஆலோசனை அறைக்கு வரையறுக்கப்பட்ட விதிமுறைகளுடன் பின்வரும் விதிமுறைகளும் நிறைவு செய்யப்பட வேண்டும்..

(1) கட்டடம் .- ஒவ்வாமை குறித்த நேர்வில் அல்லது நரம்பு வழியாக திரவம் செலுத்துவதற்காக அல்லது கூர்ந்து கவனித்தல் போன்றவற்றிற்காக, நோயாளியைப் படுக்க வைப்பதற்கு இரு படுக்கைகளுக்கு போதுமான இடவசதி இருக்க வேண்டும்.

(2) பணியாளர்.- மிகக் குறைந்த அளவாக ஒரு மருத்துவர் மற்றும் மருத்துவம் சார்ந்த பணியாளர் ஒருவர்.

(3) சாதனங்கள்.- (a) நுண்ணுயிர் நீக்கி அல்லது தொற்று நீக்கி

(b) நுண்ணுயிர் நீக்கப்பட்ட அல்லது தொற்று நீக்கப்பட்ட மருந்தூசிகள்/சாதனங்களுக்கான வசதி அல்லது மறுபயன்பாட்டுக்கு ஒவ்வாத மருந்தூசிகள் மட்டுமே பயன்படுத்தப்பட வேண்டும்.

(c) கிருமி நீக்கம் செய்யப்பட்ட கையுறைகள்

(d) இடுக்கிகள் (Cheetles Forceps)

(e) அறுவைச் சிகிச்சைக்கான கத்திரிக்கோல்

(f) ஒட்டும் தன்மையுடைய பிளாஸ்திரி

(g) மருத்துவ சாதனங்களுடன் கூடிய தையல் போடுவதற்கான பொருட்களைக் கொண்ட தட்டு

(h) சிறுநீரக வடிவிலான தட்டு

(i) சீழ்கட்டியை கீறி, சீழை வெளியெடுப்பதற்கான அறுவைசிகிச்சை சாதனங்கள்

(k) மருந்துப் பொருட்கள்

(4) கழிவுகளை அகற்றுதல்.- இந்திய அரசு மற்றும் தமிழ்நாடு மாசு கட்டுப்பாட்டு வாரியத்தின் வரையறைகளுக்கேற்ப, கழிவுகள், கட்டு போட பயன்படுத்திய பருத்தித் துணிகள் போன்றவற்றை தனித்தனியே பிரித்து வைப்பதற்கான வசதிகள் (5) மருந்து வழங்குதல்.- மருத்துவ மையம் நோயாளிகளுக்கு மருந்து வழங்குவதில் ஈடுபட்டிருந்தால், தகுதி வாய்ந்த மருந்தாளுநரால் அல்லது மருத்துவ சிகிச்சை நிலையத்தில் உள்ள மருத்துவரால் அல்லது அவருடைய நேரடி மேற்பார்வையின் கீழ் மட்டுமே மருந்து வழங்கப்பட வேண்டும்.

(6) மருத்துவ மையத்தில் உள்ள பரிசோதனைக்கூடம் .- மருத்துவ மையம் ஆய்வுக்கூட சேவைகளை வழங்குமாயின், அரசால் அங்கீகரிக்கப்பட்ட மருந்தக ஆய்வக தொழில்நுட்பவியல் பட்டயம் / மருந்தக ஆய்வியல் தொழில்நுட்பவியல் (DMLT / CMLT) சான்றிதழை வைத்திருக்கும் தகுதிவாய்ந்த தொழில்நுட்பப் பணியாளரால் ஆய்வுக்கூட பணிகள் மேற்கொள்ளப்பட வேண்டும் அல்லது மாற்றாக மருத்துவ அதிகாரியால், அவரின் நேரடி மேற்பார்வையின் கீழ் பயிற்சி பெற்ற பணியாளரால் ஆய்வுகள் மேற்கொள்ளப்பட வேண்டும். எனினும், வெண் புரத சர்க்கரைக்கான (Albumin) சிறுநீரகச் சோதனை மற்றும் இரத்தத்தில் ஹீமோகுளோபின் எண்ணிக்கை போன்றவை குறித்த பரிசோதனைகள் பயிற்சி பெற்ற நபரால் மேற்கொள்ளப்படவேண்டும்.

(7) ஊடுகதிர் படம் (நுண்கதிர் வீச்சு).- மருத்துவ மையத்தில் அதற்குச் சொந்தமான ஊடுகதிர் பிரிவு இருப்பின், அரசால் அங்கீகரிக்கப்பட்ட CRA சான்றிதழ் (Clinical Research Associate Certifi cate) வைத்துள்ள தகுதி வாய்ந்த நுண்கதிர் வீச்சாளரால் ஊடுகதிர்படம் எடுக்கப்பட வேண்டும்.

(8) சிறிய அறுவைசிகிச்சைகள்.- நீர்க்கட்டியை அகற்றுதல், மருத்துவ ரீதியிலான கருக்கலைப்பு, D & C போன்ற சிறிய அறுவை சிகிச்சைகளை மேற்கொண்டால், அந்த நோக்கங்களுக்காக சுத்தமான அறை மற்றும் சிறிய அறுவை அரங்கிற்கு தேவைப்படும் வசதிகள் அடங்கியதாக இருத்தல் வேண்டும்.

III பல்வகை நோய்களுக்கு சிகிச்சையளிக்கும் மருத்துவமனை.-

1. மருத்துவ சிகிச்சை மையம் மற்றும் மருத்துவ ஆலோசனை அறைக்கு வரையறுக்கப்பட்ட விதிமுறைகளுடன் பல்வேறு சிகிச்சைப் பிரிவுகள் இருப்பின், ஒவ்வொரு சிறப்பு சிகிச்சை பிரிவுக்கும் தனி அறை வழங்க ஏற்பாடு செய்யப்பட வேண்டும்.

2. நோயாளிகளுக்கு, காத்திருப்பதற்கான அறை மற்றும் சுத்தமான குடிநீர் வழங்க வகைசெய்யப்பட வேண்டும். காத்திருக்கும் அறையில், பல்வேறு பிரிவுகளில் பணிபுரியும் மருத்துவர்களின் பெயர்கள் பார்வைக்கு வைக்கப்பட வேண்டும்.

IV. மருத்துவமனை.-

(1) உள்கட்டமைப்பு.- மருத்துவ மையம், ஆலோசனை அறை மற்றும் பல்வகை நோய்களுக்கு சிகிச்சையளிக்கும் மருத்துவமனைக்கு வரையறுக்கப்பட்ட விதிமுறைகளுடன் மருத்துவமனைக்கு வருகை தருகிற புற நோயாளிகளின் எண்ணிக்கைக்கேற்றவாறு போதுமான இடவசதி கொண்ட காத்திருப்போர் அறைக்கு வகை செய்யப்பட வேண்டும்.

(2) வார்டுகள் அல்லது உள் நோயாளிகளுக்கான அறைகளின் தொகுப்பு.- (a) இடவசதி.-

(i) உள்நோயாளிகள் அனுமதிக்கப்படும் வார்டுகள் அல்லது உள்நோயாளி அறைகள், இடவசதியுடன் கூடிய நல்ல காற்றோட்டமான, வெளிச்சம் கொண்டதாக இருக்க வேண்டும். ஒவ்வொரு அறைக்கும் மின்விசிறி வழங்கப்பட வேண்டும். பொது அறைகளில், குறைந்தது இரண்டு படுக்கைகளுக்கு ஒரு மின்விசிறியாவது இருக்க வேண்டும்.

(ii) கழிப்பறைகள் பொதுவானதாகவோ அல்லது தனியாகவோ இருக்கலாம். ஆனால், அவை சுத்தமாகவும், தண்ணீர் வசதியுடனும் இருக்க வேண்டும்.

(iii) கிருமி நாசினி கொண்டு நாள்தோறும் வார்டுகளைத் துடைப்பதற்கு ஏற்பாடுகள் செய்யப்பட வேண்டும்.

(iv) பிராணவாயு (Oxgyen), உப்பு நீர் நிறுத்தி (Saline Stand) போன்ற அவசியமான சாதனங்களுடன், ஓவ்வொரு 20 அறைகளுக்கும் ஒரு மருத்துவப் பணியாளர் அலுவல் அறை இருக்க வேண்டும்.

(b) பணியாளர்கள்.- (i) மருத்துவர்கள்.-

- ஒவ்வொரு 30 படுக்கைகளுக்கும், ஒரு தகுதி வாய்ந்த மருத்துவர்

- இரவு நேரத்தில் மற்றும் அலுவல் இல்லாத நேரங்களில், அழைக்கும் போது வந்து சிகிச்சை அளிப்பதற்கு 30 படுக்கைகளுக்கு ஒரு மருத்துவர் இருக்க வேண்டும்.

- மருத்துவமனையில் 30 படுக்கைகள் மற்றும் அதற்கு மேலான படுக்கைகள் உள்ள பொது மருத்துவப்பிரிவில் 24 மணி நேரமும் ஒரு மருத்துவர் நேரடியாகப் பணியில் இருக்க வேண்டும்.

- மருத்துவமனை 30 படுக்கைகளுக்கும் குறைவாக பெற்றிருந்து, ஆனால் அங்கு அவசர சிகிச்சைப்பிரிவு செயல்பட்டுவருமாயின் 24 மணி நேரமும் ஒரு மருத்துவர் பணியில் இருக்க வேண்டும்.

(ii) செவிலியர்கள்.- ஒரு மருத்துவமனையில் 30 படுக்கைகள் வரைக்கும் ஒரு தகுதிவாய்ந்த செவிலியர் மற்றும் இரண்டு பயிற்சிபெற்ற செவிலியர் உதவியாளர்கள் இருக்க வேண்டும். கூடுதலாக உள்ள ஒவ்வொரு 30 படுக்கைகளுக்கும் ஒரு தகுதிவாய்ந்த செவிலியர் இருக்க வேண்டும்.

- மேற்சொன்ன பணியாளர் எண்ணிக்கையானது, மருத்துவமனையில் பணிபுரிந்துவரும் பயிற்சிபெற்ற பணியாளர்கள் எண்ணிக்கை எவ்வளவாக இருப்பினும் குறைந்தளவு தேவைப்படுகிற எண்ணிக்கை ஆகும்.

(iii) செவிலிய உதவியாளர்.- படுக்கைகளின் எண்ணிக்கைகளைப் பொறுத்து, போதுமான அளவில், காயங்களுக்கு கட்டு கட்டுதல், மலக்குடலை தூய்மை செய்தல் மற்றும் நோயாளிகளை அறுவை சிகிச்சை மற்றும் பிரசவத்திற்கு தயார் செய்தல் போன்ற பணிகளில் மருத்துவர்கள் மற்றும் செவிலியர்களுக்கு உதவுவதற்காக ஆண் மற்றும் பெண் உதவி செவிலியர்கள் ஈடுபடுத்தப்பட வேண்டும். ஆனால் ஊசிபோடுதல் அல்லது தையல் போடுதல் போன்ற பணிகள் அவர்களுக்கு வழங்கக்கூடாது, தகுதிவாய்ந்த செவிலியரின் கீழ் அவர்கள் பணிபுரிதல் வேண்டும்.

(iv) சுத்தம் செய்யும் பணிகள்.- மருத்துவ சிகிச்சை நிலையத்தில் / மருத்துவமனையில் போதுமான எண்ணிக்கையில் சுத்தம் செய்யும் பணியாளர்கள் (பெருக்குபவர் / துப்புரவுப் பணியாளர்) இருக்க வேண்டும். குறைந்தளவாக 20 படுக்கைகளுக்கு ஒரு துப்புரவுப் பணியாளர் என்பது குறைந்தபட்ச தேவையான எண்ணிக்கை ஆகும்.

3. பிற சேவைகள்.-

(a) மருந்தகம்.- மருத்துவமனையில் நோயாளிகளுக்கு மருந்துகள் வழங்குவதற்காக மருந்தகம் செயல்பட்டு வருமாயின், பதிவுபெற்ற தகுதிவாய்ந்த மருந்தாளுநரால் மட்டுமே மருந்துகள் வழங்கப்பட வேண்டும். அவர் எந்த மருந்துகளையும் பரிந்துரைக்கக் கூடாது.

(b) ஆய்வகம்.-மருத்துவமனையால் ஆய்வகம், நடத்தப்பட்டு வருமாயின் வெண்புரத சர்க்கரைக்கான (Albumin) சிறுநீரக சோதனை, இரத்தத்தில் உள்ள ஹீமோகுளோபின் அளவு ஆகியவற்றைத் தவிர மற்ற அனைத்து பரிசோதனைகளும் அங்கீகரிக்கப்பட்ட மருந்தக ஆய்வக தொழில்நுட்பவியல் பட்டயம் / மருந்தக ஆய்வியல் தொழில்நுட்பவியல் (DMLT/ CMLT) சான்றிதழ் பெற்றுள்ள தகுதிவாய்ந்த ஆய்வக தொழில்நுட்பவியலாளரால் மட்டுமே செய்யப்பட வேண்டும்.

(c) எக்ஸ்ரே (ஊடுகதிர் நிழற்படம்).- (X-ray) ஊடுகதிர் பிரிவானது, வரையறுக்கப்பட்டுள்ள அளவுகோள்களுக்கு (Norms) ஒத்திசைவாக இருக்க வேண்டும், மேலும் சி ஆர் எ (CRA) கல்வித்தகுதிபெற்ற தகுதிவாய்ந்த (Radiographer) கதிர் வரைபடப் படப் பதிவாளரால் எடுக்கப்பட வேண்டும்.

(d) ஸ்கேன் (நுண்கதிர் வீச்சு).-மருத்துவரால் மட்டுமே அல்ட்ரா சோனோகிராம் (மீயொலிவரைவி) பரிசோதனை செய்யப்பட வேண்டும்.

(e) உணவு சமைத்தல் மற்றும் உணவகம்.-உணவக வசதிகள் இருப்பின், சமையல் கூடம் சுத்தமாக இருக்க வேண்டும். சமைப்பவர்கள் காலமுறைப்படி மருத்துவப் பரிசோதனைக்கு உட்படுத்தப்பட வேண்டும். உணவகம் மற்றும் சமையலறையில் சுத்தமான நடைமுறைகள் பின்பற்றப்பட வேண்டும்.

(f) இரத்த வங்கி.- இரத்த வங்கி மருத்துவமனையால் பராமரிக்கப்பட்டு வந்தால் இந்திய அரசால் அவ்வப்போது வெளியிடப்படுகிற உத்தரவுகளை அது பின்பற்றுவதை உறுதி செய்ய வேண்டும்.

(g) அவசரகால ஊர்தி.- 100 படுக்கைகளுக்கு மேல் கொண்டுள்ள அனைத்து மருத்துவமனைகளும் ஓட்டுநருடன் கூடிய நோயாளர் ஊர்தியை வைத்திருப்பது உகந்ததாகும். அல்லது மாற்றாக, நோயாளர் ஊர்தி வைத்துள்ள மற்றொரு மருத்துவமனையுடன் அல்லது சில நோயாளர் ஊர்தி சேவை பிரிவுடன் ஒப்பந்தம் செய்துகொள்ள வேண்டும்.

(h) பொது வசதிகள்.-

(i) கழிவுகளை நீக்குதல்.- இந்திய அரசின் அளவுகோள்கள் (Norms) மற்றும் மாசுக்கட்டுப்பாட்டு வாரியத்தால் வழங்கப்பட்டுள்ள வழிகாட்டுதல்களின்படி, கழிவுகள் நீக்குதல் செய்யப்பட வேண்டும் அந்த இடத்திலேயே கழிவுகள் முறையாகப் பிரிக்கப்படுவது உறுதி செய்யப்பட வேண்டும்.

(ii) பதிவேடுகளை பராமரித்தல். (a) அறுவை சிகிச்சையை செய்த மருத்துவரின் பெயர் மற்றும் அப்போது பணியில் இருந்த மயக்கமருந்து அளிக்கும் மருத்துவரின் பெயர் உட்பட மருத்துவ நிறுவனத்தில் நடைபெற்ற அனைத்து அறுவை சிகிச்சைகள் குறித்த விவரங்கள் பதிவேட்டின் வடிவிலான ஒரு தனி பதிவுருவில் பராமரித்து வரப்பட வேண்டும். சிகிச்சை முடிந்து அனுப்பிவைக்கப்படும் அல்லது வேறு மருத்துவமனைகளுக்கு பரிந்துரைக்கப்படும் அனைத்து நோயாளிகளுக்கும், அவர்களுக்கு அளிக்கப்பட்ட சிகிச்சை பற்றிய அறிக்கை அட்டை வழங்கப்பட வேண்டும்.

(b) மருத்துவ சிகிச்சை நிலையத்தில், ஒவ்வொரு நோயாளி குறித்த பின்வரும் விவரங்களைக் கொண்ட பதிவுரு வடிவிலான மருத்துவ பதிவேடு, பத்து ஆண்டுகளுக்குக் குறையாமல், பராமரித்து வரப்பட வேண்டும், (i) நோயாளியின் உடல்நிலை குறித்த விரிவான அன்றாட அறிக்கை.

(ii) மேற்கொள்ளப்பட்ட யாதொரு பரிசோதனைகள், செய்யப்பட்ட அறுவைசிகிச்சைகள் மற்றும் அளிக்கப்பட்ட சிகிச்சைகள் குறித்த விவரங்கள் மற்றும்

(iii) மருத்துவரின் முழு பெயர் மற்றும் தகுதிகளுடன் கூடிய அனைத்து மருந்துச் சீட்டுகள், ஊடுகதிர் அறிக்கைகள், ஆய்வக அறிக்கைகள், அல்லது ஏனைய யாதொரு அறிக்கைகள் கொண்ட பதிவுருக்கள்.

(iv) மகப்பேறு மருத்துவமனை அல்லது மகப்பேறு குறித்த நேர்வில் மேற்சொன்ன நபர், ஒவ்வொரு தாய்க்கும் பிறந்த குழந்தை குறித்த பதிவுகளை வைத்துவர வேண்டும். மகப்பேறு குறித்த முழுமையான மற்றும் விரிவான பதிவுநிலை குறைந்தளவாக 10 ஆண்டுகளுக்கேனும் பராமரித்து வரப்பட வேண்டும்.

(iii) சட்டம் சார்ந்த இனங்களில் மருத்துவச் சிகிச்சை அளிக்கும் நேர்வுகள்.- அரசின் ஆணைப்படி, சட்டம் சார்ந்த இனங்களில் மருத்துவ சிகிச்சை அளிக்கும் ஒவ்வொரு மருத்துவமனையும், அத்தகைய இனங்களுடன் தொடர்புடைய அ ை ன த் து பதிவுருக்கள் மற்றும் பதிவேடுகளும், அவ்வப்போது அரசு பிறப்பிக்கின்ற ஆணைகளின்படி பாதுகாத்து வரப்பட வேண்டும். மேலும், அவர்கள் சிகிச்சைக்காக அனுமதிக்கப்படுதல், அவர்கள் சிகிச்சை முடிந்து அனுப்பப்படுதல், அவர்களின் இறப்பு குறித்து காவல்துறைக்கு தெரிவிக்கப்பட வேண்டும்.

(4) சவக்கிடங்கு .- 100 படுக்கைகளுக்கு மேலான படுக்கைவசதிகளைக் கொண்டுள்ள மருத்துவமனை, இறந்தவர்களின் உடல்களை வைப்பதற்காக ஒரு சவக்கிடங்கு அல்லது குளிர்சாதனப் பெட்டியுடன் கூடிய அறையைக் கொண்டிருக்க வேண்டும்.

(5) குடிநீர் வழங்கல்.- சுத்தமான குடிநீர் கிடைக்கச் செய்ய வேண்டும். மேலும் சூடான குடிநீரும் வழங்கப்பட வேண்டும்.

(6) துணிகள் மற்றும் லினன்.- நோயாளிகளுக்காக பயன்படுத்தப்பட்ட போர்வைகள் மற்றும் பிற துணிகள் நாள்தோறும் மாற்றப்பட வேண்டும்.

(7) தீயணைப்பு வசதி .- செயல்பாட்டிலுள்ள விதிகளின்படி தீயணைப்பு சாதனங்கள் நிறுவப்பட வேண்டும். இந்திய தரச்சான்று பெற்ற சாதனங்கள் மட்டுமே வழங்க வகைசெய்யப்பட வேண்டும். தீ விபத்து ஏற்பட்டால் தீயிலிருந்து தப்பிப்பதற்கான வழிவகைகள், தீ அணைப்பான்கள் மற்றும் முன்னெச்சரிக்கை செய்யும் கருவிகள் மற்றும் தீ விபத்தின் போது அனைத்து நபர்களையும் மருத்துவ சிகிச்சை இல்லம் அல்லது மருத்துவ சிகிச்சை மையம் அல்லது மருத்துவமனையிலிருந்து வெளியேற்றுவதற்கான வழிவகைகள் உட்பட போதிய முன்னெச்சரிக்கை நடவடிக்கைகள் மேற்கொள்ளப்பட வேண்டும்.

(8) தரச்சான்றுறுதி.- மருத்துவமனையில் பணிபுரியும் மருத்துவர்களின் பெயர்கள் மற்றும் மருத்துவமனையில் உள்ள வசதிகள் ஆகியவை எழுதிவைக்கப்பட்டு, பார்வைக்கு வைக்கப்பட வேண்டும். மருத்துவமனையானது நோயாளிகளுடனான அதன் ஒவ்வொரு செயல்பாடுகளிலும் தரத்தினை கடைப்பிடிக்க பாடுபட வேண்டும்.

(9) தொடர்புவசதிகள்.- நோயாளிகள் அவர்களுடைய உறவினர்கள் மற்றும் சிகிச்சைக்காக பரிந்துரைக்கப்படுகிற இடங்களுடன் தொடர்புகொள்வதற்காக, மருத்துவமனையில் தொலைபேசி வசதி செய்யப்பட்டிருக்க வேண்டும்.

(10) பாதுகாப்பு .- நோயாளிகள் மற்றும் அவர்களின் உடைமைகளின் பாதுகாப்பு மற்றும் குழந்தைகள் கடத்தல் போன்றவை நடைபெறாமல் இருப்பதை உறுதி செய்யும் விதமாக உரிய பாதுகாப்பு நடவடிக்கைகள் மருத்துவமனைகளால் எடுக்கப்பட வேண்டும். ஒவ்வொரு மருத்துவ சிகிச்சை வழங்கும் நிறுவனமும், 2008ஆம் ஆண்டு தமிழ்நாடு மருத்துவப்பாதுகாப்பு சேவை வழங்குகிற நபர்கள் மற்றும் மருத்துவ பாதுகாப்பு சேவை வழங்குகிற நிறுவனங்கள் (வன்முறை மற்றும் சொத்துகளுக்கு சேதம் அல்லது இழப்பு ஏற்படுவதைத் தடுத்தல்) சட்டத்தின் (தமிழ்நாடுச் சட்டம் 48/ 2008) தொடர்புடைய வகைமுறைகளை பார்வைக்குரிய இடத்தில் காட்சிப்படுத்த வேண்டும்.

(11) மருத்துவமனை துணைச்சாதனங்கள்.- மருத்துவமனையில் போதுமான எண்ணிக்கையில் சக்கர நாற்காலிகள், தள்ளுவண்டிகள் மற்றும் தூக்குப்படுக்கைகள் (Stretchers) ஆகியவை இருத்தல் வேண்டும்.

(12) மின் ஆக்கி சாதனம்:.- மின் தடையின்போது, அவசியமான பணிகளை மேற்கொள்ள மின் ஆக்கி சாதனம் ஒன்றுக்கு வகைசெய்யப்பட்டிருக்க வேண்டும்.

(13) பிரசவ அறை.- மருத்துவமனை பிரசவ அறையை பராமரித்து, அங்கு பிரசவம் நடைபெறுமாயின், அது பின்வரும் அளவுகோள்களை (Norms) கொண்டிருக்க வேண்டும்; அவையாவன:-

(a) கட்டடம்.- மகப்பேறுக்காக அனுமதிக்கப்பட்டுள்ள பெண்ணை பிரசவத்திற்கு ஆயத்தம் செய்தல், மலக்குடல் சுத்தம் செய்தல் போன்றவற்றிற்காக, போதுமான தண்ணீர் வசதியுடன்கூடிய ஒரு அறை இருக்க வேண்டும். பிரசவ அறையானது 120 சதுர அடிக்குக் குறையாமல் நல்ல காற்றோட்டத்துடனும் போதிய வெளிச்சத்துடனும் இருத்தல் வேண்டும். புதியதாகப் பிறந்த குழந்தையை வைப்பதற்கும் அதனை நன்கு சுவாசிக்க வைப்பதற்கும் இடவசதி இருத்தல் இன்றியமையாததாகும்.

(b) பணியாளர்கள்.- தகுதிவாய்ந்த மருத்துவர், செவிலியர், பேறுகால துணைச் செவிலியர் அல்லது பயிற்சிபெற்ற தாதி ஆகியோரால் பிரசவம் நடத்தப்பட வேண்டும். பயிற்சி அளிக்கப்படாத நபர்களிடம் இப்பணி ஒப்படைக்கப்படக்கூடாது. செவிலியர் அல்லது பேறுகால துணை செவிலியர் பிரசவத்தை நடத்தினாலும்கூட, அவசர சூழல்களில் புதிதாகப் பிறக்கும் குழந்தையை சுவாசிக்க வைப்பதற்காக, அழைத்தவுடன் வந்து சிகிச்சை அளிக்கும் வகையில் மருத்துவர் ஒருவர் தயாராக இருக்க வேண்டும். பிரசவ வார்டு மகப்பேறு மருத்துவமனை ஒன்றில் இருந்து பிரசவங்கள் வழக்கமாக நடைபெறுமெனில், பணியில் இருக்கும் மருத்துவர் ஒருவர் 24 மணி நேரமும் மருத்துவமனையில் இருக்க வேண்டும். எங்கெங்கெல்லாம் முடியுமோ அழைத்தவுடன் வந்து சிகிச்சையளிக்கும் வகையில் குழந்தை மருத்துவ வல்லுநர் ஒருவர் இருத்தல் விரும்பத்தக்கது.

(c) சாதனங்கள்.- பின்வரும் உபகரணங்கள் மற்றும் மருத்துவமனை துணைக்கருவிகள் இருத்தல் வேண்டும். அவையாவன:-

(i) பிரசவப்படுக்கை நிலையுடன் கூடிய பிரசவப்பலகை

(ii) உப்புநீர் நிறுத்தி (Saline Stand)

(iii) துவார முகமூடியுடனான பிராணவாயு உருளை

(iv) உபகரணத்துடன் கூடிய பி.வி. தட்டு (P.V. Tray)

(v) டி & சி (டைலடேஷன் & கியூரடேஜ்) (C&A) சி & எ தொகுதி

(vi) குவிப்பு விளக்கு

(vii) உபகரணங்களுக்கான மற்றும் குழந்தையை வைத்துக் கொள்வதற்கான வண்டி

(viii) வேக்குவம் எக்ஸ்ட்ராக்டர் / இடுக்கி (Vaccum Extractor / Forceps)

(ix) தொப்புள் நடுப்பகுதிக்கான தூய்மையாக்கப்பட்ட கவ்வி (Sterile cord Clamp)

(x) சீழ் எதிர்ப்பி குழம்பு (Antiseptic Lotion)

(xi) உறிஞ்சும் கருவி (Suction Apparatus)

(xii) குழந்தைக்கான சுவாச அமைப்பு (Baby resuscitation kit)

(xiii) குழந்தைக்கான வெப்பப்படுத்தி (விளக்கும் கூட பயன்படுத்தலாம்)

(xiv) குழந்தைக்கான சளி உறிஞ்சி (disposable)

(xv) குழந்தை மருத்துவ ஆம்பு பை (Ambu bag)

(xvi) கரவிடவாய்த்திறப்பு மற்றும் தையல் மூட்டிற்கான தையல் மூட்டுத்தட்டு (Suturing tray for episiotomy and suturing) (d) மருந்துகள்.-

- மெத்தர்ஜின் ஊசி மருந்து 5 சிமிழ்

- பிடோசின் ஊசி மருந்து 5 சிமிழ்

- கார்போபுரோஸ்ட் ஊசி மருந்து 5 சிமிழ்

- ஐ.வி. திரவங்கள் 5% டெக்ஸ்ட்ரோஸ் 2 குடுவைகள்

- 5% டெக்ஸ்ட்ரோஸ் அல்லது உப்புநீர்

- ரின்ஜர் லேக்டேட் (Ringer Lactate)

- டையாஜிபம் ஊசி மருந்து 2 amp.

- 2% லிக்னோகேன் 1 சிறுகுப்பி (vial)

- அப்புறப்படுத்தத்தக்க மருந்தூசிகள் 10 எண்ணிக்கை (Disposable syringes)

- வைட்டமின் கே ஊசி மருந்து (Inj. Vitamin-K)

- எப்டாயின் ஊசிமருந்து (Inj. Eption)

(e) பதிவுருக்கள்.-

(i) பிறப்புப்பதிவேடு

(14) அறுவை சிகிச்சை அரங்கு.-

(a) சிறு அறுவை சிகிச்சை அரங்கு.-அழுகல் மற்றும் சீழ்கட்டி, வெட்டுக்காயம், சீழ்வடிதல் போன்றவற்றிற்கான சிறு அறுவை சிகிச்சைகள் மற்றும் அழுகல் போன்ற நேர்வுகளில் உறுப்பு நீக்க சிகிச்சை முதலியனவும் மேற்கொள்ளப்படும் இடம்.

(i) இட வசதி.- போதுமான இட வசதி போதுமான வெளிச்சம் மற்றும் காற்றோட்டத்துடன் கூடிய 100 சதுர அடிக்கு குறையாத இடவசதி

(ii) பணியாளர்கள்.- சிறு அறுவை சிகிச்சை அரங்கில் மேற்கொள்ளப்படும் அனைத்து அறுவை சிகிச்சைகளும் தகுதி வாய்ந்த மருத்துவரால் மட்டுமே மேற்கொள்ளப்பட வேண்டும். எனினும், காயக்கட்டுகள், செவிலியர் அல்லது செவிலிய உதவியாளரால் மேற்கொள்ளப்படலாம்.

(iii) உபகரணங்கள்.-அறுவை சிகிச்சை மேசை

- குவி விளக்கு

- தூய்மையாக்கி/நுண்ணுயிர் நீக்கி

- ஆட்டோ கிளேவ்

- மூடுதொட்டிகள்

- உப்புநீர் தாங்கி (Saline Stand)

- சிறு அறுவை சிகிச்சை உபகரணங்கள்

- பகுதி உணர்வகற்றல் உபகரணம்

- தூய்மையான கையுறைகள்

- தூய்மையாக்கப்பட்ட பீச்சுக்குழாய்கள்

(iv) பதிவுருக்கள்.- சிறு அறுவை சிகிச்சை அரங்கில் மேற்கொள்ளப்படும் அனைத்து அறுவை சிகிச்சைகளின் பதிவுகளும் பதிவேட்டில் பதிவு செய்யப்பட வேண்டும்.

(b) முதன்மை அறுவை சிகிச்சை அரங்கு.-

(i) இட வசதி.- குறைந்தளவாக 150 சதுர அடியில் போதுமான இடவசதியுடன், நன்கு வெளிச்சமான இடம் சீர் வளி வசதியுடன் இருப்பது சிறந்தது ஆகும். அறுவை சிகிச்சை அறை மட்டுமல்லாது, நோயாளி காத்திருப்பு அறை மற்றும் அறுவை சிகிச்சைக்குப்பின்னர் மீண்டு வருதல் வரைக்கும் தேவைப்படும் படுக்கை வசதிக்கு, வழிவகை செய்யப்பட வேண்டும். ஆட்டோகிளேவ் மற்றும் தூய்மையாக்கல் மற்றும் தேய்த்துக்கழுவுவதற்கு போதுமான இடவசதி இருக்க வேண்டும். தேய்த்துக்கழுவுவதற்கான அறையில் குழாயில் எப்பொழுதும் தண்ணீர் வரும் வகையில் இருக்க வேண்டும். அறுவை சிகிச்சை அறையில் புற ஊதாக்கதிர் (Ultra Violet) விளக்குகள் பொருத்தப்பட்டிருக்க வேண்டும். அறுவை சிகிச்சை அறை போதுமான ஏற்பாடுகளுடன் ஒழுங்குபடுத்தப்பட்டிருக்க வேண்டும். குறைந்தது மாதத்திற்கொரு முறையாவது அறுவை சிகிச்சை அறை ஒற்றி துடைக்கப்பட்டிருக்க வேண்டும்.

(ii) பணியாளர்கள்.- தகுதிவாய்ந்த செவிலியர் ஒருவர் அறுவை சிகிச்சை அரங்கில் செவிலியர் பணியாளர்களுக்கு தலைமை செவிலியராக இருக்க வேண்டும். தகுதிவாய்ந்த உணர்வகற்றல்/மயக்கவியல் மருத்துவர் அல்லது உணர்வகற்றலில் பயிற்சி பெற்ற மருத்துவரால் மட்டுமே உணர்வகற்றல்/மயக்க மருந்துகள் வழங்கப்பட வேண்டும். தகுதிவாய்ந்த மருத்துவர்களால் மட்டுமே அறுவை சிகிச்சைகள் மேற்கொள்ளப்பட வேண்டும்.

(iii) உபகரணங்கள்.-

(iv) பதிவுருக்கள்.-மேற்கொள்ளப்பட்ட அனைத்து அறுவை சிகிச்சைகளிலும், நோயாளி, அறுவை சிகிச்சை நிபுணர், மயக்கமருந்து நிபுணர் மற்றும் செவிலியர் ஆகியோரின் பெயர், வயது, பாலினம், முகவரி ஆகியவை பற்றிய பதிவுகள் பதிவேட்டில் பதியப்பட்டிருக்க வேண்டும். உடற்திசு ஆய்வு அறிக்கைகளும் பதியப்பட்டிருக்க வேண்டும்.

(15) தீவிர சிகிச்சை பிரிவு .- 100 படுக்கைகளுக்கு மேலுள்ள அனைத்து மருத்துவமனைகள்/ மருத்துவ மையங்களில் தீவிர சிகிச்சை பிரிவு பராமரிக்கப்படலாம். பின்வரும் குறைந்தளவு அளவுகோல்கள் பின்பற்றப்பட வேண்டும், அவை.-

(a) இட வசதி.- நன்கு காற்றோட்டத்துடன் குளிரூட்டப்பட்டு போதுமான அளவு வெளிச்சத்துடன் இருப்பது மிக அவசியமாகும். அங்கு 4 படுக்கைகள் இருப்பின், குறைந்தளவு 240 சதுர அடி அவசியமாகும். செயற்கை சுவாசக்கருவிகள் மற்றும் கண்காணிப்புக்கருவிகள் வைக்கப்போதுமான இடம் இருக்க வேண்டும். பணியிலுள்ள மருத்துவர் மற்றும் செவிலியருக்கு வேண்டிய இடவசதி இருக்க வேண்டும். தீவிர சிகிச்சை பிரிவிற்கு வெளியே நோயாளியுடன் உடனிருப்பவருக்கான இடவசதி செய்யப்பட வேண்டும்.

(b) பணியாளர்கள்.- பகல் நேரத்தில் ஒவ்வொரு 10 படுக்கைகளுக்கும், ஒரு மருத்துவர் இருக்க வேண்டும். 10 படுக்கைகளுக்கும் மேல் இருப்பின் ஒவ்வொரு கூடுதல் 5 படுக்கைகளுக்கும் ஒரு கூடுதல் மருத்துவர் இருக்க வேண்டும். இரவு நேரத்துக்கு இது போன்ற வசதி செய்யப்படவேண்டும்.

(c) செவிலியர்.-பகல் நேரத்தில் 6 படுக்கைகளுக்கு ஒரு தகுதிவாய்ந்த செவிலியர் இருக்க வேண்டும். ஒவ்வொரு கூடுதலான 3 படுக்கைகளுக்கும் மற்றொரு தகுதிவாய்ந்த செவிலியர் இருக்க வேண்டும். இந்த ஏற்பாட்டு முறை இரவு நேரப்ப்பணிகளுக்கும் செய்யப்படவேண்டும்.

(d) உபகரணங்கள்.-

(e) பதிவுருக்கள்.-தீவிர சிகிச்சை பிரிவிலுள்ள உள்-நோயாளிகளின் உடல் நலக்குறைவு, அளிக்கப்பட்ட சிகிச்சை, வருகை தந்த மருத்துவ வல்லுநர்கள்-பலன்கள் ஆகியவை பதிவேட்டில் பதிவு செய்யப்பட்டிருக்க வேண்டும்.

(16) மருத்துவமனையில் பேணப்பட வேண்டிய மருத்துவப் பதிவுருக்கள்.-பின்வரும் பதிவுருக்கள் பராமரிக்கப்பட வேண்டும். அவை:

(i) நுழைவு மற்றும் சிகிச்சைக்குப் பின் நோயாளிகள் வீட்டிற்கு திரும்ப அனுப்பப்படுவது குறித்த பதிவேடு (ii) நோய்க்குறிப்புத்தாள்கள்

(iii) பரிந்துரைகள்

(iv) தொற்று மற்றும் பரப்பும் நோய் பற்றிய பதிவேடு

(v) நோய்த்தடுப்பு விவரங்கள்

(vi) குடும்ப நல சேவைகள்TAMIL NADU GOVERNMENT GAZETTE EXTRAORDINARY 37 (vii) சட்டம் சார்ந்த மருத்துவ நேர்வுகள் குறித்த பதிவுருக்கள்

(viii) பிரசவ இறப்புப்பதிவுருக்கள்

(ix) சிசு இறப்புப்பதிவுருக்கள்

(x) 1 முதல் 5 வயது வரையிலான குழந்தை இறப்புப்பதிவுருக்கள்

(xi) அவ்வப்போது அரசால் அறிவுறுத்தப்படும் ஏனைய யாதொரு பதிவுரு.

V மகப்பேறு மருத்துவமனைகள்.-

சிறு மருத்துவ மையங்கள் / மருத்துவமனைகளுக்கு குறிப்பிடப்பட்டுள்ள வசதிகள் / வரையறைகளுடன் பிரசவ அறை மற்றும் அறுவை அரங்கும் விரும்பத்தக்கதாகும். நோயாளர் ஊர்தி அல்லது மத்திய நோயாளர் ஊர்தி சேவையுடன் இணைந்த ஏற்பாட்டு முறை போன்ற நோயாளிகளை ஏற்றிசெல்வதற்கான வசதிகள் விரும்பத்தக்கதாகும்.

VI பல் சம்பந்தப்பட்ட சிகிச்சை மருத்துவ நிலையம்.-

(1) கட்டடம்.-பல் சம்பந்தப்பட்ட சிகிச்சை மேற்கொள்வதற்கான நாற்காலிக்கு போதுமான இடவசதியுடன் 100 சதுர அடிக்கு குறையாத ஆலோசனை அறை இருத்தல் வேண்டும். இந்த அறையானது, நல்ல காற்றோட்டத்துடனும், போதுமான வெளிச்சத்துடனும் இருக்க வேண்டும். தூய்மையாக்கி மற்றும் தட்டுகளை வைப்பதற்கு இடவசதி இருக்க வேண்டும். ஆலோசனை அறைக்கு வெளியே நோயாளிகள் காத்திருப்பதற்கு போதுமான இட வசதி செய்யப்பட்டிருக்க வேண்டும்.

(2) பணியாளர்.-1948 ஆம் ஆண்டு இந்தியப் பல் மருத்துவ சட்டத்தின்படி (மத்தியச் சட்டம் 16/1948), தகுதி வாய்ந்த பல் அறுவை சிகிச்சை மருத்துவரால் மட்டுமே, பல் அறுவை சிகிச்சை, கலந்தாய்வு மற்றும் மருந்துக் குறிப்பு எழுதுதல் போன்றவை மேற்கொள்ளப்பட வேண்டும். ஊசி குத்துதல், மயக்க மருந்து அளித்தல் போன்றவை பல் அறுவை மருத்துவரால் செய்யப்படலாம். ஊடுகதிர்(X-ray) படம், தகுதிவாய்ந்த கதிர்வீச்சுப்பதிவாளரால் (Radiographer) அல்லது பல் மருத்துவரால் எடுக்கப்பட வேண்டும்.

(3) கருவிகள்.-

(a) பல் சிகிச்சை மேற்கொள்வதற்கான நாற்காலி

(b) அறுவை சிகிச்சைக்கான நீர் ஏற்பாடு

(c) தூய்மையாக்கி (Sterilizer)

(d) தூய்மை செய்வதற்கு பயன்படுத்தும் பருத்தி கட்டுத்துணி முதலியவற்றை போடுவதற்கான தொட்டி (e) கருவிகளுடன் கூடிய தட்டுகள்

(f) பல் மருத்துவ சிகிச்சைக்கான கருவிகள்

(g) ஊடுகதிர் (X-ray) (விருப்பத்தின் பேரில்) (எக்ஸ் ரே)

(h) ஒரு முறை மட்டுமே பயன்படுத்தக்கூடிய பீச்சுக்குழாய்கள் (தேவைப்படின்)/அதி அழுத்தக் கொதி அடுப்புடனான பீச்சுக்குழாய்கள். (autoclaved syringes)

(4) பதிவுருக்கள்.-சிகிச்சை அளிக்கப்பட்ட நோயாளிகளின் பெயர், வயது, பாலினம், நோய் மற்றும் அளிக்கப்பட்ட சிகிச்சை போன்ற விவரங்கள் அடங்கிய பதிவுரு வைத்திருக்கப்பட வேண்டும். மேற்சொன்ன விவரங்களுடன் கூடிய குறிப்புச்சீட்டு நோயாளிகளுக்கு வழங்கப்பட வேண்டும்.

VII பல் மருத்துவமனைகள்.-

புற நோயாளிகள் துறை.- பல் மருத்துவமனையின் புற நோயாளிகள் பிரிவு, பல் மருத்துவ மனைக்கு வரையறுக்கப்பட்ட அளவுகோல்களை உறுதிச் செய்ய வேண்டும். இது தவிர, நோயாளிகளுக்கு போதுமான காத்திருப்பு அறைகள் வழங்கப்பட வேண்டும். வார்டுகள், அறுவை அரங்கம் மற்றும் ஏனைய அம்சங்கள், மருத்துவநிலையங்கள் மற்றும் மருத்துவமனைகளுக்கு வரையறுக்கப்பட்ட அளவுகோல்களின்படி இருக்க வேண்டும்.

VIII மருத்துவ ஆய்வகங்கள்.- மருத்துவ ஆய்வகங்கள், நோயியல் ஆய்வகங்கள், மரபியல் ஆய்வகங்கள் போன்றவற்றை அவைகள் உள்ளடக்கியதாகும்.

(1) கட்டடம்.-மாதிரி சேகரிப்பு, முதலுதவி அறை, வழிமுறை செய்தல், அறிக்கை தயாரித்தல் போன்றவற்றிற்கு குறைந்தளவாக, ஊரகப்பகுதியில் 500 சதுர அடி இடமும், நகர்ப்புறப் பகுதியில் 700-1500 சதுர அடி வரையிலான இடமும் இருக்க வேண்டும். நோயாளிகளின் பயன்பாட்டிற்காக, போதுமான தண்ணீர் வசதியுடன், குறைந்தது ஒரு கழிவறையாவது இருக்க வேண்டும். நுண் நோக்கியல் (Microscopy) மற்றும் உயிர் வேதியியல் சோதனைகளுக்காக போதுமான இடவசதி இருக்க வேண்டும். இதற்கான அறை காற்றோட்டமாகவும், போதுமான வெளிச்சத்துடனும் இருக்க வேண்டும். நோயாளிகளின் பயன்பாட்டிற்காக, கட்டில் வைப்பதற்கு போதுமான இடம் இருக்க வேண்டும். ஊடுகதிர் படம் செயல்படுமாயின், ஊடுகதிர் படம் மையத்திற்கான நெறிமுறைகள் கடைபிடிக்கப்பட வேண்டும். சோதனைக் கூடங்கள் பின்வருவன தொடர்பாக, போதுமான இடவசதியுடன் இருப்பதை உறுதி செய்திட வேண்டும்.

(a) நோயாளிகள் வரவேற்பறை

(b) மாதிரி சேகரிப்பு (Sample Collection)38 TAMIL NADU GOVERNMENT GAZETTE EXTRAORDINARY (c) ஆபத்து விளைவிக்கக்கூடிய பொருட்களை தனியாகப்பிரித்து வைத்தல்

(d) அணுசக்தி ஒழுங்குபடுத்தும் வாரியம் (AERB) என்ற முறைப்படுத்தும் முகமையின் தேவைக்கேற்ப, கதிரியக்க ஐசோடோப் தொடர்பான பணி. (Radio isotope)

பணியாளர், கல்வித் தகுதி போன்றவற்றிற்காகவும், ஆய்வகங்கள் பரிசோதனை மற்றும் செயல்பாட்டிற்காக தேசிய அங்கீகரிப்பு வாரியத்தால் குறிப்பிடப்பட்டுள்ள அளவு கோல்களை இந்த ஆய்வகங்கள் வசதி செய்ய வேண்டும். ஆய்வக சேவையில் தர உறுதி, உள்ளார்ந்த தரம் மற்றும் வெளிப்புற தரம் உறுதி செய்யப்பட வேண்டும்.

(2) பணியாளர்.- (a) நோய்க்குறி இயல் நிபுணர் அல்லது பயிற்சி பெற்ற மருத்துவரால், உடல் திசு ஆய்வுச் சோதனை மற்றும் அறிக்கையளித்தல் மேற்கொள்ளப்பட வேண்டும். நுண்ணுயிர் வளர்ப்பு மற்றும் உணர்திறன் சோதனைகள் மேற்கொள்ளப்படும் போது, மருத்துவம் அல்லது மருத்துவம் சாராத நுண்ணுயிரியியலாளர் இருக்க வேண்டும். உயிரணுவியல் அறிக்கைகள் நோய்க்குறியியல் நிபுணரால் மட்டுமே அளிக்கப்பட வேண்டும்.

(b) தொழில்நுட்ப பணியாளருக்கான தகுதிகள்.- தொழில்நுட்பம் சார்ந்த சோதனைகளை மேற்கொள்ளும் பணியாளர் கீழ்க்கண்ட தகுதிகளையும் பெற்றிருக்க வேண்டும், அவையாவன:-

(i) மருத்துவ ஆய்வகத் தொழில்நுட்பத்தில் பட்டப்படிப்பு; அல்லது

(ii) மருத்துவ ஆய்வக தொழில்நுட்பத்தால் குறைந்தளவாக இரண்டாண்டு கால பட்டயப்படிப்பு; அல்லது (iii) மருத்துவ ஆய்வகத் தொழில்நுட்பத்தில் ஓராண்டு கால அளவுடன் கூடிய சான்றிதழுடன் இரண்டாண்டு அனுபவம்; அல்லது

(iv) மருத்துவ ஆய்வகத்தில் ஓராண்டு அனுபவத்துடன் அறிவியலில் பட்டப் படிப்பு; அல்லது

(v) மருத்துவ கதிர்வீச்சு மற்றும் ரேடியோ ஐசோடோப் தொழில்நுட்பத்தில் பட்டயப்படிப்பு (DMRIT); அல்லது (vi) உயிரணுவியல் தொழில்நுட்பவியலாளருக்கான (Cytotechnology) மேற்சொன்ன தகுதியுடன் கூடுதலாக (exfoliative Cytology)-யை கண்டறிவதற்காக, இந்திய உயிரணுவியல் அகாடமியால் உயிரணுவியல் தொழில்நுட்பத்தில் (cytotechnology) கூடுதல் சான்றிதழ்.

(c) மரபணு ஆய்வகத்தில், அனைத்து சோதனைகளும் தகுதி வாய்ந்த மரபணுவியலாளர் அல்லது தகுதி வாய்ந்த மருத்துவரால் மட்டுமே மேற்கொள்ளப்பட்டு அறிக்கையளிக்கப்பட வேண்டும். சட்டத்தால் இசைவளிக்கப்பட்டவாறு, பனிக்குடத்துளைப்பு (amnio centesis) மேற்கொள்ளப்படுமாயின், சிறு மருத்துவ அறுவை சிகிச்சை கூடத்தில் மட்டுமே அது செய்யப்பட வேண்டும். சிறு மருத்துவ அறுவை சிகிச்சைக் கூடத்திற்குத் தேவைப்படக்கூடிய அனைத்து வசதிகளும் இருக்க வேண்டும். கிருமி நீக்கம் செய்யப்பட்ட சூழ்நிலைகளில், தகுதி வாய்ந்த மருத்துவரால் மேற்கொள்ளப்பட வேண்டும்.

(3) கருவிகள்.-

(a) நுண்ணோக்கிகள்

(b) கலோரிமானி (Calorimeter)

(c) மைய நீக்கி (Centrifuge)

(d) சோதனைக்குழாய்கள்

(e) நிழற்பட கருவியில் வைத்துக் காண்பிக்கப்படும் கண்ணாடி வில்லை (Slides)

(f) தேவைப்படும் வினையூக்கி (Reagent)

(g) பாதி தானியங்குப் பகுப்பாய்வி அல்லது தானியங்குப் பகுப்பாய்வி (Auto analyser) (விருப்புரிமை) (h) உயிரணு நுழைவு மையம் (விருப்புரிமை)

(i) இரத்த அணுக்கள் அளவுமானி (Haemoglobinotmeter)

(î) உயிரணுக்களை எண்ணும் அறை (Counting Chamber)

(k) குளிர் சாதனப் பெட்டி

(l) வெப்பக் காற்று அடுப்பு

(m) இயற்பியல் தராசு (Physical Balance)

(n) நிலையான மற்றும் மாறுபடும் அளவுடைய நுண் குழாய்கள் (Micro Pipettes)

(d) பதிவுருக்கள்.- ஆய்வகத்தில் மேற்கொள்ளப்படுகிற அனைத்து சோதனைகளும், பெயர், வயது, பாலினம், மேற்கொள்ளப்பட்ட ஆய்வு குறித்த அறிக்கை நாள் போன்றவற்றுடன் பதிவேட்டில் பதிவுசெய்யப்பட வேண்டும்.

பராமரிக்கப்பட வேண்டிய குறைந்த அளவு பதிவுருக்கள் கீழ்வருமாறு:-

(a) தினசரி சோதனை முடிவுகள் பதிவேடுTAMIL NADU GOVERNMENT GAZETTE EXTRAORDINARY 39 (b) உள்ளார்ந்த தரக் கட்டுப்பாட்டுப் பதிவேடு

(c) மாதிரி சேகரிப்புப் பதிவேடு (Sample Collection Register)

(d) வெளிப்புற தர மதிப்பீடு திட்ட நிகழ்ச்சிகளில் பங்கேற்பு

IX. எக்ஸ்ரே (X-ray) மையங்கள்.-

இம்மையம், மீயொலி வரைவி (imaging) ஸ்கேன் மையங்கள், சி.டி.ஸ்கேன் மையங்கள், எம்.ஆர்.ஐ.ஸ்கேன் மையங்கள், ஊடுகதிர்கள் (X-ray) ஈ.சி.ஜி,ஈ.ஈ.ஜி மற்றும் ட்ரெட்மில் எக்கோகார்டியோகிராபி ஆகிய மீயொலி வழி இயல் நிலை வரைவி மையங்களை (Ultra Sonogram Scan Centers) உள்ளடக்கியதாகும்.

1. கட்டடங்கள்.- பயன்பாட்டிலுள்ள கருவிகளுக்கும் போதுமான அளவு இட வசதி இருக்க வேண்டும். தேவைப்படும் போது, கடும் ஒவ்வாமை ஏற்படுகின்ற நேர்விலும், முரண்பாடான ஊசி (Contrast Injection) மருந்தை செலுத்தும் போது அல்லது மிதிசெக்குருளை (Tread Mill) சோதனை செய்த பின்னர் நோயாளிகள் ஓய்வெடுக்க வேண்டுமென்று விரும்புகிற போது அவர்கள் ஓய்வெடுப்பதற்கும் , பழைய நிலைமையில் மீண்டும் திரும்புவதற்கும் அதற்கு போதுமான அளவு இடவசதிக்கு வகை செய்யப்பட வேண்டும். ஊடு கதிர் மற்றும் மீயொலி வழி இயல் நிலை வரைவி "AERB" (Atomic Energy Regulatory Board) மையங்கள், அணு சக்தி ஒழுங்குப்படுத்தும் வாரியத்தின் வரையறைகளை நிறைவு செய்ய வேண்டும்.

2. பணியாளர்கள்.- அனைத்து சோதனைகளும் பின் வரும் தகுதிவாய்ந்த தொழில்நுட்பப்பணியாளர்களால் மட்டுமே மேற்கொள்ளப்பட வேண்டும்:-

3. மருத்துவ உபகரணங்கள்.-தொடர்புடைய இயந்திரத்திற்குத் தேவையானவாறான மருத்துவ உபகரணங்கள்

4. பதிவுருக்கள்.- நோயாளியின் பெயர், வயது, பாலினம், முகவரி, மருத்துவர் பரிசோதனை மேற்கொள்வதற்குப் பரிந்துரைத்த ஆய்வின் தன்மை, பரிசோதனையின் முடிவு ஆகிய தகவல்கள் ஒரு பதிவேட்டில் இடம்பெற்றிருக்க வேண்டும். பெயர், வயது, முகவரி மற்றும் பரிசோதனையின் முடிவு ஆகியவற்றுடன் கட்டணமின்றி சிகிச்சையளிக்கப்பட்ட நோயாளிகளின் எண்ணிக்கை குறித்த விவரங்களும் இப்பதிவேட்டில் இடம்பெற்றிருக்க வேண்டும்.

B. இந்திய மருத்துவ முறை மற்றும் ஓமியோபதி

I. மருத்துவ ஆலோசனை அறை / மருத்துவ மையம் / பல்வகை நோய்களுக்கு சிகிச்சை அளிக்கும் மருத்துவமனைகள்

1. கட்டடம்.-ஆலோசனை வழங்கும் அறை, நல்ல காற்றோட்டத்துடனும் போதிய வெளிச்சத்துடனும் இருக்க வேண்டும். அதற்கான இடம் 100 சதுர அடிக்குக் குறையாமல் இருக்க வேண்டும். நோயாளிகள் காத்திருப்பதற்குப் போதுமான இடவசதி இருக்க வேண்டும். பல்வகை நோய்களுக்கு சிகிச்சை அளிக்கும் மருத்துவமனையாக இருந்தால், ஒவ்வொரு மருத்துவருக்கும் தனித்தனி அறை இருக்க வேண்டும். பார்வையிடும் பல்வகை மருத்துவர்களின் பெயர்கள் மற்றும் அவர்களின் மருத்துவ முறை குறித்தத் தகவல் மருத்துவ மையத்தின் முன்பு பார்வைக்கு வைக்கப்பட வேண்டும்.

2. பணியாளர்கள்.-ஆயுர்வேதா, யோகா மற்றும் இயற்கை மருத்துவம், யுனானி, சித்தா மற்றும் ஓமியோபதி மருத்துவ மையங்கள் பதிவுசெய்யப்பட்ட மருத்துவப் பயிற்சியாளர்களால் நிர்வகிக்கப்பட வேண்டும். மருத்துவ மையத்தின், மருந்தகம் இணைக்கப்பட்டிருந்தால், முறையான அமைப்பின் கீழ் தகுதிபெற்ற மருந்தாளுநர் அல்லது அம்மருத்துவர் தாமாகவே, மருந்துகளை வழங்க வேண்டும்.

3. மருத்துவ உபகரணங்கள்.- (a) அனைத்து `ஆயுஷ்', இந்திய மருத்துவமுறை மற்றும் ஓமியோபதி மற்றும் யோகா மற்றும் இயற்கை மருத்துவ மையங்களுக்கும் பொதுவாகத் தேவைப்படும் நோயைக் கண்டறியும் மருத்துவ உபகரணங்கள்:

(i) தெர்மோமீட்டர் / வெப்பமானி (காய்ச்சல் மானி) (Thermometer)

(ii) ஸ்பிக்மோமானோமீட்டர் / இரத்த அழுத்தமானி

(iii) இருதய துடிப்பை ஆய்வு செய்யும் கருவி (Stethescope)

(iv) மூட்டு சோதனைக்கான சுத்தி (Knee Hammer)

(v) நாக்கை அழுத்தி (Tongue depressor)

(vi) டார்ச் / கிரிடோஸ்கோப் லென்சுடைய டார்ச் (Torch with Iridoscope lens)

(vii) எடைப் பார்க்கும் இயந்திரம் / உயரம் பார்க்கும் அளவை

4. மருந்துகள்.-நோயாளிகளுக்கு வழங்கப்படும் மருந்துகளின் மீது, மருந்தின் பெயர், இம்மருந்து அளிக்கப்பட்ட நோயாளியின் பெயர், கொடுக்கப்பட வேண்டிய அளவு போன்ற தகவல்கள் அடங்கிய முகப்புச்சீட்டு ஒட்டப்பட்டிருக்க வேண்டும். வழங்கப்படும் மருந்திற்குக் பயன்பாட்டுக் காலம் முடிவுறும் நாள் இருப்பின், அந்த நாள் முகப்புச் சீட்டில் குறிப்பிடப்பட்டிருக்க வேண்டும். உட்கொள்ள வழங்கப்படும் மருந்து மற்றும் வெளிப் பயன்பாட்டிற்கான மருந்துகளில் முறையே, வெள்ளை மற்றும் சிவப்பு வண்ணப் பட்டிகளில் "உள் பயன்பாட்டிற்கு" அல்லது "வெளிப் பயன்பாட்டிற்கு" என வட்டார மொழிகளில் தெளிவாகக் குறிப்பிடப்பட்டிருக்க வேண்டும்.

5. பதிவுருக்கள்.-நோயாளிகளின் பெயர், வயது, பாலினம், நோய்காண் விவரங்கள் மற்றும் சிகிச்சை விவரங்களைக் கொண்ட அனைத்தும் நோயாளிகளின் மருத்துவ சிகிச்சை பதிவுருக்கள் இருக்க வேண்டும். நோயாளியின் பெயர், வயது, பாலினம் மற்றும் சிகிச்சையளிக்கப்படும் விவரங்களைக் கொண்ட துண்டுசீட்டு ஒன்று நோயாளியிடம் வழங்கப்பட வேண்டும்.

II. சிகிச்சை மையங்கள் மற்றும் மருத்துவமனைகள்.-

1. கட்டடம்.- ஆங்கில வழி மருத்துவ முறையில் குறிப்பிடப்பட்டிருக்கும் வரையறைகளை நிறைவு செய்யும் வகையில், மருத்துவ ஆலோசனை அறை அல்லது புற நோயாளிகள் பிரிவு இருக்க வேண்டும். நோயாளிகள் காத்திருப்பதற்குப் போதுமான இடமும், வெவ்வேறு மருத்துவர்கள் பணி புரிவதற்கான தனித்தனி அறைகளும் இருக்க வேண்டும். மருத்துவர்களின் பெயரும் அதனைத் தொடர்ந்து அவர்கள் அளிக்கும் சிறப்பு சிகிச்சை முறைகள் மற்றும் அந்த மருத்துவமனையிலுள்ள பல்வேறு வசதிகள் / சேவைகள் கொண்ட விவர பலகை ஒன்றில் குறிப்பிடப்பட்டிருக்க வேண்டும்.

2. நோயாளி அறைகள் (Wards) அல்லது உள்நோயாளிகளுக்கான அறைகள்.-உள்நோயாளிகளுக்கான அறைகள் நல்ல காற்றோட்டத்துடனும் போதிய வெளிச்சத்துடனும் இருக்க வேண்டும். ஒவ்வொரு அறையிலும் மின்விசிறி வசதி செய்யப்பட்டிருக்க வேண்டும். பொது அறைகளில் குறைந்தபட்சம் நான்கு படுக்கைகளுக்கு ஒரு மின்விசிறி வசதி செய்யப்பட்டிருக்க வேண்டும். கழிவறை வசதி பொதுவாகவோ அல்லது தனித்தனிக் கழிவறை வசதியோ செய்யப்பட்டிருக்கலாம். ஆனால் தூய்மையாகவும் குழாய் நீர் வசதியுடனும் பராமரிக்கப்பட வேண்டும். கிருமிநாசினியினைப் பயன்படுத்தி அன்றாடம் நோயாளி அறைகளை துடைத்து சுத்தம் செய்வதற்குரிய ஏற்பாடுகள் செய்யப்பட்டிருக்க வேண்டும்.

3. பணியாளர்கள்.-(a) மருத்துவர்கள்.- நோயாளிகளுக்குச் சிகிச்சையளிப்பதற்கு, பதிவுசெய்யப்பட்ட மருத்துவர் மட்டுமே மருத்துவமனைகளில் பணியமர்த்த வேண்டும். மருத்துவரும் அவருக்குரிய மருத்துவ முறையின் கீழ்வரும் மருத்துவ மன்றத்தில் அல்லது இந்திய முறை மருத்துவத்திற்கான மன்றத்தில் பதிவு செய்திருக்க வேண்டும்.

(b) செவிலியர்.- 30 படுக்கைகள் வரையில் ஒரு பதிவு பெற்ற செவிலியர் செவிலியர் மற்றும் இரண்டு செவிலிய உதவியாளர்கள் என்ற வீதத்தில் தகுதி வாய்ந்த செவிலியர் ஒருவர் இருக்க வேண்டும்.

(c) மருந்தாளுநர் / சிகிச்சையளிப்பவர் (Therapist).- மருந்துகள் வழங்குவதற்காக, அவர்களுக்குரிய மருத்துவ முறையில் தகுதிபெற்ற மருத்துவர் / சிகிச்சையளிப்பவர் நியமனம் செய்யப்பட வேண்டும். மருத்துவமனையில் 30 படுக்கைகளுக்கு மேல் இருந்தால் மருந்துகளை வழங்குவதற்காக, அந்தந்த மருத்துவ முறையில் தகுதிபெற்ற மருந்தாளுநர் / சிகிச்சையளிப்பவர் பணியமர்த்தப்பட வேண்டும். 30 எண்ணிக்கைக்கும் குறைவாக படுக்கைகள் இருந்தால் மருத்துவரே மருந்துகளை வழங்கலாம் அல்லது தகுதிவாய்ந்த மருந்தாளுநர் / சிகிச்சையளிப்பவர் ஒருவரைப் பணியமர்த்தலாம்.

(d) பிற பணியாளர்கள்.- மஸ்தூர் போன்ற பிற பணியாளர்கள் தேவைக்கேற்ப பணியமர்த்தப்பட வேண்டும். ஒவ்வொரு 30 படுக்கைகளுக்கு குறைந்தபட்சம் ஒரு துப்புரவுப் பணியாளர் என்றளவில் போதுமான எண்ணிக்கையில் துப்புரவுப் பணியாளர்கள் இருக்க வேண்டும்.

4. மருத்துவ சாதனங்கள்.- (a) அனைத்து இந்திய முறை மருத்துவமனைகளுக்கும் கீழ்க்கண்ட உடற்பயிற்சி சார்ந்த சிகிச்சை சாதனங்கள் தேவைப்படுகின்றன அவை:-

(i) இழுவை சாதனம் (Traction Kit)

(ii) நடைப் பயிற்சி சாதனம். (Walker)

(iii) அகச்சிவப்பு கதிர் உமிழ் சாதனம் (Infrared light)

(iv) சிந்தனைப்புலசிகிச்சை (TFT) மற்றும் மீயொலி சிகிச்சை சாதனம். (Ultrasound therapy instrument) (v) அதிர்வியற்றி (கைகளால் இயக்கப்படும் கருவி மற்றும் மின்னணு சாதனம்)

(b) அனைத்து ஆயுர்வேதம்/சித்தா முறை மருத்துவமனைகளுக்கு பொதுவாக கீழ்கண்ட மருத்துவ சாதனங்கள் தேவைப்படுகின்றன அவை.-

6. உள்கட்டமைப்பு.- வெளிநோயாளிகளுக்கு சிகிச்சையளிப்பதற்கு மருத்துவ ஆலோசனை வகையினங்களும், குறைந்தளவு அதற்கான சாதனங்களும் மருத்துவ மையத்தில் இருக்க வேண்டும். பல்வேறு நோய்களுக்கான பசுமை சரி உணவு,(Bland Diet) இயற்கை சரி உணவு, மூலிகைச் சாறு ஆகியவற்றை தயாரிப்பதற்கும் ஏனைய சிறப்பு சரிஉணவு சிகிச்சையளிப்பதற்கும் கூடிய சரிஉணவு மையத்துடன் அனைத்து சாதனங்களும் மருத்துவமனையில் இருக்க வேண்டும். மேற்சொன்னவற்றுடன் கூடுதலாக முதல் உதவி மற்றும் அவசரகால மேலாண்மை தேவைகளும் வழங்கப்பட வேண்டும். ஒவ்வொரு முப்பது படுக்கைகளுக்கும் குறைந்தளவு ஒன்று என்றளவில், சக்கர நாற்காலிகளும் தூக்குப்படுக்கைகளும் போதுமான எண்ணிக்கையில் இருக்க வேண்டும்.

7 பதிவுருக்கள்.- (1) அனுமதிக்கப்பட்ட / சிகிச்சையளிக்கப்பட்ட அனைத்து நோயாளிகள் தொடர்பான பரிசோதனை அறிக்கைகள், நோய்கண்டறிதல், சிகிச்சையளித்தல் ஆகிய விவரங்களைக் கொண்ட ஒரு பதிவுரு பராமரிக்கப்பட வேண்டும். உடற்பரிசோதனைகள், நோய்கண்டறிதல் சிகிச்சையளித்தல் மற்றும் தொடர்ந்து பின்பற்றவேண்டிய சிகிச்சை முறைகள் முதலிய விவரங்கள் அடங்கிய விடுவிப்புச் சீட்டினை மருத்துவமனையிலிருந்து வெளியே செல்லும்போது நோயாளிக்கு வழங்க வேண்டும்.

(2) பின்வரும் பதிவேடும் பராமரிக்கப்பட வேண்டும் நோயாளியின்.கோரிக்கையின்பேரில் அவருடன் தொடர்புடைய அனைத்து விவரங்களையும் வழங்குவது மருத்துவ நிலையத்தின் கடமையாகும்.

(i) அனுமதி மற்றும் உள்சிகிச்சை விடுவிப்புப் பதிவேடு

(ii) பரவக் கூடிய மற்றும் தொற்றக்கூடிய வியாதிகள் குறித்த பதிவேடு.

(iii) சட்டம் சார்ந்த மருத்துவ சிகிச்சையாளர் பதிவேடு எந்தவொரு கட்டத்திலும், சிகிச்சைக்காக சட்டம் சார்ந்த மருத்துவ சிகிச்சையாளர்கள் அனுமதிக்கப்படும் நேர்வில்)

மாவட்டக் குழுவால் சரிபார்க்கப்படுவதற்காக முழு முகவரியுடன் ஆவணங்கள் தயாராக வைத்திருக்க வேண்டும்.

C. அலோபதி மற்றும் ஆயுஷ் ஆகியவற்றின் கீழ், செயல்படும் அனைத்து மருத்துவமனைகளும் பூர்த்தி செய்யப்பட வேண்டிய பொதுவான

வரையறைகள்.

1. தகவல் தொடர்பு.- நோயாளிகள் பயன்படுத்துவதற்காக ஒரு தொலைபேசி இணைப்பு வசதி இருக்க வேண்டும்.(கட்டணம் செலுத்துவதன் பேரில்)

2. பாதுகாப்பு.- உள்நோயாளிகளாக தங்கியிருப்பவர்களின் பாதுகாப்பிற்காகவும் திருட்டை தடுப்பதற்காகவும் போதுமான பாதுகாப்பு வசதி வழங்கப்பட வேண்டும்.

3. தீயணைப்பு வசதி.- மருத்துவமனையில், விதிகளின்படி, இந்திய தர நிறுவன முத்திரையுடன் கூடிய தீயணைப்பு சாதனங்கள் இருக்க வேண்டும்.

4. சமையலறை.- உள்-நோயாளிகளுக்கு உணவு வழங்கப்படுமாயின், சமையலறை தூய்மையாக இருக்க வேண்டும் மற்றும் சமையல்காரருக்கு/ சமையல்காரர்களுக்கு தொற்றக்கூடிய அல்லது பரவக்கூடிய நோய்கள் குறித்து குறிப்பிட்ட கால இடைவெளிகளில், மருத்துவப் பரிசோதனை செய்யப்பட வேண்டும்.

5. துணி மற்றும் நார்த்துணி (Linen) . அவை தூய்மையாக பராமரிக்கப்பட வேண்டும் மற்றும் அன்றாடம் மாற்றப்பட வேண்டும்.

6. குடிநீர் வழங்கல்.- நோயாளிக்கு குடிக்கத்தக்க நல்ல தண்ணீர் வழங்கப்பட வேண்டும்.

7. கழிவுகளை அப்புறப்படுத்துதல்.- மத்திய அரசின் வரையறைகள் மற்றும் தமிழ்நாடு மாசுக் கட்டுப்பாட்டு வாரியத்தின் வழி முறைகளையும் பின்பற்றி இது செய்யப்பட வேண்டும்.

8. பதிவுரு பராமரிப்பு.- (a) ஒவ்வொரு மருத்துவ நிலையமும், பணியாளர்கள் விவரங்கள் தொடர்பான நிலையான ஆவணங்கள் மற்றும் நோயாளிகள் தொடர்பான மருத்துவ சிகிச்சை விவரங்கள் கொண்ட நிரந்தர ஆவணங்களை பராமரித்து வர வேண்டும். உரிய அலுவலரால் அல்லது இது தொடர்பாக அதிகாரமளிக்கப்பட்டுள்ள வேறுஒரு அலுவலரால் ஆய்வு செய்யப்படுவதற்காக அந்த பதிவுருக்கள் தயாராக இருக்க வேண்டும்.

(b) ஒவ்வொரு தனியார் மருத்துவமனையும், நோயாளி தொடர்பான மருத்துவ கண்காணிப்பு, சிகிச்சை போன்ற விவரங்களின் நகலினை, குறிப்பிட்ட காலத்திற்குள், தேவையான கட்டணங்களை செலுத்தியதின் பேரில் வழங்க வழிவகை செய்திருக்க வேண்டும். அத்தகைய தகவல்களைப் பெறுவதற்கான கட்டண விவரங்களை, அனைவரும் அறியக் கூடிய இடத்தில் ஒவ்வொரு மருத்துவமனை நிருவாகமும் காட்சிப்படுத்த வேண்டும்.

(c) அத்தகைய தகவல்கள், சிகிச்சை பெற்ற நபருக்கோ அல்லது அவருடைய குடும்ப உறுப்பினர்களுக்கோ தீங்கு ஏற்படுவதற்கு காரணமாக அமையும் என கருதினால், அல்லது ஒரு அரசு அதிகார அமைப்பின் உத்தரவின் பேரில் சிகிச்சை அளிக்கப்பட்டிருந்தால், அத்தகைய தகவலைத் தெரிவிப்பதற்கு ஒவ்வொரு மருத்துவ நிலையமும் மறுக்கலாம்.

1. ----------முறைப்படி மருத்துவ சிகிச்சையளிப்பதற்காக ----------முகவரியில் செயல்பட்டுவரும் (முழு முகவரி) ----------படுக்கைகளுடன் கூடிய ----------மருத்துவமனையை 1997 ஆம் ஆண்டு தமிழ்நாடு மருத்துவ நிறுவனங்கள் (முறைப்படுத்துதல்) சட்டத்தின் காப்புரைகளின் படி,-----------(மருத்துவமனையின் பெயர்) முறை மருத்துவமனையாக இதனால் பதிவு செய்யப்படுகிறது.

2. இப்பதிவுச் சான்றிதழ் 1997 ஆம் ஆண்டு தமிழ்நாடு மருத்துவ நிறுவனங்கள் (முறைப்படுத்துதல்) சட்டம் மற்றும் 2018 ஆம் ஆண்டு தமிழ்நாடு மருத்துவ நிறுவனங்கள் (முறைப்படுத்துதல்) விதிகளில் வரையறுக்கப்பட்டுள்ள நிபந்தனைகளுக்குட்பட்டதாகும்.
