चू ँकि सेवाएं या प्रसुविधाएं या राज्य सहायता प्रदान करने के लिए पहचान दस्तावेजों के रूप में आधार का उपयोग किये जाने से सरकारी परिदान प्र(िcid:7059)याओं में सुगमता होती है, पारद(cid:7414)शता एवं दक्षता आती है और लाभार्थी इस बात के लिए समर्थ हो जाते हैं कि वे किसी (cid:7390)क्ति की पहचान सि(cid:7373) करने के लिए ब(cid:7263)विकल्पीय दस्तावेज प्रस्तुत करने की आवश्यकता का निराकरण करते (cid:7263)ए सुविधापूर्वक और अबाध रूप से सीधे अपना हक प्राप्त कर सकें ।
और, चू ँकि, उत्तर प्रदेश सरकार का बाल विकास सेवा एवं पुष्टाहार विभाग, एकीकृ त बाल विकास सेवा (ICDS) योजना (आंगनवाड़ी सेवाएं), के अन्तर्गत ब(cid:6994)चों (0 से 6 वर्ष) और गर्भवती महिलाओं तथा धात्री माताओं के लिए एक सार्वभौमिक स्व-चयन योजना के रूप में संचालित कर रहा है; राज्य सरकार द्वारा सम्पूर्ण राज्य में (िcid:7059)यान्वित की जाने वाली जो छः सेवाएं आंगनवाड़ी कें (cid:7076)ों के माध्यम से प्रदान की जाती हैं अर्थात् (1) अनुपूरक पोषाहार, (2) अनऔपचारिक प्री-स्कू ल शिक्षा, (3) पोषण और स्वास्थ्य शिक्षा, (4) टीकाकरण, (5) स्वास्थ्य जांच और (6) रेफरल सेवाएं और इन सेवाओं में से टीकाकरण, स्वास्थ्य जांच और रेफरल सेवाएं स्वास्थ्य से संबंधित हैं और राष्ट्रीय स्वास्थ्य मिशन और सार्वजनिक स्वास्थ्य बुनियादी ढांचे द्वारा प्रदान की जाती हैं,
और, चू ँकि, पूव(cid:7299)क्त अनुपूरक पोषाहार कार्य(cid:7059)म, जो आंगनवाड़ी के न्(cid:7076)ों पर प्रदान किया जा रहा है, का, आवर्ती (cid:7390)य उत्तर प्रदेश की समेकित निधि से उपगत हैः 2 उार प्रदेश असाधारण गजट, 18 नवम्बर, 2020 अतएव, अब, आधार (वित्तीय एवं अन्य राज्य सहायता प्रसुविधाओं तथा सेवाओं का लक्षित परिदान) अधिनियम, 2016 (अधिनियम संख्या 18 सन् 2016) (जिसे आगे उक्त अधिनियम कहा गया है) की धारा 7 के उपबंधों के अनुसरण में, उत्तर प्रदेश सरकार एतद्द्वारा निम्नलिखित को अधिसूचित करती है, अर्थात्-
1-(1)-आंगनवाड़ी के न्(cid:7076)ों पर प्रदत्त अनुपूरक पोषाहार कार्य(cid:7059)म की उपलब्धता के लिये इ(cid:6994)छु क (cid:7018)यक्तियों से आधार संख्या धारित करने का सबूत उपलब्ध कराने या आधार अभिप्रमाणन कराने की अपेक्षा की जायेगी।
(2)- आंगनवाड़ी के न्(cid:7076)ों पर प्रदत्त अनुपूरक पोषाहार कार्य(cid:7059)म की उपलब्धता के लिये इ(cid:6994)छु क किसी (cid:7018)यक्ति, जो आधार संख्या धारित न करता हो अथवा अभी तक आधार के लिए नामांकन न किया हो, से उक्त योजना हेतु रजिस्ट्रीकरण करने के पूर्व अपने माता-पिता या संरक्षक की सहमति के अध्यधीन आधार नामांकन हेतु आवेदन करने की अपेक्षा की जायेगी, परन्तु यह कि वह उक्त अधिनियम की धारा 3 के उपबंधों के अनुसार आधार प्राप्त करने के लिए हकदार हो और ऐसे ब(cid:6994)चों को आधार नामांकन कराने हेतु किसी आधार नामांकन के न्(cid:7076) पर जाना होगा। (आधार नामांकन के न्(cid:7076) की सूची भारतीय विशिष्ट पहचान प्राधिकरण की वेबसाइट www.uidai.gov.in पर उपलब्ध है)।
(3)-आधार (नामांकन और अद्यतन) नियमावली, 2016 के विनियम 12 के अनुसार विभाग द्वारा अपनी कार्यान्वयन एजेन्सी (अभिकरण) के माध्यम से लाभा(cid:7414)थयों के लिए आधार नामांकन सुविधाएं प्रदान करना आवश्यक है जो अभी तक आधार के लिए नामांकित नह(cid:7277) हैं और यदि कोई आधार नामांकन के न्(cid:7076), सम्बन्धित ब्लाक (विकास ख(cid:7003)ड) अथवा तालुका या तहसील में स्थित नह(cid:7277) है तो अपनी कार्यान्वयन एजेन्सी (अभिकरण) के माध्यम से विभाग को भारतीय विशिष्ट पहचान प्राधिकरण (यू0आई0डी0ए0आई0) के विद्यमान रजिस्ट्रार के साथ या स्वयं भारतीय विशिष्ट पहचान प्राधिकरण रजिस्ट्रार बनकर समन्वय में सुविधाजनक स्थानों पर आधार नामांकन सुविधायें प्रदान करेगा ः
परन्तु जब तक ब(cid:6994)चे को आधार समनुदेशित नह(cid:7277) किया जाता है, तब तक उक्त योजना के अधीन लाभ ऐसे ब(cid:6994)चों को दिये जायेंगे जो निम्नलिखित दस्तावेजों की प्रस्तुति के अध्यधीन हैं, अर्थात्:-
(क) यदि ब(cid:6994)चे का पांच वर्ष की आयु (बायोमेट्रिक्स सं(cid:7061)हण के साथ) प्राप्त करने के बाद नामांकन किया गया है, तो उसकी आधार नामांकन पहचान पर्ची, या बायोमेट्रिक अपडेट पहचान पर्ची और (ख) निम्नलिखित में से कोई भी, यथा: -
(1) जन्म प्रमाणपत्र; या उपयुक्त प्राधिकारी द्वारा जारी किए गए जन्म का अभिलेख, या (2) स्कू ल के प्रधानाचार्य द्वारा विधिवत हस्ताक्षरित स्कू ल का पहचान पत्र, जिस पर माता-पिता के नाम अंकित हों, तथा
(ग) निम्न दस्तावेजों में से कोई एक, माता-पिता या विधिक संरक्षक के साथ लाभार्थी के संबंध के प्रमाण के रूप में, जो मौजूदा योजना के मार्गदर्शी सि(cid:7373)ान्तों के अनुसार है, अर्थात्: -
(1) जन्म प्रमाणपत्र, या उपयुक्त प्राधिकारी द्वारा जारी किए गए जन्म का अभिलेख, या (2) राशन कार्ड, या
(3) भूतपूर्व सैनिक अंशदायी स्वास्थ्य योजना (ECHS) कार्ड, या कर्मचारी राज्य बीमा निगम (ESIC) कार्ड, या कें (cid:7076) सरकार स्वास्थ्य योजना (सीजीएचएस) कार्ड, या
(4) पेंशन कार्ड, या
(5) आर्मी कैं टीन कार्ड, या
(6) कोई भी सरकारी परिवार हक प्रवेश पत्र, या
(7) विभाग द्वारा यथा विनि(cid:7416)दष्ट कोई अन्य दस्तावेजः
परन्तु यह और कि उपरोक्त दस्तावेजों को इस प्रयोजनार्थ राज्य सरकार द्वारा विनि(cid:7416)दष्ट रूप से अभिहित किसी अधिकारी द्वारा जांचा जा सकता है।
2- योजना के अधीन लाभा(cid:7414)थयों को सुविधाजनक रूप में प्रसुविधाएं प्रदान करने के लिए इसके कार्यान्वयन अभिकरण के माध्यम से विभाग यह सुनि(िcid:7391)त करने के लिए समस्त आवश्यक (cid:7390)वस्था करेगा कि मीडिया के माध्यम से लाभा(cid:7414)थयों को उक्त आवश्यकता से अवगत कराने के लिए (cid:7390)ापक प्रचार किया जायेगा। उत्तर प्रदेश असाधारण गजट, 18 नवम्बर, 2020 3 3- समस्त मामलों में जहाँ आधार प्रमाणीकरण लाभा(cid:7414)थयों के खराब बायोमेट्रिक्स के कारण या अन्य किसी कारण से विफल हो जाता है, निम्नलिखित उपचारात्मक तंत्र को अपनाया जायेगा, अर्थात्-
(क) खराब (cid:7412)फगर प्रिट गुणवत्ता के मामले में प्रमाणीकरण के लिए स्कै न या फे स प्रमाणीकरण सुविधा अपनायी जायेगी। विभाग अपने (िcid:7059)यान्वयनकर्ता अभिकरण के माध्यम से अबाध रीति से प्रसुविधाओं के परिदान के लिए स्कै नर या फे स प्रमाणीकरण के साथ-साथ (cid:7412)फगर प्रिट प्रमाणीकरण के लिए उपबंध करेगा।
(ख) यदि (cid:7412)फगर प्रिट या एकीकृ त जोखिम सूचना प्रणाली स्कै न या फे स प्रमाणीकरण के माध्यम से बायोमेट्रिक प्रमाणीकरण सफल नह(cid:7277) है तो जहां तक सम्भव हो, यथास्थिति आधार वन टाइम पासवर्ड या टाइम-बेस्ड वन टाइम पासवर्ड द्वारा सीमित समय वैलीडिटी के साथ अनु(cid:6980)ेय अधिप्रमाणन की पेशकश की जायेगी।
(ग) अन्य सभी मामलों में जहां बायोमेट्रिक या आधार वन टाइम पासवर्ड या टाइम-आधारित वन-टाइम पासवर्ड प्रमाणीकरण संभव नह(cid:7277) है, योजना के अधीन लाभ, भौतिक आधार-पत्र के आधार पर दिए जा सकते हैं, जिनकी प्रामाणिकता को आधार पत्र पर मु(िcid:7076)त त्वरित प्रति(िcid:7059)या कोड के माध्यम से सत्यापित किया जाएगा और (िcid:7332)क रेस्पॉन्स कोड रीडर की आवश्यक (cid:7390)वस्था विभाग द्वारा अपनी कार्यान्वयन एजेंसी के माध्यम से सुविधाजनक स्थानों पर प्रदान की जाएगी।
4-उपयु र्क्त में अन्त(cid:7414)वष्ट किसी बात के होते (cid:7263)ए भी, किसी भी ब(cid:6994)चे को प्रमाणीकरण के माध्यम से अपनी पहचान स्थापित करने, या आधार नंबर रखने के सा(cid:7023)य प्रस्तुत करने या ब(cid:6994)चे के मामले में जिसे कोई आधार संख्या नह(cid:7277) स(cid:7301)पी गई है, नामांकन के लिए आवेदन प्रस्तुत करने में विफल होने की स्थिति में योजना के अधीन लाभ से वंचित नह(cid:7277) किया जायेगा। इसका लाभ प्रस्तर-1 के उप-प्रस्तर (3) के परन्तुक के खंड (ख) और (ग) में यथाउिल्लखित अन्य दस्तावेजों के आधार पर उसकी पहचान को सत्यापित करके दिया जाएगा, और जहां लाभ इस प्रकार के अन्य दस्तावेजों के आधार पर दिया गया है वहाँ इस प्रकार के अन्य दस्तावेजों को रिकॉर्ड करने के लिए, एक अलग रजिस्टर रखा जाएगा, जिसकी विभाग द्वारा समय-समय पर समीक्षा और लेखा-परीक्षा की जाएगी।
5-यह अधिसूचना गजट में प्रकाशित किये जाने के दिनांक से प्रवृत्त होगी।

S.O. ……(E):– WHEREAS, the use of Aadhaar as identity document for delivery of services or benefits or subsidies simplifies the Government delivery processes, brings in transparency and efficiency, and enables beneficiaries to get their entitlements directly to them in a convenient and seamless manner by obviating the need for producing multiple documents to prove one's identity;
AND, WHEREAS, the Bal Vikas Seva Evam Pushtahar Department in the Government of Uttar Pradesh is administering Integrated Child Development Services (ICDS) Scheme [Anganwadi Services] as a universal self-selecting Scheme to children (0 to 6 years) and pregnant women and lactating mothers which is implemented by the State Government through the Anganwadi Centres spread across the State and it offers six services, namely, (i) Supplementary Nutrition; (ii) Pre-School non-formal education; (iii) Nutrition and Health Education; (iv) Immunization; (v) Health check-up and (vi) Referral services, and out of these services the immunization, health check-up and referral services are related to health and are provided by National Health Mission and public health infrastructure;
AND, WHEREAS, the aforesaid Supplementary Nutrition Program offered at Anganwadi Centres involves recurring expenditure incurred from the Consolidated Fund of Uttar Pradesh;
4 उार प्रदेश असाधारण गजट, 18 नवम्बर, 2020NOW, THEREFORE, in pursuance of the provisions of section 7 of the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2016 (Act no. 18 of 2016) (hereinafter referred to as the said Act), the Uttar Pradesh Government hereby notifies the following, namely:–
1. (1) Individuals desirous of availing the Supplementary Nutrition Program offered at the Anganwadi Centres are required to furnish proof of possession of Aadhaar number, or undergo Aadhaar authentication.
(2) Any individual desirous of availing the Supplementary Nutrition Program offered at the Anganwadi Centres, who does not possess the Aadhaar number or, has not yet enrolled for Aadhaar, shall be required to make application for Aadhaar enrolment subject to the consent of his parents or guardians, before registering for the Scheme provided that he is entitled to obtain Aadhaar as per the provisions of section 3 of the said Act and such children shall visit any Aadhaar enrolment centre (list available at UIDAI website www.uidai.gov.in) for Aadhaar enrolment.
(3) As per regulation 12 of the Aadhaar (Enrolment and Update) Regulations, 2016, the Department through its Implementing Agency, is required to offer Aadhaar enrolment facilities for the beneficiaries who are not yet enrolled for Aadhaar and in case there is no Aadhaar enrolment centre located in respective Block or Taluka or Tehsil, the Department though its Implementing Agency shall provide Aadhaar enrolment facilities at convenient locations in co-ordination with the existing Registrar of UIDAI or by becoming a UIDAI Registrar themselves:
Provided that till the time Aadhaar is assigned to the child, the benefit under the Scheme shall be given to such children subject to production of the following documents, namely:-
(a) if the child has been enrolled after attaining the age of five years (with bio-metrics collection), his Aadhaar Enrolment Identification slip; or bio-metric update identification slip, and (b) any one of the following, namely: –
(i) Birth Certificate; or Record of birth issued by the appropriate authority; or
(ii) School identity card, duly signed by the Principal of the School, containing parents' names; and
(c) any one of the following documents as proof of relationship of the beneficiary with the parent or legal guardians as per the extant Scheme guidelines, namely :-
(i) Birth Certificate; or Record of birth issued by the appropriate authority; or
(ii) Ration Card; or
(iii) Ex-Servicemen Contributory Health Scheme (ECHS) Card; or Employees State Insurance Corporation (ESIC) Card; or Central Government Health Scheme(CGHS) Card; or (iv) Pension Card; or
(v) Army Canteen Card; or
(vi) Any Government Family Entitlement Card; or
(vii) Any other document as specified by the Department:
Provided further that the above documents may be checked by an officer specifically designated by State Government for that purpose.
2. In order to provide benefits to the beneficiaries under the Scheme conveniently, the Department through its Implementing Agency shall make all the required arrangements to ensure that wide publicity through the media shall be given to the beneficiaries to make them aware of the said requirements.
3. In all cases, where Aadhaar authentication fails due to poor biometrics of the beneficiaries or due to any other reason, the following remedial mechanisms shall be adopted, namely :-
(a) in case of poor fingerprint quality, iris scan or face authentication facility shall be adopted for authentication. The Department through its Implementing Agency shall make उत्तर प्रदेश असाधारण गजट, 18 नवम्बर, 2020 5 provisions for iris scanners or face authentication along with fingerprint authentication for delivery of benefits in seamless manner;
(b) in case the biometric authentication through fingerprints or iris scan or face authentication is not successful, wherever feasible/admissible authentication by Aadhaar One Time Password or time-based One-Time Password with limited time validity, as the case may be, shall be offered;
(c) in all other cases where biometric or Aadhaar One Time Password or time-based One- Time Password authentication is not possible, benefits under the Scheme may be given on the basis of physical Aadhaar letter authenticity of which shall be verified through the Quick Response Code printed on the Aadhaar letter and the necessary arrangement of Quick Response Code reader shall be provided at the convenient locations by the Department through its Implementing Agency.
4. Notwithstanding anything contained hereinabove, no child shall be denied benefit under the Scheme in case of failure to establish his identity by undergoing authentication, or furnishing proof of possession of Aadhaar number, or in case of a child to whom no Aadhaar number has been assigned, producing an application for enrolment. The benefit shall be given to him by verifying his identity on the basis of other documents as mentioned in clause (b) and (c) of the proviso of sub-paragraph (3) of paragraph 1, and where benefit is given on the basis of such other documents, a separate register shall be maintained to record the same, which shall be reviewed and audited periodically by the Department through its Implementing Agency.
5. This notification shall come into effect from the date of its publication in the Gazette.
