चूँकि केन्द्रीय आधार (वित्तीय और अन्य सहायकियों, लाभों और सेवाओं का लक्षित परिदान) अधिनियम, 2016 (जिसे आगे ''उक्त अधिनियम'' कहा गया है) की धारा 4(4)(ख)(दो) में ऐसे प्रयोजन, जैसा कि केन्द्र सरकार प्राधिकरण के परामर्श से और राज्य के हित में विहित करे, के लिये स्वैच्छिक आधार पर अधिप्रमाणन कराने की अनुज्ञा प्रदान की गयी है;
और, चूँकि, भारत सरकार ने सुशासन के लिये आधार अधिप्रमाणन (समाज कल्याण, नवाचार, ज्ञान) नियमावली, 2020 तैयार की है जिसके अधीन राज्य सरकार स्वैच्छिक आधार पर आधार अधिप्रमाणन का उपयोग करने की अनुज्ञा प्राप्त कर सकती है;
और, चूँकि, कार्यालय ज्ञाप संख्या 13(4)/2020-ईजी-2(वाल्यूम-चार) दिनांक, 19 जनवरी, 2021 द्वारा भारत सरकार में पूर्वोक्त अधिनियम की धारा 4(4)(ख)(दो) के साथ पठित पूर्वोक्त नियमावली के नियम 5 के निबन्धनों में पहचान के प्रयोजनार्थ स्वैच्छिक आधार पर आधार अधिप्रमाणन का उपयोग करने के लिये राज्य सरकार की पूर्व सहमति से उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड द्वारा विनिर्मित और अधिसूचित विभिन्न योजनाओं के अधीन लाभार्थियों को प्रवास प्रमाण पत्र जारी करने और मौद्रिक तथा अन्य प्रसुविधायें प्रदान करने हेतु भवन और अन्य सन्निर्माण कर्मकारों का रजिस्ट्रीकरण और नवीकरण किये जाने के लिये उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड को अनुज्ञा प्रदान करने के निमित्त सक्षम प्राधिकारी के अनुमोदन से अवगत करा दिया है। 2 उत्तर प्रदेश असाधारण गजट, 2 मार्च, 2021 अतएव, अब, सुशासन के लिये आधार अधिप्रमाणन (समाज कल्याण, नवाचार, ज्ञान) नियमावली, 2020 के नियम 5 के अधीन शक्तियों का प्रयोग करके राज्यपाल, पहचान के प्रयोजन के लिये, राज्य सरकार की पूर्व सहमति से उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड द्वारा विनिर्मित और अधिसूचित विभिन्न योजनाओं के अधीन भवन और अन्य सन्निर्माण कर्मकारों का रजिस्ट्रीकरण तथा नवीकरण किये जाने और लाभार्थियों को प्रवास-प्रमाण पत्र जारी किये जाने तथा मौद्रिक एवं अन्य प्रसुविधायें प्रदान किये जाने हेतु स्वैच्छिक आधार अधिप्रमाण के निमित्त उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड को अनुज्ञा प्रदान करती है। मुख्य कार्यपालक अधिकारी, उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड, पहचान की पद्धतियां पृथक-पृथक अधिसूचित करेगा, यदि दावेदार अपने आधार का उपयोग करते हुये अधिप्रमाणन कराना न चाहता हो।
दावेदारों से संग्रहीत सहमति प्रपत्र इस अधिसूचना से संलग्न अनुसूची ''क'' में विनिर्दिष्ट है।

मैं...................................आधार संख्या.............................धारक, यू0आई0डी0ए0आई0 के माध्यम से ई-के वाई सी के प्रयोजनार्थ यू0आई0डी0ए0आई0 के साथ आधार ओ0टी0पी0 या बायोमेट्रिक अधिप्रमाण का उपयोग करके और इसे उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड के डाटा बेस के साथ सीडिंग करके उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड, उत्तर प्रदेश को अपनी सहमति प्रदान करता/करती हूँ। मैं आधार (वित्तीय और अन्य सहायकियों, लाभों और सेवाओं का लक्षित परिदान) अधिनियम, 2016 की धारा 7 के अधीन अधिसूचित सरकारी कल्याण योजना के लिये डी0बी0टी0 के माध्यम प्रसुविधाओं के संवितरण के लिये अपने आधार संख्या का उपयोग करने के लिये भी अपनी सहमति प्रदान करता/करती हूँ।
आधार एकत्र करने की सहमति और उसके प्रयोजन के संबंध में मुझे स्थानीय भाषा में स्पष्ट किया जा चुका है। विभाग ने मुझे सूचित किया है कि मेरे आधार का उपयोग उपरोल्लिखित प्रयोजन से भिन्न किसी प्रयोजन के लिये नही किया जायेगा।
मुझे केवाईसी के प्रयोजनों के लिये विभाग द्वारा अन्य वैकल्पिक साधन (वोटर आइडी/ड्राइविंग लाइसेंस/राशन कार्ड आदि) प्रदान किये गये हैं जिनमें आधिकारिक रूप से विधिमान्य दस्तावेज जमा करके भौतिक के0वाई0सी0 किया जाना सम्मिलित है और मैने स्वैच्छिक रूप से आधार आधारित के0वाईसी0 चुना है।
मैं समझता/समझती हूँ कि मेरे द्वारा प्रदान किये जाने वाले बायोमेट्रिक्स और/या ओ0टी0पी0 का उपयोग, मेरे द्वारा विनिर्दिष्ट संव्यवहरण हेतु न कि अन्य प्रयोजनों हेतु आधार अधिप्रमाणन प्रणाली के माध्यम से अपनी पहचान अधिप्रमाणित करने के लिये ही किया जायेगा।

धारक, एतद्द्वारा उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड के डेटाबेस बनाने के लिये उपलब्ध अपने व्यक्तिगत डेटा का उपयोग करने के लिये अपनी सहमति देता हूँ। मुझे ज्ञात है कि इस डेटाबेस का उपयोग उत्तर प्रदेश भवन और अन्य सन्निर्माण कर्मकार कल्याण बोर्ड, उत्तर प्रदेश की विभिन्न योजनाओं, सेवाओं और परियोजनाओं के लिये मेरी पहचान तय करने के लिए किया जायेगा। 4 उत्तर प्रदेश असाधारण गजट, 2 मार्च, 2021 IN pursuance of the provisions of clause (3) of Article 348 of the Constitution of India, the Governor is pleased to order the publication of the following English translation of notification no. 288/XXXVI-2–2021-5(G)-2020, dated March 2, 2021:

WHEREAS section 4 (4) (b) (ii) of the Central Aadhaar (Targeted Delivery of Financial and other Subsidies, Benefits and Services) Act, 2016 (hereinafter referred to as "the said Act"), allows performing authentication on voluntary basis for such purpose as the Central Government in consultation with the Authority and in the interest of the State, may prescribe;
AND, WHEREAS, the Government of India has framed the Aadhaar Authentication for Good Governance (Social Welfare, Innovation, Knowledge) Rules, 2020 under which the State Government can seek permission to use Aadhaar authentication on voluntary basis;
AND, WHEREAS, Government of India vide Official Memorandum no. 13(4)/2020-EG-II(Vol-IV) dated 19th January, 2021 has conveyed the approval of the Competent Authority to allow Uttar Pradesh Building and Other Construction Workers Welfare Board for registration and renewal of building and other construction workers, issuance of migration certificate and providing monetary and other benefits to the beneficiaries under various schemes formulated and notified by Uttar Pradesh Building and Other Construction Workers Welfare Board with prior concurrence of the State Government, for using Aadhaar authentication on voluntary basis, for the purpose of identification in terms of rule 5 of the aforesaid rules read with section 4(4)(b)(ii) of the aforesaid Act.
NOW, THEREFORE, in exercise of the powers under rule 5 of the Aadhaar Authentication for Good Governance (Social Welfare, innovation Knowledge) Rules, 2020, the Governor is pleased to allow Uttar Pradesh Building and Other Construction Workers Welfare Board (UPBOCWWB) to seek voluntary Aadhaar authentication for registration and renewal of building and other construction workers,issuance of migration certificate and providing monetary and other benefits to the beneficiaries under various scheme formulated and notified by UPBOCWWB with prior concurrence of the State Government, for the purpose of identification.
The Chief Executive Officer, Uttar Pradesh Building and Other Construction Workers Welfare Board shall separately notify the methods of identification in case the claimant does not intend to authenticate using his Aadhaar.
The consent form to be collected from the claimants is specified in Schedule "A" appended to this notification.

Consent Form Format
I----------holder of Aadhaar No.-----------give my consent to Uttar Pradesh Building and Other Construction Workers Welfare Board for fetching my identity and other information for the purpose of e-KYC through UIDAI, using the Aadhaar OTP or Biometric authentication with UIDAI and seeding it with date base of Uttar Pradesh Building and Other Construction Workers Welfare Board, Uttar Pradesh. I also give my consent for using my Aadhaar Number for disbursement of benefits through DBT for the Government Welfare Scheme notified under section 7 of the Aadhaar (Targeted Delivery of Financial and other Subsidies, Benefits and Services) Act, 2016.
The consent and purpose of collecting Aadhaar has been explained to me in local language. The department has informed me that my Aadhaar shall not be used for any purpose other than that mentioned above.
I have been given other alternative means (Voter ID/Driving License/Ration Card, etc.), by the department for KYC purposes including physical KYC by submitting officially valid documents and I have voluntarily chosen Aadhaar based KYC.
I understand that the Biometrics and/or OTP I provide for authentication shall be used only for authenticating my identity through the Aadhaar authentication system for that specific transaction and for no other purposes.

I..............holder of UPBOCW ID .....................hereby give consent to use my personal data available for creating database of UPBOCW, I am aware that this database shall be used for deciding my eligibility for various schemes, services and projects of the Uttar Pradesh Building and Other Construction Workers Welfare Board, Government of Uttar Pradesh.
