चूँकि राज्यपाल की यह राय है कि दिनांक 3 अक्टूबर, 2021 को जनपद-लखीमपुर खीरी में 08 व्यक्तियों की हुई मृत्यु की सम्पूर्ण घटना से संंंबंधित लोक महत्व के विषय की जाँच आयोजित करना आवश्यक है। 2-अतएव, अब, जाँच आयोग अधिनियम, 1952 (अधिनियम संख्या 60 सन् 1952) की धारा 3 द्वारा प्रदत्त शक्तियों का प्रयोग करके राज्यपाल, न्यायमूर्ति मा0 उच्च न्यायालय, इलाहाबाद (सेवानिवृत्त) श्री प्रदीप कुमार श्रीवास्तव को एकल सदस्यीय जाँच आयोग, जिसका मुख्यालय लखीमपुर खीरी में होगा, के रूप में नियुक्त करती हैं।
3-चूँकि, राज्यपाल की राय है कि, की जाने वाली जाँच की प्र—ति और मामले की अन्य परिस्थितियों को ध्यान में रखते हुए ऐसा करना आवश्यक है, अतः राज्यपाल उक्त अधिनियम की धारा 5 की उपधारा (1) के अधीन यह निदेश देती हैं कि उक्त धारा 5 की उपधारा (2), (3), (4) और (5) के उपबन्ध आयोग पर लागू होंगे। 4-आयोग, इस अधिसूचना के जारी किये जाने के दिनांक से 02 मास की अवधि के भीतर अपनी जाँच पूरी करेगा। इसकी अवधि में किसी प्रकार का परिवर्तन, शासन की ओर से किया जायेगा।

WHEREAS the Governor is of the opinion that it is necessary to hold an enquiry with regard to matter of public importance regarding the entire incident of death of 08 persons in Lakhimpur Kheri District on dated 3 October, 2021.
2. NOW, THEREFORE, in exercise of the powers conferred by section 3 of the Commissions of Enquiry Act, 1952 (Act no. 60 of 1952), the Governor is pleased to appoint Justice High Court, Allahabad (Retd.) Shri Pradeep Kumar Srivastava as a single member Commission of Enquiry with Headquarters at Lakhimpur Kheri.
3. The Governor, being of the opinion that having regard to the nature of the Enquiry to be made and other circumstances of the case, it is necessary so to do, is further pleased to direct under sub-section (1) of section 5 of the said Act, that the provisions of sub-section (2), (3), (4) and (5) of section 5 shall apply to the Commission.
4. The Commission shall complete the enquiry within a period of two months from the date of the issue of this notification. Any change in its tenure shall be at the behest of the Government.
