चू ँकि आधार (वित्तीय और अन्य सहायकियों, लाभों और सेवाओं का लक्षित परिदान) अधिनियम, 2016 (अधिनियम संख्या 18 सन् 2016) (जिसे आगे "उक् त अधिनियम" कहा गया है) की धारा 4(4)(ख)(दो) में ऐसे प्रयोजन, जैसा कि के न् (cid:7076) सरकार, प्राधिकरण के परामर्श से और राज्य के हित में विहित करे, के लिये स्वै(िcid:6994)छक आधार पर अधिप्रमाणन कराने की अनु(cid:6980)ा प्रदान की गयी है;

और, चू ँकि, भारत सरकार ने सुशासन के लिये आधार अधिप्रमाणन (समाज कल्याण, नवाचार, (cid:6980)ान) नियमावली, 2020 बनायी है जिसके अधीन राज्य सरकार स्वै(िcid:6994)छक आधार पर आधार अधिप्रमाणन का उपयोग करने की अनु(cid:6980)ा प्राप्त कर सकती है;

और, चू ँकि, भारत सरकार ने कार्यालय (cid:6980)ाप संख्या 13(4)/2020-ईजी-2(वाल्यूम-चार), दिनांक 17 अगस्त, 2022 }kjk पूव(cid:7299)क् त अधिनियम की धारा 4(4)(ख)(दो) के साथ पठित पूव(cid:7299)क् त नियमावली के नियम 5 के निबन्धनों में पहचान के प्रयोजनार्थ स्वै(िcid:6994)छक आधार पर आधार अधिप्रमाणन का उपयोग करने के लिये राज्य सरकार की पूर्व सहमति से नियोजन विभाग, उत्तर प्रदेश शासन (जनशक्ति नियोजन प्रभाग) }kjk विनि(cid:7414)मत और संचालित परिवार कल्याण योजना के अधीन निवासियों के परिवार आई0डी0 बनाये जाने के लिये नियोजन विभाग, उत्तर प्रदेश शासन (जनशक्ति नियोजन प्रभाग) को अनु(cid:6980)ा प्रदान करने के निमित्त सक्षम प्राधिकारी के अनुमोदन से अवगत करा दिया है।

अतएव, अब, सुशासन के लिये आधार अधिप्रमाणन (समाज कल्याण, नवाचार, (cid:6980)ान)नियमावली, 2020 के नियम 5 के अधीन शक्तियों का प्रयोग करके राज्यपाल, पहचान के प्रयोजन के 578 RPH-data 4e Niyojan fold. 2 mÙkj çns'k vlk/kkj.k xtV] 3 vDVwcj] 2022 लिये, राज्य सरकार की पूर्व सहमति से नियोजन विभाग, उत्तर प्रदेश शासन (जनशक्ति नियोजन प्रभाग) }kjk विनि(cid:7414)मत और संचालित परिवार कल्याण योजना के अधीन निवासियों के परिवार आई0डी0 बनाये जाने हेतु स्वै(िcid:6994)छक आधार अधिप्रमाणन के निमित्त नियोजन विभाग, उत्तर प्रदेश शासन (जनशक्ति नियोजन प्रभाग) को अनु(cid:6980)ा प्रदान करती हैं।

नियोजन विभाग, उत्तर प्रदेश शासन (जनशक्ति नियोजन प्रभाग), पहचान की i)fr;ka पृथक-पृथक अधिसूचित करेगा, यदि दावेदार अपने आधार का उपयोग करते (cid:7263)ये अधिप्रमाणन कराना न चाहता हो।

दावेदारों से संगृहीत किये जाने वाले सहमति प्रपत्र इस अधिसूचना से संलग्न अनुसूची "क" में विनि(cid:7416)दष् ट है।

1–मैं समझता/समझती (cid:7264)ँ कि आधार (वित्तीय और अन्य सहायकियों, लाभों और सेवाओं का लक्षित परिदान) अधिनियम, 2016 (अधिनियम संख्या 18 सन् 2016) और त(cid:7373)ीन बनाये गये विनियमों के अधीन यथा बोध्य मेरी आधार संख्या, फोटो(cid:7061)ाफ और जनसांख्यिकीय जानकारी नियोजन विभाग, उत्तर प्रदेश शासन (जनशक्ति नियोजन प्रभाग) }kjk निम्नलिखित प्रयोजनों के लिये एकत्र की जा रही है –:

एक-आधार अधिप्रमाणन प्रणाली के माध्यम से मेरी पहचान को अधिप्रमाणित करना; दो-आधार (वित्तीय और अन् य सहायकियों, लाभों और सेवाओं का लक्षित परिदान) अधिनियम, 2016 (18 सन् 2016) के अधीन सहायकियों, लाभों और सेवाओं को प्राप्त करने के लिये अधिप्रमाणन के उपरान्त पोर्टल पर रजिस् ट्रीकरण (परिवार आई0डी0/फै मिली आई0डी0/परिवार कल्याण योजना पोर्टल) करना;

तीन-कें (cid:7076) सरकार और राज्य सरकारों }kjk आधार (वित्तीय और अन्य सहायकियों, लाभों और सेवाओं का लक्षित परिदान) अधिनियम, 2016 (अधिनियम संख्या 18 सन् 2016) के अधीन संचालित सरकारी कल्याणकारी कार्य(cid:7059)मों, जो fo|eku हैं और भविष्य के कार्य(cid:7059)मों के लिये मेरी पात्रता निर्धारित करने के प्रयोजन से मेरी पहचान सत्यापित करने हेतु मेरी आधार संख्या और जनसांख्यिकीय जानकारी और फोटो(cid:7061)ाफ को साझा करना;

2–मैं समझता/समझती (cid:7264)ँ कि नियोजन विभाग, उत्तर प्रदेश शासन (जनशक्ति नियोजन प्रभाग) इस प्रपत्र के पैरा 1 (एक)-(तीन) में lwphc) समस् त या किसी प्रयोजन के लिए मेरी आधार संख्या, फोटो(cid:7061)ाफ और जनसांख्यिकीय जानकारी युक्त आधार-सीडेड डेटाबेस तैयार करेगा, और नियोजन विभाग, उत्तर प्रदेश शासन (जनशक्ति नियोजन प्रभाग) यह सुनि(िcid:7391)त करेगा कि लागू विधियों, नियमों और विनियमों के अनुसार ऐसी जानकारी की रक्षा, सुरक्षा और गोपनीयता सुनि(िcid:7391)त करने के लिए आवश्यक तंत्र स्थापित किये गये हैं।

3–मुझे इस प्रपत्र के पैरा 1 (एक)-(तीन) में lwphc) प्रयोजनों के लिये आधार आधारित अधिप्रमाणन और इसके उपरान्त इस प्रपत्र के पैरा 2 में व(cid:7414)णत आधार-सीडेड डेटाबेस के निर्माण के लिये अपनी आधार संख्या, फोटो(cid:7061)ाफ और जनसांख्यिकीय जानकारी प्रदान करने में कोई आपत्ति नह(cid:7277) है।

4–मैं यह भी समझता/समझती (cid:7264)ँ कि इस प्रपत्र में प्रदान की गयी मेरी 'अनापत्ति' परिवर्तनशील रहेगी और मुझे भविष्य में किसी भी समय इसे वापस लेने का अधिकार है जिसके लिये मेरे }kjk संसूचित किया जायेगा।

IN pursuance of the provisions of clause (3) of Article 348 of the Constitution of India, the Governor is pleased to order the publication of the following English translation of notification no. 998/XXXV-1–2022, dated October 3, 2022:

WHEREAS section 4(4)(b)(ii) of the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2016 (Act no. 18 of 2016) (hereinafter referred to as "the said Act"), allows performing authentication on voluntary basis for such purpose as the Central Government in consultation with the Authority and in the interest of the State, may prescribe;

AND WHEREAS the Government of India has framed the Aadhaar Authentication for Good Governance (Social Welfare, Innovation, Knowledge) Rules, 2020 under which the State Government can seek permission to use Aadhaar authentication on voluntary basis;

AND WHEREAS Government of India vide Official Memorandum no. 13(4)/2020-EG-II(Vol-IV), dated 17th August, 2022 has conveyed the approval of the Competent Authority to allow Planning Department, Government of U.P. (Manpower Planning Division) for generation of Parivar ID of residents under Parivar Kalyan Yojna formulated and operated by Planning Department, Government of Uttar Pradesh (Manpower Planning Division) with prior concurrence of the State Government, for using Aadhaar authentication on voluntary basis, for the purpose of identification in terms of rule 5 of the aforesaid rules read with section 4(4)(b)(ii) of the aforesaid Act.

NOW, THEREFORE, in exercise of the powers under rule 5 of the Aadhaar Authentication for Good Governance (Social Welfare, Innovation, Knowledge) Rules, 2020, the Governor is pleased to allow Planning Department, Government of Uttar Pradesh (Manpower Planning Division) to seek voluntary Aadhaar authentication for generation of Parivar ID of residents under Parivar Kalyan Yojna formulated and operated by Planning Department, Government of Uttar Pradesh (Manpower Planning Division) with prior concurrence of the State Government, for the purpose of identification.

The Planning Department, Government of Uttar Pradesh (Manpower Planning Division) shall separately notify the methods of identification in case the claimant does not intend to authenticate using his Aadhaar.

The consent form to be collected from the claimants is specified in Schedule "A" appended to this notification.

SCHEDULE "A"

Form for Intimation of Purposes for prospective data sharing

1. I understand that my Aadhaar number, photograph and demographic information, as understood under the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2016 (Act no. 18 of 2016) and regulations framed there under, is being collected by the Planning Department, Government of Uttar Pradesh (Manpower Planning Division) for the following purposes:-

i. Authenticating my identity by way of the Aadhaar authentication system;

ii. Registering on the Portal (Parivar ID/Family ID/Parivar Kalyan Yojana Portal), after authentication, for availing subsidies, benefits & services under the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2016 (18 of 2016);

iii. Sharing of my Aadhaar number and demographic information and photograph, for verifying my identity for the purpose of determining my eligibility across Government welfare programmes, which are in existence and for future programmes, run by the Central Government and State Governments under the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2016 (18 of 2016);

2. I understand that the Planning Department, Government of U.P. (Manpower Planning Division) shall create an Aadhaar-seeded database containing my Aadhaar number, photograph and demographic information for all or any of the purposes enlisted in paragraphs 1(i)-(iii) of this form, and that the Planning Department, Government of Uttar Pradesh (Manpower Planning Division) shall ensure that requisite mechanisms have been put in place to ensure safety, security and privacy of such information in accordance with applicable laws, rules and regulations.

3. I have no objection to provide my Aadhaar number, photograph and demographic information for Aadhaar based authentication for the purposes enlisted in paragraphs 1(i)-(iii) of this form and further for creation of an Aadhaar-seeded database as described Paragraph 2 of this form.

4. I also understand that my 'no-objection' accorded in this form is revocable and I have the right to withdraw the same at any time in future, through a communication of opting out.

Consent For Using Personal Data

I………………………., hereby give consent to use my personal data available with different schemes/programmes/services/Government/Quasi-Government databases for creation of Family ID/Parivar ID/Parivar Kalyan Yojana Portal database of Planning Department, Government of U.P. (Manpower Planning Division) and for availing Government services, benefits under Government schemes, estabilishing identity and/or other purposes. I am aware that this database shall be used for above purposes to provide benefits of different schemes/programmes/services etc. and/or to establish my identity.
