1. I say that my name recorded as per my Birth Certificate Registration No. 615 date of reg. 21-08-1954 before the Civil Registrar Canacona, Goa is Panduranga Naique Nirpono which is my correct name.
2. I say that my name also mentioned in my PAN card No. ADRPN6440P and in Election card No. BTZ6155866 is Panduranga Naique Nirpono.
3. I say that my name recorded in my old Aadhaar card No. 4810 1384 6812 is Pandurang Vithoba Naik, wherein recorded as per Hindu rituals wherein middle name is my father's name and the same was subsequently rectified in my new Aadhaar card as Panduranga Naique Nirpono as per my birth certificate.
6. I say that all above stated names "Panduranga Naique Nirpono" "Pandurang Vithoba Naik" and "Umesh Vithoba Naik" refers to me and are of one and same person i.e. myself only.
7. I say that I am swearing this Affidavit to publish the change of name in the Official Gazette from "Pandurang Vithoba Naik", "Umesh Vithoba Naik" to 'Panduranga Naique Nirpono" to produce in PAN card office.
8. I say that whatever I have stated in the foregoing paras are true to the best of knowledge and belief.
