In addition to areas notified vide the above read notification, Government of Goa is hereby pleased to notify the demarcation of additional areas for watersports activities on North and South Goa beaches with a view to regulate and monitor these activities in the entire State of Goa in terms of Clause 24(b) of "the Policy for regulation of watersports in Goa" published in Official Gazette, Series I No. 34 dated 19th November, 2020 and the same is published herewith for information of concerned stakeholders/general public.

In exercise of the powers conferred on me under Section 112 of the Motor Vehicles Act, 1988 and Rule 264A of the Motor Vehicles Rules, 1991 as amended in 2005 and as recommended by the Assistant Engineer, S.D.III, Works Division XXIII, PWD, Bicholim-Goa and the Deputy Superintendent of Police (Traffic), Altinho, I, Ajit Roy, IAS, District Magistrate, North Goa hereby notify erection of "Speed Breakers" on the MDR-23 Road at the places mentioned in column No. 2 of the schedule herein below within the jurisdiction of Bicholim Municipal Council, Bicholim Taluka.

Two hump type speed breakers in Ward No. 12 of Bicholim Municipal Council: Two hump type speed breakers to be constructed on either side of the T-junction one in front of Vaishnav store and the other in front of Amul Ice Cream Parlour at Gaonkarwada in Ward No. 12 of Bicholim Municipal Council, Bicholim-Goa
The above Speed Breakers shall be painted with white thermoplastic fluorescent paint and fixed with reflected road studs around it for better visibility.
Further, in exercise of the powers conferred on me under Section 116 of the above Act, I also authorize the erection of cautionary signboards "Speed Breaker" at about 40 mts. on either side of the speed breaker in advance of the rumbler strips and traffic signboard showing the sign "Speed Breaker" at the placement of the speed breaker in order to regulate the motor vehicular traffic.
The Chief Officer, Bicholim Municipal Council, is directed to publish this notification through the Director of Information and Publicity in local newspapers to make necessary arrangements to construct the Speed Breakers as also Rumbles Speed Breakers and P.W.D. (Engineering Cell) to erect the signboards at the places indicated above as per the specification laid down by the Ministry of Road Transport and Highways and as published OFFICIAL GAZETTE — GOVT. OF GOA SERIES III No. 41 6TH JANUARY, 2022 in the Official Gazette, Series I No. 15 dated 15-07-2005 by the Government of Goa and report compliance within thirty days time.

Affidavit 29. I, the undersigned, Mr. Ram Sharan Rajput, son of Late Maha Ram Rajput, aged 58 years, married, service, Indian National, resident of H. No. 43/1, Ganganagar, near Dada Vaidya Higher Secondary, Curti, Ponda-Goa do hereby affirm on oath as under:-
1. I say that I am resident of above mentioned address along with my family members and shall continue to be resided therein.
2. I say that in my Aadhaar Card bearing No. xxxx xxxx xxxx and in my State Bank of India Savings Bank passbook bearing account No. 10905555338 my name is recorded as Ram Sharan Rajput, whereas on my Employee's Pension Scheme Certificate No. GA/GOA/ /00012051 my name is appearing as Ramsharan Lodhi.
3. I say that the names Ram Sharan Rajput and Ramsharan Lodhi are of one and the same person, that is myself.
4. I say that similarly on my Aadhar card and in my State Bank of India Savings Bank passbook name of my father is appearing as Maha Ram Rajput whereas on my Employee's Pension Scheme Certificate No. GA/GOA/00012051 Father's Name is appearing as Maharam Lodhi.
5. I say that the names Maha Ram Rajput and Maharam Lodhi are of one and the same person, that is of my father.
6. I say that I have sworn this affidavit in order to produce before the Regional PF Commissioner, Employee's Provident Fund Organisation, Bhavishya Nidhi Bhavan, Patto Plaza, Panaji-Goa in connection with correction in names in the said Employee's Pension Scheme Certificate No. GA/GOA/ /00012051.
7. I say that whatever I have stated herein above is true to my knowledge and no part of it is false.

25-02-2020 till 24-02-2030, my name is recorded
there in as Chazen Hipolito Savio Fernandes.
2. I say that my name as per my Continuous
Discharge Certificate (CDC) bearing No. MUM
194975, issued at Mumbai by Government of
India, on 17-11-2011 till 16-11-2021, my name
is recorded there in as Fernandes Chazen.
3. I say that both the above mentioned names
pertain to one and the same person i.e. myself
and I am ready to undergo any punitive action
in case any of my statement is found to be
false.
4. I say that the copies in respect of the above
referred names are annexed herewith
alongwith this Affidavit.

that I am known by the aforesaid names
