82. I, Mrs. Sanchita Sachin Godkar, 30 years of age, w/o Sachin Moreshwar Godkar, married, housewife, r/o H. No. 349, Alto Desterro, Bogda, Vasco-da-Gama, Goa, Indian National, do hereby solemnly affirm and states on oath as under:-
1. I say that I am resident of Goa and residing at
the above mentioned address.
2. I say that on my Birth certificate my name is mentioned as "Pratima Purushottam Gawandi".
3. I say on my marriage certificate my name is changed from "Pratima Purushottam Gawandi" to "Sanchita Sachin Godkar".
4. I say that on my Aadhaar card my name is
mentioned as "Sanchita Sachin Godkar".
5. I say that on my PAN card my name is mentioned
as "Sanchita Sachin Godkar".
6. I say that from clause 2 to 5 the above referred mentioned names belong to one and the same person i.e. myself.
7. I say that I have sworn this Affidavit-cum- -Declaration to clarify the anomaly/discrepancy in my name and submit the same to the Government Printing Press to publish the same in the Official Gazette of Government of Goa.
8. I say that I have sworn this Affidavit-cum- -Declaration in order to produce in different Government Departments and for publishing in Daily Local & National Newspaper.
9. I say that the contents of the above Affidavit- -cum-Declaration from para No. 1 to 8 are true to my own knowledge and belief.
