51. I, Jeronimo John Fernandes, son of late Jose Philip Fernandes, aged about 73 years, married, retired, Indian National, holding Adhaar No. xxxx xxxx xxxx, resident of House No. L/20, Housing Board Colony, Alto-Betim, Penha-de-Franca, Bardez-Goa, do hereby solemnly affirm and state as under:-
1. I say that my correct name is Jeronimo John Fernandes and same name stands recorded in my Aadhaar card bearing No. xxxx xxxx xxxx.
2. I say that on my PAN card bearing No. AAEPF7499Q my name is mentioned as John Jeronimo Fernandes.
3. I say that Jeronimo John Fernandes and John Jeronimo Fernandes is one and the same person i.e. myself.
4. I say that present Affidavit is sworn by me in order to produce in future before Office of Competent Authority for correcting my name in Demat Account and or Physicial Share Certificates i.e. Jeronimo John Fernandes.
5. I say that contents of the foregoing paras are
true and correct to my knowledge and belief.
3. I state that, on my Bank Passbook at State Bank of India, Panaji, Market Branch, my name is recorded as Ravi Lamani.
4. I say that, on my PAN card No. BEYPL2610G my
name is recorded as Ravi Lamani.
5. I state that my correct name is Ravi Lamani however, it was recorded as Ravi Jadhav by error in my Aadhaar card and now I wish to correct the same.
6. I state that, I am swearing this Affidavit to be produced to the Government Printing Press, Depar tment of Printing & Stationer y, Government of Goa to be published in the Official Gazette so as to produce the same for correction of my name in Government Departments or such concerned authorities.
