1. I say that as per Aadhaar card my name is shown as Anjani Prakash Baliga.
2. I say that my Share certificate issued by Canara Bank shows my name as Anjani P Bawga.
3. I say that my correct name is Anjani Prakash Baliga.
4. I say that Anjani Prakash Baliga and Anjani P Bawga is one and the same person.
5. I say that I have sworn this Affidavit for the purpose of submitting the same to Canara Bank.
