In exercise of the powers conferred under Section 112 and 116 of the Motor Vehicles Act, 1988 (Central Act 59 of 1988) read with Government Notification No. 5/28/88/TPT (Part) dated 26th September, 1989 and Rule 264-A of the Goa Motor Vehicles (Amendment) Rules, 2005, I, Egna Cleetus, IAS, District Magistrate, South Goa, Margao, in public interest, do hereby notify the "No Entry" and traffic signages as indicated in the schedule below and also direct cautionary signboards demarcating the location for the visibility of the road users for the purpose of regulating motor vehicle traffic.

The signboards shall be installed as per specifications laid in the seventh schedule under the Goa Motor Vehicles (Amendment) Rules, 2005.

4. I thus desire to change my said son's name in the school records from Prithviraj Mahesh Vibhuti to Prithviraj Mallappa Vibhuti.
5. I say that both the names that are Prithviraj Mahesh Vibhuti and Prithviraj Mallappa Vibhuti are names of one and the same person being my said son. However since on the Birth certificate my said son's name is recorded as Prithviraj Mallappa Vibhuti, I desire to change my said son's name from Prithviraj Mahesh Vibhuti to Prithviraj Mallappa Vibhuti in the school record register maintained by the PM Shri Kendriya Vidyalaya I. N. S. Mandovi, Verem, Bardez-Goa which is the correct name of my said son.
6. I say that I am swearing the present Affidavit to be produced before the PM Shri Kendriya Vidyalaya I. N. S. Mandovi, Verem, Bardez, Goa seeking correction of my said son's name from Prithviraj Mahesh Vibhuti to Prithviraj Mallappa Vibhuti in the school records and register maintained by the school.
7. I say that whatever stated hereinabove is true to the best of my knowledge and nothing material is concealed.
