1. संस्था का नाम और पता । 2. निर्माता का न्यूनतम टर्न ओवर एक करोड़ रूपये का होना चाहिए । 3. पिछले तीन वर्षो का वार्षिक अंकेक्षण व्योरा होना चाहिए । 4. पिछले तीन वर्षो का आयकर निर्धारण विवरण होना चाहिए । 5. वाणिज्य कर पंजीकरण का प्रमाण-पत्र । 6. आपूर्तिकत्र्ता को निविदा के साथ उल्लेखित मशीन/उपकरणों के उपभोक्ताओं या वर्तमान ग्राहकों की सूची देनी आवश्यक होगी ।
7. आपूर्तिकत्र्ताओं को निविदा के साथ उत्पाद के निर्माण एवं प्रत्येक मशीन/उपकरण का Technical Specification देना आवश्यक होगा ।
8. निविदादाताओं को कम्पनी के निगमन का प्रमाण-पत्र एवं इसके प्रवर्तकों का बायो-डाटा देना आवश्यक होगा । 9. कम्पनी के पास मशीन उपकरण सर्विस सेन्टर पटना में होना चाहिए तथा दक्ष जनता का पता फोन नं0/ मोवाईल नं0/ सम्पर्क नं0 देना होगा।
10. कम्पनी के पास किसी सक्षम प्राधिकार से अपने दोष रहित होने का प्रमाण-पत्र देना होगा । 11. निविदादाता को रू. 5000 (पांच हजार) की सुरक्षा/ निविदा राशि डिमाण्ड ड्राफ्ट के रूप में प्रबंधक, राजकीय आयुर्वेदिक यूनानी औषध निर्माणशाला, बिहार, पटना के पक्ष में जमा करना होगा ।
12. सरकार के निदेशानुसार वैट की कटौती आपूर्तिकत्र्ता के विपत्र से कर ली जायेगी और चालान द्वारा सरकारी खजाने में जमा कर दिया जायेगा।
13. मशीन उपकरण का क्रय आदेश का कुल मूल्य का 2 प्रतिशत बैंक गारंटी/वारंटी बतौर जमानत आपूर्तिकत्र्ता को 15 दिनों के अन्दर जमा करना होगा । उक्त बैंक गारंटी की एक साल तक वैलीडीटी होनी चाहिए । 14. आपूर्तिकत्र्ता मशीनों के निर्माणशाला में प्रतिष्ठापित एवं प्रदर्शन कर संबंधित पदा0/कर्मचारी को उपयोग करने का तरीका बताना सुनिश्चित करेंगे तथा मशीन/उपकरण की गारन्टी वारंटी की अवधि समाप्त होने पर संबंधित6 (cid:18)बहार गजट, 23 माच* 2011 आपूर्तिकत्र्ता अगले पांच सालों तक मशीनों का Annual Maintanance करने की व्यवस्था करेंगे तथा इसके लिए अलग से दर निर्धारित करेंगे ।
15. निविदा कम्प्यूटर द्वारा टाईप होनी चाहिए तथा निविदा में जिन कागजातों को दिया गया है उसकी सूची संलग्न करनी होगी ।
16. आपूर्तिकत्र्ता को स्वीकृत दर पर मशीनों की आपूर्ति एवं प्रतिष्ठापन एवं संचालित कर प्रदर्शन अपने खर्च पर राजकीय आयुर्वेदिक एवं यूनानी औषध निर्माणशाला, बिहार, पटना के भवन में करनी होगी । मशीन/उपकरण का संतोषप्रद कार्य प्रदर्शन होने के बाद ही राशि का भुगतान किया जायेगा ।
17. क्रय समिति का निर्णय अन्तिम होगा तथा किसी तरह का कानूनी मतभेद का निपटारा पटना उच्च न्यायालय, पटना से होगा ।
18. निविदा की सूचना www.prdbihar.gov.in पर देखा जा सकता है।

(1) निविदादाताओं को मशीन का लैंडेड प्राइस (प्रतिष्ठापन एवं प्रदर्शन तथा एक साल का गारण्टी/वारण्टी का रख-रखाव जोड़कर)/स्थानीय बिक्रीकर/वैट/लागू होने वाले अन्य कर सभी को जोड़कर अंतिम दर देना होगा जिसे न्यूनतम दर माना जायेगा तथा जिसे L 1 जाना जायेगा ।
(2) मशीन की गारण्टी/वाराण्टी अवधि समाप्त हो जाने के बाद मशीन के वार्षिक रख-रखाव के लिए वार्षिक दर अगले पांच सालों के लिए अलग से देना होगा ।
एक बार निविदा प्राप्त हो जाने के बाद निविदादाता को निविदा में दिये गये विवरण में किसी प्रकार की संशोधन की अनुमति नहीं दी जायेगी।
निविदा जमा करने संबंधित आवश्यक सूचना :- इच्छुक निविदादाता अपनी निविदा एक बड़े लिफाफे जिसपर ''औषध निर्माणशाला के लिए मशीन/उपकरण क्रय के लिए निविदा'' लिखा होना चाहिए। इस बड़े लिफाफे के अन्दर दो अलग-अलग मुहरबन्द लिफाफे डालें जिसमें एक में तकनिकी निविदा (जिसे Cover-A के रूप में चिन्हित करें) एवं दूसरे में वित्तीय निविदा (जिसे Cover-B के रूप में चिन्हित करें)
निविदा जमा करने की अंतिम तिथि :- मुहरबन्द निविदा प्रबंधक, राजकीय आयुर्वेदिक एवं यूनानी औषध निर्माणशाला, बिहार, पटना के लिए आमंत्रित की जाती है। निविदा कार्य दिवस में प्रकाशन की तिथि से 30वें (तिसवें) दिन के अन्दर अपराह्न 5.00 बजे तक स्पीड पोस्ट/निबंधित डाक से आमंत्रित की जाती है । अगर निविदा प्राप्त होने की अंतिम तिथि को अवकाश होगा तो अगला दिन कार्य दिवस माना जायेगा । निर्धारित तिथि के बाद निविदा पर विचार नहीं किया जायेगा ।

The procurement of equipment/machinery are processing according to Advisor (Ayurveda) / Dy.Advisor (Ayurveda)Government Of India, Ministry of Health & Family Welfare (Department of ISM & H).Letter no.– 17016/7/2002-Dcc (IMS) dt. 9th July 2002 & DO. NO. Z- 17016/7/2002-Dcc (IMS) dt. 23rd September 2002
Tender is invited from reputed agencies for procurement of equipment/machinery for State
Pharmacy of Ayurvedic and Unani Medicines,Bihar, Patna
- The list of equipment/machinery are enclosed as Annexure-I.
- The detail of technical specification is enclosed as Annexure-II.
Eligibility criteria of Bid
TECHNICAL BID:-
1. Name and address of the agency.
2. A minimum turnover of the manufacturer must be around 1 Crore per year.
3. Audit annual report for the last 3 years.
4. Income tax assessment report for the last 3 years.
5. Certificate of Sale tax Registration.
6. The supplier must attach a user or existing client list for the mentioned equipments with the bid
documents.
7. The supplier must specify the make of the product and its technical data sheet of each machine with
the bid document.
8. The bidder must also attach promoters' bio data and the certificate of incorporation of company.
9. The company must have an office in Patna/ authorized service center with address and phone/mobile
no. of expert persons.
10. Non-conviction certificate issued by the competent authority.
11. Bidder will be required to deposit Rs. 5000/- (five thousand) security/tender money in the form of
Demand Draft in favour of " Manager , State Pharmacy of Ayurvedic and Unani Medicines , Bihar ,
Patna".
12. As per the Government`s instructions VAT will be deducted from the supplier`s bill and deposited to
the treasury through challan.
13. The supplier will have to deposit a bank guarantee/warrantee of 2% of the total cost of purchasing
order of the machine equipments as security within 15 days. The validityof the above said bank
guarantee/warrantee should be up to one year.
14. The supplier will have to install the machine and demonstrate its functioning to the concerned
officers/employees. Also, the supplier will have to make an arrangement for the annual maintenance
of the machine for the next five year after the expiration of the guarantee/warrantee on them and for
this a separate rate should be mentioned.
15. The tender should be computerized and a list of all the documents produced with it should be
enclosed.
16. The supplier will have to ensure the supply and installation of machine as well as itsdemonstration at
their own expenses in the building of,State Pharmacy of Ayurvedic and Unani Medicines,Bihar,
Patna. It should be noted that the payment will be made to the supplier only after the satisfactory
functioning of the machines.
17. The decision of the purchasing committee will be final and all the disputes, if any, will be settled
within the Jurisdiction of the Patna High Court.

FINANCIAL BID
1. Printed sheet with a landed price (including installation , demonstration and one year
guarranty/warrenty mantainance)/local sale tax/VAT/rate for applicable and then the final rate
will be criteria for determining the lowest quoted rate, henceforth known as L1 rate.

The bids in sealed envelope shall be submitted to the "Manager,State Pharmacy of Ayurvedic and Unani
Medicines,Bihar,Patna" on or before the 30th day from the date of publication of this tender notice,till 5 P.M.
only through Registered/Speed-Post.It should be noted that if the last date for submission of tender falls on a
holiday,then the next working day shall be considered as the last date.
