No. FDS-KNR-LPG/2022.—In supersession of all previous notifications and in exercise of the powers conferred on me under Clause 9(d) (e) of the L.P.G. (Regulation of Supply and 72—राजपत्र/2022-04-07-2022 (2451)2452 राजपत्र, हिमाचल प्रदेश, 04 जुलाई 2022/13 आषाढ़, 1944 Distribution) Order, 2000, I, Abid Hussain Sadiq, IAS, District Magistrate, Kinnaur at Reckong Peo (H.P.) do hereby fix the additional labour and transportation rates that may be charged on the delivery of the 14.2 kg. domestic LPG cylinder refills by the various LPG Agencies operating in District Kinnaur.

Guidelines and Instructions for the LPG Agencies/ Distributors in District Kinnaur:

1. At the time of arrival of truck carrying filled LPG cylinders, the Distributor should match the details of the Invoice with the receipt of consignment in the following manner: (i) Truck No. (ii) Type of cylinders (iii) Number of cylinders (iv) The time when truck left Supply Point (v) The time when truck reported at godown.

2. The Distributor to carry out the following: (i) Separate area should be marked in the LPG storage godown for keeping filled, empty and defective cylinders for each type of cylinders. (ii) The platform type Digital Weighing Scale of least count +/-10 grams should be checked with standard weight for accuracy. (iii) While unloading filled LPG cylinders, weight of 10% cylinders taken randomly should be checked and records should be maintained in the prescribed format given by OMCs. (iv) Cylinders that have net weight beyond permissible limit should be returned in the same truck. Record of the same should be maintained.

3. Distributor must display opening stock and Retail Selling Prices of products / various categories of cylinders and must maintain a daily stock-register for filled, empty and defective cylinders for all types / categories of cylinders in the following format.

4. The Distributor/ Incharge must ensure that 100% cylinders should be visually inspected for dents and cuts on the body of cylinders and such damaged cylinders which may be likely to cause a safety risk should be segregated and returned to Bottling Plant. All the cylinders taken out for delivery should be checked for correctness of net weight on the platform type digital weighing scale and cylinders that have net weight beyond permissible limit prescribed in the Legal Metrology (Packaged Commodities) Rules, 2011 as amended from time to time (at time of this notification +/- 150 grams) should be segregated and returned to concerned Bottling Plant. It is the2454 राजपत्र, हिमाचल प्रदेश, 04 जुलाई 2022/13 आषाढ़, 1944 duty of the Distributor/ Incharge to ensure that a cylinder with correct net weight only should be taken out for delivery to the consumers.

5. Delivery boys engaged for effecting home delivery should be in uniform and having Identity Card issued by the distributor. While delivering the filled LPG cylinder to the customer, distributor staff should verify the address of the customer. In each and every case, the delivery man of the distributor should carry weighing equipment and deliver the cylinder after demonstrating weight to the customer or his representative. The Distributor is to ensure that in each and every case, the delivery man carries weighing equipment and the cylinder is delivered to the customer after demonstrating weight in the consumer's presence.

6. It is imperative that the Distributor/ Incharge ensures that Cash Memos are issued to consumers showing complete name, address with consumer number and keeps duplicate copies of the same for the purpose of inspection. Also the current Retail Sale Price (RSP), labour and transportation charges, GST and Grand Total to be paid on delivery must be clearly mentioned.

7. All statutory rules and regulations in connection with storage and sale of petroleum products must be followed and implemented. Also the Distributor/ Incharge is liable to action under the Irregularities mentioned in the "Marketing Discipline Guidelines for LPG Dealerships" if any such violations are observed.

8. The LPG cylinder will be supplied by door delivery only in urban areas and at Focal Point in the rural areas as notified in the District.

9. The LPG Distributer shall not charge more than the price/Coolidge fixed by the District Magistrate, Kinnaur. The rates of Door Delivery shall be charged only when door delivery is made. Where the distance of locality is greater than will be focal point for distribution of LPG refills at central place suitable to the consumers of respective area.

10. The Incharge /Distributor shall keep at least thirty Five (35) Refills (50 in District H.Q. Reckong Peo in reserve at all time to meet out the demand of essential service, which will be released as per the directions of the District Administration & the District Controller/F&SO/Inspector, Food, Civil Supplies & Consumer Affairs of the area concerned.

11. The Incharge /Distributor shall not deliver domestic cylinder to the Hoteliers/ Dhaba- walas/ Halwaies etc. or to other commercial establishments.

12. The Incharge/Distributor shall also ensure immediate redressal of complaints lodged by the consumer for leakage and other reasons without further loss of time and render the services of a trained mechanic, free of cost keeping in view the security measures.

13. In the event of accident all registered LPG consumers are covered under an insurance policy taken by the PSU Oil Companies. In case of the unfortunate event of an accident, the customer must immediately inform the distributor/Incharge in writing. The distributor will offer assistance to the customer in completing the formalities of insurance claims arising out of the accident.

14. The Incharge/Distributor shall also ensure to furnish timely monthly report immediately at the end of every month on a prescribed Performa, a daily report on2455 राजपत्र, हिमाचल प्रदेश, 04 जुलाई 2022/13 आषाढ़, 1944 Whatsapp group or as and when required by the District Controller, Food, Civil Supplies & Consumer Affairs, Kinnaur in respect of receipt and distribution of LPG cylinders.

15. During the period of shortage of LPG refills the distributor shall distribute LPG refills to the consumers on the directions of the District Magistrate and update position to the District Controller Food Civil Supplies & Consumer Affairs on a daily basis.

16. Every Distributer shall ensure public address system (Loud Speaker) on the vehicles, weighing Machine, Laminated copy of Route Chart as well as latest RSP.

17. GST would be chargeable on the per cylinder labour and transport expenses at the rate 5% and rounded off to nearest rupee and would be subject to modification of GST rate by the government.

18. All agencies must strictly comply to rates notified and shall submit their updated route chart to the office of the District Controller, Food, Civil Supplies & Consumer Affairs, Kinnaur within seven days of this notification.

19. Any contravention will be viewed seriously and action under the provision of the L.P.G. (Regulation of Supply and Distribution) Order, 2000 and the H.P. Hoarding and Profiteering Prevention Order, 1977 issued under the Essential Commodities Act, 1955, shall be taken against the defaulters.

संख्याःपीईआर-(एस.ए.एस.-I)ए(3)-4/2021.--हिमाचल प्रदेश के राज्यपाल, भारत के संविधान के अनुच्छेद 309 के परन्तुक द्वारा प्रदत्त शक्तियों का प्रयोग करते हुए, हिमाचल प्रदेश, कार्मिक विभाग (सचिवालय प्रशासन सेवाएं) में रोकड़ एवं बिल वाहक, वर्ग-III (अराजपत्रित) के पद के लिए इस अधिसूचना से संलग्न उपाबन्ध-"क" के अनुसार भर्ती और प्रोन्नति नियम बनाते हैं, अर्थात्‌ः--

1. संक्षिप्त नाम और प्रारम्भ.--(1) इन नियमों का संक्षिप्त नाम हिमाचल प्रदेश, कार्मिक विभाग (सचिवालय प्रशासन सेवाएं) रोकड़ एवं बिल वाहक, वर्ग-III (अराजपत्रित) भर्ती और प्रोन्नति नियम, 2022 है। (2) ये नियम राजपत्र (ई-गजट), हिमाचल प्रदेश में प्रकाशन की तारीख से प्रवृत्त होंगे। 2. निरसन और व्यावृत्तियां.--(1) अधिसूचना संख्या पर0-(एस.ए.एस.)ए0-ए0(3)-1/95, तारीख 25 जुलाई, 1996 द्वारा अधिसूचित, हिमाचल प्रदेश कार्मिक विभाग (सचिवालय प्रशासन सेवाएं) रोकड़ एवं बिल वाहक, (वर्ग-III, अराजपत्रित) के भर्ती और प्रोन्नति नियम, 1996 का एतद्‌द्वारा निरसन किया जाता है। 2456 राजपत्र, हिमाचल प्रदेश, 04 जुलाई 2022/13 आषाढ़, 1944 (2) ऐसे निरसन के होते हुए भी उपर्युक्त उप नियम 2(1) के अधीन इस प्रकार निरसित नियमों के अधीन की गई कोई नियुक्ति, बात या कार्रवाई इन नियमों के अधीन विधिमान्य रूप में की गई समझी जाएगी।

उपाबन्ध-"क" हिमाचल प्रदेश, कार्मिक विभाग (सचिवालय प्रशासन सेवाएं) में, रोकड़ एवं बिल वाहक, वर्ग-III (अराजपत्रित) के पद के लिए भर्ती और प्रोन्नति नियम

1. पद का नाम.--रोकड़ एवं बिल वाहक

2. पद की संख्या.--02 (दो)

3. वर्गीकरण.--वर्ग-III (अराजपत्रित) (गैर-तकनीकी सेवाएं)

4. वेतनमान.--हिमाचल प्रदेश सिविल सेवाएं (संशोधित वेतन) नियम, 2022 का पे मेट्रिक्स स्तर-6 (रु. 25600-81200)।

5. "चयन" पद अथवा "अचयन" पद.--अचयन।

6. सीधी भर्ती के लिए आयु.--लागू नहीं।

7. सीधे भर्ती किए जाने वाले व्यक्ति (व्यक्तियों) के लिए अपेक्षित न्यूनतम शैक्षिक और अन्य अर्हताएं.--लागू नहीं।

8. सीधे भर्ती किए जाने वाले व्यक्ति (व्यक्तियों) के लिए विहित आयु और शैक्षिक अर्हता (अर्हताएं) प्रोन्नत व्यक्ति (व्यक्तियों) की दशा में लागू होंगी या नहीं.--आयुः-लागू नहीं।

शैक्षिक अर्हता(एं)ः--लागू नहीं।

9. परिवीक्षा की अवधि, यदि कोई हो.--लागू नहीं।

10. भर्ती की पद्धति, भर्ती सीधी होगी या प्रोन्नति/सैकेण्डमैंट/स्थानान्तरण द्वारा और विभिन्न पद्धतियों द्वारा भरे जाने वाले पद (पदों) की प्रतिशतता.--शत प्रतिशत प्रोन्नति द्वारा।

11. प्रोन्नति/सैकेण्डमैंट/स्थानान्तरण द्वारा भर्ती की दशा में वे श्रेणियां (ग्रेड) जिनसे प्रोन्नति/ सैकेण्डमैंट/स्थानान्तरण किया जाएगा.--जिल्दसाज, रेस्टोरर और केयर टेकर में से प्रोन्नति द्वारा जिनका कम-से-कम तीन वर्ष का नियमित सेवाकाल या अपने-अपने ग्रेड में की गई लगातार तदर्थ सेवा, यदि कोई हो, को सम्मिलित करके नियमित सेवाकाल हो।

परन्तु प्रोन्नति के प्रयोजन के लिए समस्त पात्र कर्मचारियों की, जो जिल्दसाज, रेस्टोरर और केयर टेकर के पद पर कार्यरत हैं, उनके अपने-अपने संवर्ग में, सेवाकाल के आधार पर वरिष्ठता को छेडे़ बिना संयुक्त वरिष्ठता सूची तैयार की जाएगी।2457 राजपत्र, हिमाचल प्रदेश, 04 जुलाई 2022/13 आषाढ़, 1944 प्रोन्नति के सभी मामलों में नियमित नियुक्ति से पूर्व सम्भरक पद में की गई लगातार तदर्थ सेवा, यदि कोई हो, इन नियमों में यथाविहित सेवाकाल के लिए, इस शर्त के अध्यधीन प्रोन्नति के लिए गणना में ली जाएगी, कि सम्भरक प्रवर्ग में तदर्थ नियुक्ति/प्रोन्नति, भर्ती और प्रोन्नति नियमों के उपबन्धों के अनुसार चयन की उचित स्वीकार्य प्रक्रिया को अपनाने के पश्चात्‌ की गई थीः

(i) परन्तु उन सभी मामलों में जिनमें कोई कनिष्ठ व्यक्ति सम्भरक पद में अपने कुल सेवाकाल (तदर्थ आधार पर की गई सेवा सहित, जो नियमित सेवा/नियुक्ति के अनुसरण में हो) के आधार पर उपर्युक्त निर्दिष्ट उपबन्धों के कारण विचार किए जाने का पात्र हो जाता है, वहां उससे वरिष्ठ सभी व्यक्ति अपने-अपने प्रवर्ग/ पद/काडर में विचार किए जाने के पात्र समझे जाएंगे और विचार करते समय कनिष्ठ व्यक्ति से ऊपर रखे जाएंगेः

परन्तु यह और कि उन सभी पदधारियों की, जिन पर प्रोन्नति के लिए विचार किया जाना है, की कम से कम तीन वर्ष की न्यूनतम अर्हता सेवा या पद के भर्ती और प्रोन्नति नियमों में विहित सेवा, जो भी कम हो, होगीः

परन्तु यह और भी की जहां कोई व्यक्ति पूर्वगामी परन्तुक की अपेक्षाओं के कारण प्रोन्नति किए जाने सम्बन्धी विचार के लिए अपात्र हो जाता है, वहां उससे कनिष्ठ व्यक्ति भी ऐसी प्रोन्नति के विचार के लिए अपात्र समझा जाएगा/समझे जाएंगे।

स्पष्टीकरणः-अंतिम परन्तुक के अन्तर्गत कनिष्ठ पदधारी प्रोन्नति के लिए अपात्र नहीं समझा जाएगा यदि वरिष्ठ अपात्र व्यक्ति भूतपूर्व सैनिक है जिसने आपातकाल के दौरान सशस्त्र बल में कार्य ग्रहण किया है और जिसे डिमोबिलाइज्ड आर्मडफोर्सिज परसोनल (रिजर्वेशन ऑफ वैकेन्सीज इन हिमाचल स्टेट नॉन टैक्नीकल सर्विसिज) रुल्ज, 1972 के नियम-3 के उपबन्धों के अन्तर्गत भर्ती किया गया है और तद्‌धीन वरीयता लाभ दिए गए हों या जिसे एक्स-सर्विसमैन (रिजर्वेशन ऑफ वैकेन्सीज इन दी हिमाचल प्रदेश टैक्नीकल सर्विसिज) रुल्ज, 1985 के नियम-3 के उपबन्धों के अन्तर्गत भर्ती किया गया हो और तद्‌धीन वरीयता लाभ दिए गए हों।

इसी प्रकार, स्थायीकरण के सभी मामलों में ऐसे पद पर नियमित नियुक्ति से पूर्व सम्भरक (पोषक) पद पर की गई लगातार तदर्थ सेवा, यदि कोई हो, सेवाकाल के लिए गणना में ली जाएगी, यदि तदर्थ नियुक्ति/ प्रोन्नति, उचित चयन के पश्चात्‌ और भर्ती और प्रोन्नति नियमों के उपबन्धों के अनुसार की गई थीः परन्तु की गई तदर्थ सेवा को गणना में लेने के पश्चात्‌ स्थायीकरण के फलस्वरुप पारस्परिक वरीयता अपरिवर्तित रहेगी।

12. यदि विभागीय प्रोन्नति समिति/विभागीय स्थायीकरण समिति विद्यमान हो, तो उसकी संरचना.--(क) विभागीय प्रोन्नति समिति.--जैसी सरकार द्वारा समय-समय पर गठित की जाए। (ख) विभागीय स्थायीकरण समिति.--लागू नहीं।

13. भर्ती करने में किन परिस्थितियों में हिमाचल प्रदेश लोक सेवा आयोग से परामर्श किया जाएगा.--जैसा विधि द्वारा अपेक्षित हो।

14. सीधी भर्ती के लिए अनिवार्य अपेक्षा.--लागू नहीं।

15. सीधी भर्ती किए जाने वाले व्यक्तियों के लिए अनिवार्य अपेक्षा.--लागू नहीं।

16. आरक्षण.--सेवा में नियुक्ति, हिमाचल प्रदेश सरकार द्वारा समय-समय पर अनुसूचित जातियों/ अनुसूचित जनजातियों/अन्य पिछडे़ वर्गों और अन्य प्रवर्ग के व्यक्तियों के लिए सेवा में आरक्षण की बाबत जारी किए गए आदेशों के अध्यधीन होगी।2458 राजपत्र, हिमाचल प्रदेश, 04 जुलाई 2022/13 आषाढ़, 1944 17. विभागीय परीक्षा.--लागू नहीं। 18. शिथिल करने की शक्ति.--जहां राज्य सरकार की यह राय हो कि ऐसा करना आवश्यक या समीचीन है, वहां यह, कारणों को लिखित में अभिलिखित करके, और हिमाचल प्रदेश लोक सेवा आयोग के परामर्श से, आदेश द्वारा, इन नियमों के किसी उपबन्ध (उपबन्धों) को किसी वर्ग या व्यक्ति (व्यक्तियों) के प्रवर्ग या पद (पदों) की बावत, शिथिल कर सकेगी।

No. Per (SAS-I)A(3)-4/2021.—In exercise of the powers conferred by proviso to Article 309 of the Constitution of India, the Governor of Himachal Pradesh, is pleased to make the Recruitment and Promotion Rules for the post of Cash & Bill Messenger, Class-III (Non-Gazetted) in the Department of Personnel (Secretariat Administration Services), Himachal Pradesh, as per Annexure-"A" attached with this notification, namely:—

1. Short title and Commencement.—(1) These rules may be called the Himachal Pradesh, Department of Personnel (Secretariat Administration Services) Cash & Bill Messenger, Class-III (Non Gazetted), Recruitment and Promotion Rules, 2022

(2) These rules shall come into force from the date of publication in the Rajpatra (e-Gazette), Himachal Pradesh.

2. Repeal & Savings.—(1) The Himachal Pradesh, Department of Personnel (Secretariat Administration Services) Cash & Bill Messenger (Class-III, Non Gazetted) Recruitment and Promotion Rules-1996 notified vide notification No. Per (SAS)-A-A(3)-1/95 dated 25th July, 1996, are hereby repealed.

(2) Notwithstanding such repeal, any appointment made or anything done or any action taken under the rules, so repealed under sub rule 2(1) supra, shall be deemed to have been validly made, done or taken under these rules.

RECRUITMENT AND PROMOTION RULES FOR THE POST OF CASH AND BILL

MESSENGER, CLASS-III (NON-GAZETTED) IN THE DEPARTMENT OF

PERSONNEL (SECRETARIAT ADMINISTRATION SERVICES),

HIMACHAL PRADESH

2. Number of post(s).—02 (Two)

3. Classification.—Class-III (Non-Gazetted) (Non-Technical Services).

4. Scale of Pay.—Pay Matrix Level-06 (Rs. 25,600-81200) of HP Civil Services (Revised Pay) Rules, 2022.

5. Whether "Selection" post or "Non-Selection" post.—Non-Selection

6. Age for direct recruitment.—Not applicable

7. Minimum Educational and other qualifications required for direct recruit(s).— Not applicable

8. Whether age and educational qualification(s) prescribed for direct recruit(s) will

apply in the case of the promotee(s).—(a) Age:—Not applicable.

(b) Educational qualification.—Not applicable

10. Method of recruitment, whether by direct recruitment or by promotion/

secondment/transfer and the percentage of post(s) to be filled in by various methods.—100%

In case of recruitment by promotion/secondment/transfer, grade(s) from which promotion/ secondment/transfer is to be made.—By promotion from amongst the Book Binders, Restorers and Care-takers who possess atleast three years regular service or regular combined with continuous adhoc service, if any, in the respective grade :

Provided that for the purpose of promotion, a combined seniority list of all the eligible officials holding the posts each of Book Binders, Restorers and Care-takers respectively without disturbing their cadre wise inter-se-seniority shall be prepared.

In all cases of promotion, the continuous adhoc service rendered in the feeder post if any, prior to regular appointment to the post shall be taken into account towards the length of service as prescribed in these rules for promotion subject to the conditions that the adhoc appointment/ promotion in the feeder category had been made after following proper acceptable process of selection in accordance with the provisions of R & P Rules:

(i) Provided that in all cases where a junior person becomes eligible for consideration by virtue of his total length of service (including the service rendered on adhoc basis followed by regular service/appointment) in the feeder post in view of the provisions referred to above, all persons senior to him in the respective category/post/cadre shall be deemed to be eligible for consideration and placed above the junior person in the field of consideration:

Provided that all incumbents to be considered for promotion shall possess the minimum qualifying service of at least three years or that prescribed in the Recruitment & Promotion Rules for the post, whichever is less:

Provided further that where a person becomes ineligible to be considered for promotion on account of the requirements of the preceding proviso, the person(s) junior to him shall also be deemed to be ineligible for consideration for such promotion.

Explanation:—The last proviso shall not render the junior incumbents ineligible for consideration for promotion if the senior ineligible persons happened to be Ex-Servicemen who have joined armed forces during the period of emergency and recruited under the provisions of Rule-3 of Demobilized Armed Forces Personnel (Reservation of vacancies in Himachal State Non- Technical Service) Rules, 1972 and having been given the benefit of seniority thereunder or recruited under the provision of Rule-3 of Ex-Servicemen (Reservation of vacancies in the Himachal Pradesh Technical Service) Rules, 1985 as amended from time to time and having been given the benefit of seniority thereunder.

Similarly, in all cases of confirmation, continuous adhoc service rendered on the feeder post if any, prior to the regular appointment against such posts shall be taken into account towards the length of service, if the adhoc appointment/promotion had been made after proper selection and in accordance with the provision of the Recruitment & Promotion Rules:

Provided that inter-se-seniority as a result of confirmation after taking into account, adhoc service rendered shall remain unchanged.

12. If a Departmental Promotion Committee/Departmental Confirmation Committee exists, what is its composition?.—(a) Departmental Promotion Committee:—As may be constituted by the Government from time to time.

(b) Departmental Confirmation Committee.—Not applicable.

13. Circumstances under which the Himachal Pradesh Public Service Commission

(HPPSC) is to be consulted in making recruitment.—As required under the law.

14. Essential requirement for a direct recruitment.—Not applicable.

15. Selection for appointment to the post by direct recruitment.—Not applicable.

16. Reservation.—The appointment to the service shall be subject to orders regarding reservation in the service for Scheduled Castes/Scheduled Tribes/Other Backward Classes/other categories of persons issued by the Himachal Pradesh Government from time to time.

17. Departmental Examination.—Not applicable.

18. Power to Relax.—Where the State Government is of the opinion that it is necessary or expedient to do so, it may, by order for reasons to be recorded in writing and in consultation with the Himachal Pradesh Public Service Commission relax any of the provision(s) of these rules with respect to any class or category of person(s) or post(s).

No. HFW-B(B)2-9-2021-VOL-I-Loose.—The Governor, Himachal Pradesh is pleased to constitute a Screening Committee to examine the eligibility of DNB as well as PG Medical Officers (Specialist) of the State for appointment/promotion to the faculty post in the Government Medical/Dental Colleges/institutions of the State with immediate effect, in the public interest, in following manner:—

The Committee shall examine the requests/ representations received from Medical Officers (Specialist)/faculty members of the State strictly on the basis of prevailing R&P rules and NMC norms and after examining all the aspects will make recommendations to the Government accordingly.

विषय.--दरखास्त बराये कागजात माल में नाम की दरुस्ती बारे।

श्रीमती कान्ता देवी पुत्री स्व0 श्री घमीरू पुत्र श्री नोई हाल पत्नी श्री नाथु राम, निवासी गांव उच्च, डाकघर बरशैणी फाटी मनीकर्ण, कोठी कनावर उप-तहसील जरी, जिला कुल्लू, हि0 प्र0 द्वारा दिनांक 23-05-2022 को इस अदालत में प्रार्थना-पत्र पेश किया है कि उसका नाम सहबन गलती से फाटी मणीकर्ण कोठी कनावर के राजस्व दस्तावेज में श्रीमती कान्ता देवी पुत्री श्री घमीरू की जगह भीमावती पुत्री श्री घमीरू दर्ज हुआ है जो कि गलत इन्द्राज हुआ है। अब प्रार्थी अराजी हजा के इन्द्राज में अपना नाम दुरुस्त करके श्रीमती भीमावती उर्फ कान्ता देवी पुत्री घमीरू दर्ज करना चाहती है।

अतः सर्वसाधारण को इस इश्तहार द्वारा सूचित किया जाता है कि यदि किसी को उपरोक्त प्रार्थीन के के नाम की दुरुस्ती का इन्द्राज करने बारे कोई एतराज हो तो वह दिनांक 15-07-2022 को सुबह 10.00 बजे या इससे पूर्व असालतन या वकालतन हाजिर अदालत आकर अपना एतराज दर्ज करवा सकता है।2469 राजपत्र, हिमाचल प्रदेश, 04 जुलाई 2022/13 आषाढ़, 1944 इसके उपरान्त कोई भी एतराज समायत न होगा तथा नियमानुसार नाम दुरुस्ती का इन्द्राज करने के आदेश पारित कर दिए जाएंगे।

आज दिनांक 20-06-2022 को मेरे हस्ताक्षर व मोहर अदालत द्वारा जारी हुआ।

I, Poonam Kumari w/o Sh. Vijay Kumar, r/o Village Tiar, P.O. Khurwain, Tehsil Bangana, District Una (H.P.) declare that in my daughter Vanshika Sharma's certificate my name wrongly entered as Poonam Devi instead of Poonam Kumari. Hence, in my all documents my correct name is Poonam Kumari. So that, in future understood my correct name Poonam Kumari instead of Poonam Devi. Kindly note all.
