ആലപ്പുഴ ഗവ. റ്റി.ഡി. മെഡിക്കൽ കോളേജ് ആശുപത്രി കോമ്പൗ≠ിൽ ജെ.1 കെട്ടിടത്തിന്റെ വടക്കു കിഴക്കു വശം സി.റ്റി. സ്കാൻ സെന്ററിന് പടിഞ്ഞാറ് ഭാഗത്തായി സ്ഥിതി ചെയ്യുന്ന കോഫീഷോപ്പ് (ആകെ 417 ചതുരശ്ര അടി) ര≠് വർഷത്തേക്കുള്ള നടത്തിപ്പിനായി താഴെ പറയുന്ന നിബന്ധനകളോടുകൂടി മൽസരാധിഷ്ഠിത ഇ-ടെ≠ർ ക്ഷണിക്കുന്നു. വിശദവിവരങ്ങൾ ആലപ്പുഴ ഗവ. റ്റി.ഡി. മെഡിക്കൽ കോളേജ് സൂപ്ര≠് ഓഫീസിൽ നിന്നും https://etenders.kerala.gov.in എന്ന വെബ്സൈറ്റിൽ നിന്നും അറിയാവുന്നതാണ്.

1. ഇ-ടെ≠റിന്റെ ഉദ്ദേശമൂല്യം ` 47,85,000 (നാൽപ്പത്തിയേഴ് ലക്ഷത്തി എൺപത്തായ്യായിരം രൂപ മാത്രം).

2. ഇ-ടെ≠ർ സമർപ്പിക്കുവാനുള്ള ഫീസ് ` 7,200+18% ജി.എസ്.റ്റി. ` 1,296 (ആകെ ` 8,496).

3. ഇ-ടെ≠ർ WWW.etenders.kerala.gov.in എന്ന വെബ്സൈറ്റിലൂടെ ഓൺലൈനായി മാത്രം സമർപ്പിക്കേ≠താണ്.

4. ഇ-ടെ≠ർ സമർപ്പിക്കേ≠ അവസാന തീയതി—

5. ഇ-ടെ≠ർ തുറക്കുന്ന തീയതി—10-3-2020.

6. ഇ-ടെ≠റിൽ രേഖപ്പെടുത്തുന്ന നിരക്കിന് പുറമെ ജി.എസ്.റ്റി. നൽകാൻ കരാറുകാരൻ ബാദ്ധ്യസ്ഥൻ ആയിരിക്കും. കൂടാതെ വാടകയിനത്തിൽ നൽകുവാൻ രേഖപ്പെടുത്തുന്ന തുകയുടെ 1% നിരതദ്രവ്യം അടയ്ക്കേ≠തുമാണ്.

7. കോഫീഷോപ്പിന് ആവശ്യമായ പാത്രങ്ങളും ഫർണിച്ചറും സ്വന്തം ചെലവിൽ ഉൾപ്പെടുത്തേ≠ താണ്.

8. കോഫീഷോപ്പിലു≠ാകുന്ന ആഹാരാവശിഷ്ടങ്ങളും മാലിന്യങ്ങളും സ്വന്തം ചെലവിൽ ആശുപത്രി കോമ്പൗ≠ിൽ നിന്നും നീക്കം ചെയ്യേ≠താണ്.

നീക്കം ചെയ്യുന്ന രീതി ദർഘാസിൽ Upload ചെയ്യേ≠താണ്. യാതൊരു കാരണവശാലും മാലിന്യ സംസ്കരണം ആശുപത്രി കോമ്പൗ≠ിൽ അനുവദിക്കുന്നതല്ല.

9. കോഫീഷോപ്പിൽ വെൻഡിംഗ് മെഷീൻ ഉപയോഗിച്ചുള്ള ചായ, കാപ്പി എന്നിവ കൂടാതെ കുപ്പിവെള്ളം, ലഘുപാനീയങ്ങൾ, ലഘുഭക്ഷണങ്ങൾ, ബിസ്ക്കറ്റ്, പായ്ക്കറ്റിലുള്ള മറ്റ് ഭക്ഷണ പദാർ∞ങ്ങൾ എന്നിവയുടെ വിപണനം മാത്രമേ നടത്താവൂ. ഇത്തരം ഭക്ഷണ സാധനങ്ങളുടെ വില നി›യിച്ച് ഉറപ്പാക്കേ ≠തു≠്. മറ്റ് സാധനസാമഗ്രികളുടെ വിപണനം യാതൊരു കാരണവശാലും അനുവദിക്കുന്നതല്ല. കൂടാതെ യാതൊരുവിധ പാചകവും വെൻഡിംഗ് ഷോപ്പിൽ അനുവദിക്കുന്നതല്ല.

10.ജീവനക്കാരും പൊതുജനങ്ങളും കാണത്തക്ക വിധത്തിൽ വിലനിലവാരം പ്രദർശിപ്പിക്കേ≠താണ്.

11. കരാറുകാർക്ക് കേരളത്തിലെ നിയമാനുസൃത അധികാര കേന്ദ്രങ്ങൾ നിഷ്ക്കർഷിക്കുന്ന ലൈസൻസുകൾ ഉ≠ായിരിക്കേ≠തും കരാർ ഉറപ്പിച്ച് 30 ദിവസത്തിനകം താഴെപ്പറയുന്ന പ്രമാണങ്ങൾ ഓഫീസിൽ സമർപ്പിക്കേ≠തുമാണ്.

(a) FSS Act 2006 പ്രകാരം Food & Safety Standards Authority of India നൽകുന്ന ലൈസൻസ്.

(b) GST Registration Certificate.

(c) കോഫീഷോപ്പ് കരാറുമായി ബന്ധപ്പെട്ട ഇനത്തിൽ ഇടപാടുകൾക്ക് ഉപയോഗിക്കുന്ന ബാങ്ക് അക്കൗ≠് വിവരം (സ്ഥാപനത്തിന്റെയോ ഉടമയുടേയോ).

(d) ആദായ നികുതി വകുപ്പിന്റെ പാൻകാർഡ് (സ്ഥാപനത്തിന്റെയോ ഉടമയുടേയോ).

12. കരാറുകാരനെ സർക്കാർ/പൊതുമേഖലാ/സ്വയംഭരണ/ ആര്യോഗ്യ സ്ഥാപനങ്ങളോ കരിമ്പട്ടികയിൽ ഉൾപ്പെടു ത്തിയതാവരുത്. ഇപ്രകാരം കരിമ്പട്ടികയിൽ ഉൾപ്പെട്ടിട്ടില്ല എന്ന് ഇതോടൊപ്പമുള്ള അനുബന്ധം എ-യിൽ കയ്യൊപ്പ് പതിച്ച് Upload ചെയ്യേ≠താണ്.

13. കോഫീഷോപ്പ് നടത്തുന്നതിനുള്ള എല്ലാ ചെലവുകളും കരാറുകാരൻ വഹിക്കേ≠താണ്.

മോഷണം, പൊള്ളൽ, തീപിടുത്തം, വൈദ്യുതാഘാതം എന്നിവമൂലം കോഫീഷോപ്പ് ജീവനക്കാർക്ക് ജോലി സമയത്ത് ഉ≠ാകുന്ന യാതൊരു കഷ്ടനഷ്ടങ്ങൾക്കും ആശുപത്രി അധികൃതർ ഉത്തരവാദികൾ ആയിരിക്കില്ല.

14. കോഫീഷോപ്പ് നടത്തിപ്പിനല്ലാതെ മറ്റ് യാതൊരാ വശ്യങ്ങൾക്കും അനുവദിച്ചിരിക്കുന്ന സ്ഥലം ഉപയോഗിച്ചുകൂടാ.

15. കരാറുകാരന് കോഫീഷോപ്പ് നടത്തുന്നതിനുള്ള ബന്ധപ്പെട്ട അധികാര സ്ഥാപനങ്ങളിൽ നിന്നുള്ള നിയമപ്രകാരമുള്ള എല്ലാ ലൈസൻസുകളും ഉ≠ായി രിക്കേ≠താണ്.

16. കോഫീഷോപ്പിൽ വിതരണം ചെയ്യുന്ന ഭക്ഷണം മെച്ചപ്പെട്ട ഗുണനിലവാരം ഉറപ്പ് വരുത്തേ≠തും ഇത്തരത്തിലു≠ാകുന്ന യാതൊരു പരാതിയും ഭക്ഷണത്തിൽ മായം ചേർക്കുന്നതുൾപ്പെടെയുള്ള സംഭവങ്ങൾ ഉ≠ായാൽ സ്ഥാപനത്തെ കരിമ്പട്ടികയിൽ ഉൾപ്പെടുത്തുന്ന തടക്കമുള്ള നടപടികൾ എടുക്കുന്നതായിരിക്കും.

17. ആശുപത്രിയും പരിസരവും "ലഹരി വിമു‡ പ്രദേശം'' ആയതിനാൽ പുകവലി, മദ്യം, മറ്റ് ലഹരി വസ്തുക്കൾ എന്നിവയുടെ വിൽപ്പനയും ഉപയോഗവും നിരോധിച്ചിരിക്കുന്നു.

18. സൂപ്ര≠് നിയോഗിച്ചിട്ടുള്ള ഉദ്യോഗസ്ഥർ കോഫീഷോപ്പിൽ മിന്നൽ പരിശോധന നടത്തുന്ന തായിരിക്കും.

19. ഉപഭോ‡ാക്കളും ആശുപത്രി ജീവനക്കാരും കാണത്തക്ക വിധത്തിൽ Suggestion Box സ്ഥാപിക്കേ≠താണ്. 20. കോഫീഷോപ്പിനുള്ളിൽ Fire Extinguisher സ്ഥാപിക്കേ≠താണ്.

21. കരാറുകാരൻ യാതൊരു കാരണവശാലും കരാർ പൂർണ്ണമായോ ഭാഗികമായോ മറ്റൊരാൾക്ക് മാറ്റി നൽകുവാൻ പാടില്ല. ഇതിന് വിരുദ്ധമായി ബോധ്യപ്പെട്ടാൽ ആശുപത്രിയുടെ ഭാഗത്ത് നിന്നും കരാറുകാരനെതിരായി കരാർ അവസാനിപ്പിക്കുന്ന തടക്കമുള്ള നടപടികൾ സ്വീകരിക്കുന്നതാണ്.

22. കരാറുകാരൻ കെ. എസ്. ഇ. ബി.-യുടെ നോട്ടീസ് പ്രകാരമുള്ള വൈദ്യുതി ചാർ÷് കൃത്യമായി കെ. എസ്. ഇ. ബി. ഓഫീസിൽ ഒടുക്കി രസീത് ഹാജരാക്കേ≠താണ്.

23. ടെ≠ർ ഉറപ്പിച്ച് ലഭിക്കുന്ന കരാറുകാരൻ മൂന്ന് മാസത്തെ വാടക ഡിമാന്റ ് ഡ്രാഫ്റ്റ് മുഖേന സെക്യൂരിറ്റി ഡെപ്പോസിറ്റായി ആശുപത്രി വികസന സൊസൈറ്റി ഓഫീസിൽ അടച്ച് രസീത് വാങ്ങേ≠താണ്. പ്രതിമാസ വാടക എല്ലാ മാസവും അഞ്ചാം തീയതിക്ക് മുമ്പായി ഡിമാന്റ ് ഡ്രാഫ്റ്റായി അടയ്ക്കേ≠താണ്. പ്രതിമാസ വാടകയിനത്തിൽ കുടിശ്ശിഖ വന്നാൽ യാതൊരു അറിയിപ്പും കൂടാതെ കരാർ അവസാനിപ്പിക്കുന്നതായിരിക്കും.

24. മതിയായ കാരണത്താൽ ഒരു മാസത്തെ അറിയിപ്പ് നൽകിക്കൊ≠ും ഗുരുതരമായ കാര്യങ്ങളു≠ായാൽ ഉടനടി സ്ഥലവും നടത്തിപ്പ് അവകാശങ്ങളും തിരിച്ചെടുക്കുന്നതിനും സൂപ്ര≠ിന് പൂർണ്ണ അധികാരമു≠ായിരിക്കും.

25. അവധി ദിവസങ്ങൾ ഉൾപ്പെടെ എല്ലാ ദിവസവും കോഫീഷോപ്പ് പ്രവർത്തിപ്പിക്കേ≠താണ്. കുറഞ്ഞത് രാവിലെ 5.30 മുതൽ രാത്രി 9.30 വരെയെങ്കിലും കോഫീഷോപ്പ് പ്രവർത്തിപ്പിച്ചിരിക്കണം.

26. കോഫീഷോപ്പ് ജീവനക്കാർ പ്രായപൂത്തിയായവരും ഈ രംഗത്ത് വൈദഗ്ദ്ധ്യമുള്ളവരുമായിരിക്കണം. ബാലവേല അനുവദിക്കുന്നതല്ല, ശ്രദ്ധയിൽപ്പെട്ടാൽ കരാർ അവസാനിപ്പിക്കുന്നതായിരിക്കും.

27. കോഫീഷോപ്പ് ജീവനക്കാരുടെ കാര്യത്തിൽ തൊഴിൽ നിയമം അനുശാസിക്കുന്ന എല്ലാ ചട്ടങ്ങളും നിയമങ്ങളും, കാലാകാലങ്ങളിലു≠ാകുന്ന ഭേദഗതികളും കരാറുകാരൻ നിർബന്ധമായും പാലിക്കേ≠താണ്. ഇത്തരത്തിലു ≠ാകുന്ന തർക്കങ്ങൾക്ക് ആശുപത്രി അധികൃതർ കക്ഷികളാവുന്നതോ, ഉത്തരവാദികളാവുന്നതോ അല്ല.

28. തെരഞ്ഞെടുക്കപ്പെടുന്ന സ്ഥാപനങ്ങളുമായുള്ള കരാർ, കരാർ ആരംഭിക്കുന്ന തീയതി മുതൽ ര≠് വർഷത്തേക്കോ പുതിയ ടെ≠ർ നിലവിൽ വരുന്നതു വരെയോ ഏതാണോ അവസാനം അതുവരെയായിരിക്കും. എന്നിരുന്നാലും യാതൊരു മുന്നറിയിപ്പും കൂടാതെ കരാർ എപ്പോൾ വേണമെങ്കിലും അവസാനിപ്പിക്കുന്ന തിനുള്ള അധികാരം സൂപ്ര≠ിൽ നിക്ഷിപ്തമായിരിക്കും.

29. കോഫീഷോപ്പ് നടത്തിപ്പുമായി ബന്ധപ്പെട്ട് ആശുപത്രിക്ക് ഉ≠ാകുന്ന എല്ലാവിധ കഷ്ടനഷ്ട ങ്ങൾക്കും കരാറുകാരൻ ഉത്തരവാദി ആയിരിക്കുന്നതും കരാറുകാരന് എതിരായി സിവിൽ/ക്രിമിനൽ നടപടികൾ കൈക്കൊള്ളുന്നതും റവന്യൂ റിക്കവറി നടപടികൾ സ്വീകരിക്കുന്നതുമായിരിക്കും.

30. ഇ-ടെ≠ർ നോട്ടീസ് പ്രകാരമുള്ള വ്യവസ്ഥകൾ അംഗീകരിച്ച് കരാർ ഒപ്പ് വെയ്ക്കേ≠ ചുമതല പൂർണ്ണമായും കരാറുകാരനിൽ മാത്രമായിരിക്കും.

31. ഈ കരാർ പ്രാഥമികമായും കരാർ ഉറപ്പിക്കുന്ന തീയതി മുതൽ ഒരു വർഷത്തേക്ക് മാത്രമായി രിക്കുന്നതും ഈ കാലയളവിലെ പ്രവർത്തന മികവ് കണക്കാക്കി തുടർന്ന് ഒരു വർഷത്തേക്കുകൂടി നൽകുന്നതുമായിരിക്കും. പ്രവർത്തനം തൃപ്തികര മല്ലെങ്കിൽ കരാർ അവസാനിപ്പിക്കുന്നതായിരിക്കും.

ഇ-ടെ≠റിനോടൊപ്പം ` 500-ന്റെ കേരള സ്റ്റാമ്പ് പേപ്പറിൽ പ്രാഥമിക സമ്മതപത്രം എഴുതി Upload ചെയ്യേ≠തും രേഖപ്പെടുത്തുന്ന നിരക്കിന്റെ 1% നിരതദ്രവ്യം ഓൺലൈനായി അടയ്ക്കേ≠തും അനുമതി ലഭിച്ച കരാറുകാരൻ ` 500-ന്റെ കേരള സ്റ്റാമ്പ് പേപ്പറിൽ വ്യവസ്ഥകൾ അംഗീകരിച്ച് കോഫീഷോപ്പ് നടത്താമെന്നുള്ള സമ്മതപത്രം എഴുതി നൽകേ≠തുമാണ്.

എല്ലാ ഇ-ടെ≠റുകൾക്കും ബാധകമായ നിയമങ്ങൾ ഇതിൽ ഉൾപ്പെട്ടിട്ടില്ലെങ്കിൽ കൂടി ഇതിനും ബാധകമായിരിക്കും.

ഇ-ടെ≠ർ സമർപ്പിക്കുന്നതിനും ടെ≠റിന്റെ ക്രിറ്റിക്കൽ തീയതികൾ അറിയുന്നതിനും www.etenders.kerala.gov.in എന്ന വെബ്സൈറ്റ് സന്ദർശിക്കുക. ഏതെങ്കിലും ടെ≠ർ നിരസിക്കുന്നതിനോ സ്വീകരിക്കുന്നതിനോ ഉള്ള അധികാരം ആലപ്പുഴ ഗവ. റ്റി. ഡി. മെഡിക്കൽ കോളേജ് ആശുപത്രി സൂപ്ര≠ിൽ നിക്ഷിപ്തമായിരിക്കും. കൂടുതൽ വിവരങ്ങൾ പ്രവർത്തിദിവസങ്ങളിൽ സൂപ്ര≠് ഓഫീസിൽ നിന്നും ലഭിക്കുന്നതാണ്.

മേൽപ്പറഞ്ഞ നിബന്ധനകൾ പാലിക്കാത്ത ടെ≠റുകൾ യാതൊരു അറിയിപ്പും കൂടാതെ നിരസിക്കുന്നതായിരിക്കും.

ടെ≠ർ സംബന്ധിച്ച എല്ലാ നിയമ പ്രശ്നങ്ങളും ആലപ്പുഴ ജൂറിസ്ഡിക്ഷൻ പരിധിയിലായിരിക്കും.

The Superintendent, Govt. T.D. Medical College Hospital, Alappuzha invites online competitive bid for providing service of running coffee shop for a period of two years which is situated in the North West side of J1 block and Wester n side of CT Scan centre which contain a total area of 417sq.ft. The contract will be awarded as per the below noted criteria.

1. Estimated cost of e-tender—` 47,85,000 (Rupees forty seven lakh and eighty five thousand only).

2. Tender submission fee—` 7,200 + 18% GST ` 1,296, Total ` 8,496.

3. Mode of submission of tender—Tender should be submitted online in www.etenders.kerala.gov.in 4. Last date for e-Tender submission—5-3-2020.

5. e-Tender opening date—10-3-2020.

6. lndenting tenderers should be liable to pay GST against the rate quoted in e-tender in all stages of the deed. Moreover should remit Earnest Money Deposit equal to 1% of the rate quoted as coffee shop rent.

7. The service provider has to provide by himself all the furniture and utensils required for running of coffee shop at his own cost.

8. The service provider will be responsible for removing the food waste and other waste generated in the coffee shop from the hospital compound at his own cost. Mode of waste disposal should be detailed and uploaded in the e-tender. At any cost waste disposal will not be permitted in the hospital compound.

9. Coffe shop shall be meant for serving tea and coffee which are made using wending machine, bottled water, bottled drinks, buscuits, snacks, other packed food items etc. Such other items and such prices as may be settled between the service provider and the hospital authorities. The service provider shall not be entitled to sale any other items other than packed food. In addition to this cooking is not allowed in the coffee shop.

10. The service provider should display a price list of every item in a prominent place of coffee shop.

11. The service provider should have all valid license issued by statutory authorities in Kerala. The documentary proof should be submitted within 30 days by the successful tenderers as detailed below:

(a) License issued by Food and Safety standard authority of India under FSS Act, 2006.

(b) GST registration certificate

(c) The bank account details (the firm or proprietor) which will be used in this transaction

(d) Income Tax department PAN card of the firm or proprietor should be attached.

12. The tenderer should not have been black listed by any government/public sector undertaking/medical institution etc. The bidders should furnish an undertaking to this effect as per Annexure A attached to this duly signed and uploaded.

13. The service provider shall bear all expenses for running the coffee shop and the hospital shall not in any manner liable for any damage caused on incidents like theft, burn, fire, electric shock etc. or bear any compensation for damage or injury caused to the workmen during discharging of their duties.

14. The service provider shall not be entitled to use the allotted space for any other purpose other than running of coffee shop.

15. The service provider must possess all the requisite valid trade and food license issued by the competent authority.

16. There shall be no compromise on the quality of food supplied by the service provider. If any such incidents or food adulteration is found action will be taken including black listing the firm.

17. The hospital campus is "tobacco and alcohol free zone" hence sale and the use of tobacco and alcohol is strictly prohibited in coffee shop area.

18. Surprise inspection will be conducted by officers/ inspection team constituted by superintendent.

19. Suggestion box should be made available to customers of coffee shop at an accessable area.

20. Fire extinguisher must be kept in the coffee shop.

21. The contract assigned by the bidder shall not be let either in full or in partial to others under any circumstances. If anything is found to the contrarary action as deemed fit will be initiated against the contractor including cancellation of the contract.

22. The contractor should remit the electricity charge as per the notice issued by the KSEB authorities and a copy of the payment receipt should be submitted to this office.

23. The service provider for whom the tender is awarded should remit three months rent in advance by way of demand draft as security deposit. The receipt for the same will be issued from the HDS office. Monthly rent must be remitted before 5th day of every month by way of demand draft in the name of superintendent, GTDMCH Alappuzha. There shall no compromise for delay in the payment of monthly rent and occurrence if any, the contract shall be cancelled without further notice.

24. In case of any default by the service provider the Superintendent has the right to terminate the contract by giving one month notice and immediate action will be taken to terminate the contract in case of any serious valid reason without giving previous notice.

25. The coffee shop will run on all days including Sundays and Holidays and shall remain open atleast 5.30 am to 9.30 pm.

26. The service provider must employee adult and skilled labourer only. Employment of child labour will leave to the termination of contract.

28. The period of contract will be two years from the date of execution of agreement or till the new tender is inforce or termination of existing contract whichever is last. Eventhough the Superintendent has the right to cancel the agreement without further notice.

29. The service provider shall be liable for any damage or loss caused to hospital. Occurrence if any Civil/ Criminal procedures will be initiated. If any amount found to be liable the hospital authorities have the right to recover such amount from the contractor as per the provisions of Revenue Recovery Act.

30.The service provider is to be entirely responsible for the execution of the contract in all respects in accordance with the terms and conditions as specified in the acceptance of tender.

31.The contract is awarded, shall be valid initially for a period of one year from the date of award of contract. Further extension of one year will be subject to continuous satisfactory performance.

Failure on this aspects by the service provider, the competent authority will reserve the right to terminate the contract.

e-tender should be accompanied by a preliminary agreement in Kerala Stamp Paper worth ` 500 and should remit Earnest Money Deposit @ 1% of the rate quoted as monthly rent.

The successful tenderers should remit three months rent in advance as security deposit and should submit an agreement in Kerala Stamp Paper worth ` 500.

The rules applicable to all e-Tenders will be applicable to this e-Tender also eventhough it is not mentioned in it.
